Diabetes Self-Management Education and Support Program & Medical Nutrition Therapy

What is Diabetes Self-Management Education and Support (DSME/S)?

DSME/S is the ongoing process of providing individuals with diabetes the tools they need to improve their quality of life. A Certified Diabetes Educator will teach you how to effectively control and manage diabetes while incorporating your needs, goals and life experiences.

We focus on the “7 Self-Care Behaviors” essential for successful diabetes self-management as defined by the American Association of Diabetes Educators: 1. Healthy Eating: practical meal planning & smart

food choices 2. Being Active: keeping blood sugar, blood pressure

& stress under control with an activity you love3. Monitoring: understanding blood glucose goals to

keep your body performing at its best4. Taking Medication: safe & effective use of

recommended medications 5. Problem Solving: planning ahead & knowing how

to handle unexpected situations6. Healthy Coping: individual & community support

that help reduce the burden of diabetes care7. Reducing Risks: eliminating or lowering your

chance for diabetes complications

In individualized or group classes, our Certified Diabetes Educators:• Teach and encourage effective day to day self-

management skills• Update you on the latest treatment options • Initiate shared decision-making between you and

your health care providers• Connect you with local support programs and

community resources• Help create practical solutions for lifelong diabetes

management

Our CapitalCare DSME/S program is recognized by the American Diabetes Association for meeting its standards of excellence.

DSME is proven to positively affect diabetes control and improve outcomes. A Certified Diabetes Educator should be part of your health care team and should be seen at least once a year. Ask your provider to make a referral or call us to schedule an appointment today!

What is Medical Nutrition Therapy?

Medical Nutrition Therapy is a therapeutic approach to treating medical conditions and their associated symptoms by using specifically tailored meal plans created by a Registered Dietitian.

CapitalCare provides Medical Nutrition Therapy for all age groups. Our services include, but are not limited to, counseling and education for the following conditions:• General Nutrition and Wellness• Childhood, Adolescent and Adult obesity and Weight

Management• Prediabetes• Cholesterol Management• Hypertension (High Blood Pressure)• Eating Disorders• Food Allergy and Intolerance• Digestive Disorders• Metabolic Syndrome

We understand that following a healthy diet can be challenging. Learn the art and science of eating well, along with strategies for increasing physical activity and choices for a healthier lifestyle. Support Groups and

Specialized Diabetes Programs

Support Groups offer a wonderful opportunity to share your knowledge and experience as well as your challenges, and learn from others. We offer an Adult Support group and Insulin Pump Support Group, both of which are free of charge and open to the community.

CapitalCare offers several specialized and advanced programs:• Gestational Diabetes and Diabetes in Pregnancy• Insulin Pump Program• Continuous Glucose Monitoring (CGM)
