While a majority of providers address needs for those with Alzheimer’s and other dementias, these pages in the SeniorCare GuideBook show providers specializing in the area of Memory Care and providing separate, distinct and specific services geared exclusively to the needs of those with dementia.This section has been created to assist clients who require or desire such dementia / memory specific care.

Skilled Nursing FacilitiesThe facilities designated as Memory Care offer dementia programs in a designated unit not integrated with the general population. Many times this is a secured setting. Some of the other features to look for may include: consistent staff who receive ongoing dementia training, specialized programs, and a supportive environment. These facilities may be accredited by the Joint Commission of Hospitals as a Special Care Unit.

Assisted Living and Residential Care Homes Assisted Living and Residential Care Homes with Special Care Units providing dementia programs must comply with the disclosure law advanced by the state legislature and training requirements. The points of disclosure are:

1. A statement of the overall philosophy and how it reflects the special needs of residents with Alzheimer’s disease or other dementias;2. The criteria for determining who may reside in the Special Care Unit;3. The process used for assessment and establishment of the service plan or agreement, including how the plan is responsive to the changes in the resident’s condition;4. Staffing credentials, job descriptions, and staff duties and availability including any training specific to dementia;5. Physical environment as well as design and security features that specifically address the needs of residents with Alzheimer’s disease or other dementias;6. Frequency and type of programs and activities for the residents of the special care unit;7. Involvement of families in resident care and availability of family support programs;8. Fee schedules for additional services to the residents of the Special Care Unit; and9. A statement that residents will be given a written notice 30 days prior to changes in the fee schedule.

Adult Day ServicesAdult day programs that provide specific dementia care offer specialized programming that is separate, for a majority of the programming day, from those clients who are not cognitively impaired. This specialized programming considers the unique needs of the cognitively impaired and implements techniques that promote success. This can happen in a separate physical area or in a separate area of a large room, as long as a therapeutic environment can be created.

Support GroupsLed by trained facilitators, Support Groups provide assistance and encouragement to help manage and cope with the disease. Members are typically the spouse, adult child, other family member or friend of someone with Alzheimer’s disease or another dementia. There are also support group for people living with Alzheimer’s disease and other dementias.

MedicAlert® + Alzheimer’s Association Safe Return®

Nationwide identification, support and enrollment program that provides assistance when a person with Alzheimer’s or another dementia wanders and becomes lost locally or far away from home and access to vital medical information in the time of need.

Care ConsultationProvides individualized assistance to persons with dementia and their family andprofessional care partners. Assessment, problem solving, emotional support and resource referrals provided by phone or in-person.

Support GroupsLed by trained facilitators, Support Groups provide assistance and encouragement to help manage and cope with the disease. Members are typically the spouse, adult child, other family member or friend of someone with Alzheimer’s disease or another dementia. There are also support group for people living with Alzheimer’s disease and other dementias.

MedicAlert® + Alzheimer’s Association Safe Return®

Nationwide identification, support and enrollment program that provides assistance when a person with Alzheimer’s or another dementia wanders and becomes lost locally or far away from home and access to vital medical information in the time of need.
