For multiple samples and all same family

Step 1: Click here.Step2: Click the checkbox (to indicate all the same family)

Step 3: Enter the number of patients (samples). Step 4: Click here (GO).

Example below

1. If a DNA sample is coming from another lab, enter the original lab’s ID number, otherwise leave this blank. For example, patient samples where we prepared the DNA .

2. Select indication (Epilepsy, known microdeletion, Misc)

3 Select gender

The predigree ID numbers will be automatically filled out. Proband is a four digit number followed by “-1”. Mother has the same four digit number followed by “-2”. Father has “-3” appended. All other family members have “-4”, -5, etc.”.

You will not need the radio buttons if you fill out the form starting with proband (-1) and going down in order to father (-3) or other family members (-4) . As an example, if you wanted the first entry to be “father”, you would select the father radio button. That would change the gender to “M” and the appended number would change to “-3”.

Click to fill out information for this patient. Do the same for the others, one by one.

Top portion of patient information form.

Fill out each item. Click the green “E” button to enter it. You can leave items blank if that information is not known.

If you make a mistake, click the “R “ button next to the item where the mistake occurred and re-enter with the correct information and then click the green “E”.

Dates must be in this format (yyyy-mm-dd). If you click the “Select DOB” button, and calendar will appear and you can select a date fro it.

Middle portion of patient form

Fill out each item and enter each one by clicking the green “E” button.

If you are not filling out sample physical information (i.e. concentration, volume, storage locale etc.), then click the “DNA” checkbox to make the final entry button active (at bottom of page). This temporary trick will be changing soon.

2. Select an available slot to store your sample

Final set for this patient: Click “Send To DB”

Slots that are unavailable have the red background.

If you are not filling out physical information, just leave the physical info blank and click “Send To DB”

Start the process again with the next sample on the list

Step 1: Click here.

Step 2: Enter the number of samples to enter.

Step 3: Click “GO”.

Fill out all the others below in the same way. You may fill all these out first before clicking any of the “Enter Patient Info” buttons or you can do each line by line. The order does not matter.

4 . Click here to pull up a separate form to enter this patient’s information

Filling out the patient info is the same as shown in slides 5 through 10

This is an example of where they are not in the same family

Adding one or more samples to a family already in the database.

Step 1: Click here.

Step 2: Select the “Indication” from the drop down menu (“Epilepsy”, “Known Microdeletion” or “Misc”).
