  • By electronically signing, the athlete/parent agrees to the following:

    Athletic Code of Conduct The athlete is pledging to abide by OHS Code of Conduct for Athletics

    and Co-Curricular Participants. The parent/guardian is acknowledging that they agree to and understand these terms.

    Random Drug Testing Policy Addition OASD School Board approved the new Random Drug Testing Policy. District Students who choose to exercise the privilege of participating

    in athletics or co-curricular activities that are defined in the Code of Conduct must agree to participate in the random suspicionless drug testing program. Student participants registering for sports or co-curricular activities will be provided with a consent form. The completion of the consent form by the student participant and, if the student is a minor, his/her parent or guardian(s), constitutes express permission and agreement that the student will submit to random suspicionless drug testing. Signing the consent, and adherence to the conditions thereafter, shall be a condition for participation in athletics and co-curricular activities.

    Emergency Medical Information All Family Access Student Emergency Contact and Health information is

    accurate and up to date.

    Concussion Agreement The Athlete/Parent Concussion information has been read and the

    importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury is understood.

    Additional Parent Information The Assumption of Risk as an athlete, Financial Responsibility for Uniforms

    and Equipment, and the need for adequate insurance coverage is understood.

  • Signature Athlete/Parent Both athlete and a parent/guardian will have to log in to Skyward Family Access

    separately to sign their portion of the Online Athletic Registration Form.

    When approved the athletic office will send a confirmation email to the athlete/parent and the athletes name will go on an approved list that will be shared electronically with coaches.

    Coaches will verify athletes eligibility to participate with the list we provide them.

  • TRYOUT GREEN Cards need to be handed to the coach before tryouts begin in May 2016. We will use the same registration process as we did this 2015-2016 school year. Because tryouts occur the year prior, we will not be collecting fees until the 2016-2017 school year.

    Athletic Transportation

    Guidelines

    1) It is an expectation that all athletes are to be transported by school transportation (bus, van) to and from away competitions.

    2) In case of a conflict IE: school or family, a Non-use of School Provided

    Transportation form must be completed at least 24 hours in advance of the date of competition. Each request requires a new signed and dated form.

    3) When dealing with a last minute request from a parent/ legal guardian at the

    event, a coach may allow the child to go with his/her parent after a Non-use of School Provided Transportation form is signed by the parent/legal guardian and given to the coach. Custody of the athlete can ONLY be granted to his/her parent or legal guardian. Each request requires a new signed and dated form.

    4) Coaches are to keep this signed form for future reference.

  • This event is a celebration and recognition of student athletes who have attained a cumulative 3.5 GPA and have completed a fall or winter sports season or are out for a spring sport. A freshman or sophomore who has participated at the varsity level and has a cumulative 3.5 GPA also qualifies as a scholar athlete.

    The Banquet is desserts onlyand is held in the OACAuditorium in the Spring.

  • Who are we and what do we do

    The Oconomowoc Sports Booster Club (OSBC) is an organization dedicated to the support of all Oconomowoc athletic programs.

    OSBC is a volunteer based organization run by a volunteer board and with the support of it members, parents, and the community.

    OSBC contributes nearly $100,000 annually to Oconomowoc athletic programs.

    Contribution requests are prioritized with direction from the Oconomowoc Athletic Director based on program needs.

  • Examples of recent contributions made by OSBC Large contributor to AFF (Athletic Fields Forever) Annual significant contributions to the PDC (Personal Development Center). Training table taping stations Ski hill time Coaching clinics Hockey ice time Tennis nets Wrestling mats Dance team choreography Batting cage Leadership retreats Track timing system Golf bags Soccer goals Cheer leading support Yard markers/trailer The list goes on and on and touches every sport

  • How do we raise funds Concession Stands: OSBC runs and operates both the indoor and outdoor

    concessions stands at OHS. We have a concession manager, and staff the stands with the volunteer support of parents with kids participating in athletic programs.

    Gold Cards: OSBC coordinates the sale of Gold Cards through the kids in our athletic programs. The cards can be used for discounts at participating community businesses, and awards are available to teams and kids based on their level of sales.

    Sports Program Guide: OSBC sells ads and publishes the Fall, Winter, and Spring Program Guide that is distributed free during the sports seasons. This guide contains pictures of the participating teams, ads from our community, schedules, and other Oconomowoc athletic historical pieces of information.

    Jack Bub Memorial Golf Outing: OSBC hosts this event annually. 2016 Date: June 17th at La Belle Golf Club

    Spirit Wear Sales, Paddle Fest Event, Winter Fest event, and more.

  • How can you help Be supportive of requests made by your team reps when asked to help staff the

    Concession Stand. This is critically important to the stand running smoothly, and raising funds for our student athletes.

    Be supportive of other OSBC fund raising events throughout the year: Buy a Gold Card and encourage friends and neighbors to do the same. Place an ad in our Sports Program Guide for your business, and support the

    businesses that place ads in our guide. Participate in our annual Jack Bub Memorial Golf Outing.

    Become a member of OSBC. Only $20 annually as a family, or $10 annually as an individual

    Free entry to annual tailgate event and football game (September 16th) Join online when registering your student athlete for a sport. Join in person at the annual tailgate event. Join through OSBC website
