Message from Principal Director of Oral Health

Sekalung budi, Assalamualaikum dan Salam Sejahtera, Terlebih dahulu izinkan saya merakamkan setinggi-tinggi penghargaan dan tahniah kepada Fakulti Pergigian, Universiti Teknologi MARA di atas penganjuran ‘Different Ability Health Conference 2019’. Dengan tema “Providing Holistic Support: A Way Forward”, ini adalah merupakan per-sidangan kali kedua, yang diadakan selepas kejayaan penganjuran konferens ‘Disability We Care 2017’. Saya berbangga dengan usaha Fakulti ini dalam usaha untuk memper-juangkan ‘konsep penjagaan holistik’, di samping mewujudkan kesedaran mengenai golon-gan kelainan upaya di kalangan masyarakat Malaysia. Adalah diharapkan agar para delegasi persidangan akan mendapat manfaat melalui per-kongsian ilmu dan pengalaman yang berguna daripada pakar-pakar pelbagai bidang yang hadir dan berkumpul di sini untuk menjayakan program 2 hari ini. Saya percaya, dengan sesi wacana ilmu ini, konsep penjagaan holistik warga kelainan upaya akan dapat diterap-kan dan seterusnya diamalkan dengan lebih meluas dan komprehensif. Di kesempatan ini juga saya ingin menyeru kepada semua hadirin, termasuk ahli akademia dan pengamal, sama ada daripada bidang kesihatan atau pelbagai disiplin yang lain, untuk bersama-sama menggembleng tenaga dalam menyediakan persekitaran, kemudahan dan perkhidmatan yang lebih kondusif untuk warga kelainan kurang upaya. Jalinan antara pemikir, penyelidik dan pengamal, bersama-sama masyarakat dan warga kurang upaya di Malaysia perlu terus dipergiatkan demi transformasi ekonomi dan sosial negara. Akhir kata, sekali lagi saya mengucapkan syabas kepada Fakulti Pergigian, Universiti Teknologi MARA di atas penganjuran 'Different Ability Health Conference 2019’. Semoga usaha murni ini akan menjadi inspirasi kepada semua individu amnya, dan pengamal Per-gigian khususnya, dalam usaha memartabatkan taraf kesihatan dan kehidupan golongan kurang upaya di negara ini. Message from Vice Chancellor

Assalamualaikum Warahmatullahi Wabarakatuh dan Salam Sejahtera Alhamdulillah, bersyukur kita ke hadrat Allah SWT kerana dengan limpah kurnia-Nya, dapat kita bersama-sama berkumpul dalam Majlis Perasmian Konferens “Different Ability Health 2.0” anjuran Fakulti Pergigian, Universiti Teknologi MARA. Terlebih dahulu, saya dan seluruh warga universiti mengalu-alukan kehadiran dan merakamkan setinggi-tinggi penghargaan kepada semua tetamu. Fakulti Pergigian UITM merupakan institusi pendidikan pergigian terbesar di Malaysia yang menempatkan lebih 100 ahli akademik dan lebih 400 mahasiswa yang berganding bahu mendalami, memperkembangkan dan menyebarkan ilmu pergigian. Sumbangan fakulti pergigian terhadap masyarakat secara langsung dan tidak langsung telah banyak mendapat pengiktirafan negara. Pada masa yang sama, saya percaya fakulti pergigian UiTM sentiasa cakna akan isu global ini dengan menjadi satu-satunya universiti awam yang menerapkan modul pergigian keperluan khas yang lengkap dalam kurikulum. Selain itu, UiTM turut berbangga apabila menerajui perkhidmatan pakar keperluan khas di Malaysia yang menawarkan rawatan kepada pesakit pada semua peringkat umur di klinik pergigian keperluan khas yang ditubuhkan pada tahun ini. Malahan, persidangan “Different Ability Health 2.0” yang dianjurkan pada hari ini dapat mencerminkan UiTM yang maju ke hadapan dalam pendidikan, perkhidmatan dan penyelidikan dalam bidang pergigian keper-luan khas. Bertemakan “Providing holistic support: The way forward”, persidangan ini mengeten-gahkan pendekatan holistic dalam membina satu bentuk penjagaan kepada golongan yang mempunyai kebolehan berbeza. Saya gembira melihat golongan profesional dari pelbagai bidang bersama-sama berhimpun pada hari ini dan berkongsi ilmu sepanjang persidangan ini berlangsung. Akhir kata, saya berharap persidangan ini dapat memberi manfaat kepada semua golon-gan dan berupaya menyuntik rasa kepentingan menjadi masyarakat yang inklusif. Tahniah saya ucapkan kepada Dekan dan warga Fakuti Pergigian UiTM atas penganjuran program pergigian keperluan khas dan penubuhan Klinik Pakar Pergigian Keperluan Khas, yang saya percaya akan dimanfaatkan bukan sahaja untuk rawatan pesakit keperluan khas, malah sebagai platform pembelajaran pelajar pergigian. Message from the Dean

Assalamualikum and Welcome to Faculty of Dentistry, Teknologi MARA.

This is the second year that the Faculty of Dentistry, Universiti Teknologi MARA is

hosting the Different Ability Health Conference. Arising from the success and feedback

from the first conference last year, we thought there are still area that need to be ex-

plore upon to ensure that a complete and proper management can be offered to the

special group of individual.

In this conference, we have invited speakers from different perspective to share their

experience in managing patient with different ability. We hope that with this sharing

session, we will be able to manage patients with different ability with the highest

standard of care.

I take this opportunity to thank all the sponsors who had support our conference. Also

I would like to congratulate Dr Ilham and her team who had worked very hard to en-

sure the conference run smoothly.

Lastly, I hope you will have a fruitful program and the knowledge gain can be used to

give a better management to our patient with the different ability.

Message from Chairperson Assalamualaikum wbt Following the success of Disability We Care Conference that was first introduced in 2017, this year, Faculty of Dentistry UiTM is proudly to host the Different Ability Health Conference 2.0. This time we uphold the theme of "Providing Holistic Support, The Way Forward" that focusing on disseminating and sharing information regarding multi-supports that these target groups needed. In addition, we are also accentuating on empowerment of future possibilites and opportunities. The faculty is blessed to have a dedicated Special Care Dentistry Team under the leadership of the Dean, Prof Dato' Dr Mohamed Ibrahim Abu Hassan. The team consists of multidisciplinary Center of Studies namely Special Care Dentistry , Paediatric Dentistry and Oral Maxillofacial Surgery. This year we hope to reach out more families and individuals with or without different abilities to enhance their quality health of life and improv-ing awareness through health seminars and sharing sessions conducted by participating non-govermental organizations as well as governmental organization. To walk the talk, as of now we managed to induce collabo-rations with PLPP, MRDS, PERMATA KURNIA, PPDK Puncak Alam, Cafe Autism Project, Kiwanis Down Syndrome Foundation and ORKIDS to bring the first hand experience and most importantly, we get to learn from them. I would also like to take this opportunity to express my gratitude and thankfulness to the dedicated committee members of the conference as well as the student committee members for your full effort and time in making the conference a success. Professor Emeritus Dato’ Dr. Lian was formerly a professor at the Department of Oral & Maxillofacial

Surgery of the Faculty of Dentistry, University of Ma-laya, Kuala Lumpur. This Perak born nativeobtained his

Bachelor of Dental Surgery from Gujarat, India in 1971. He started his career as a dental officer at the Ministry of

Health, and in 1976, he was able to further his training at the Manchester Dental Hos-pital in the specialty of Oral & Maxillofacial Surgery. He obtained his Fellowship in Dental Surgery from the Royal College of Surgeons of Glasgow in 1979, upon which he returned to Malaysia to become a lecturer at the University of Malaya. Having won the Commonwealth Medical Fellowship Award in 1984, he found himself appointed as an Honorary Registrar at the Eastman dental Institute, London. He obtained a second fellowship in Dental Surgery from the Royal College of Surgeons of Edinburgh in 1996, making him among the few rare oral surgeons to have double fellowships from two Royal Colleges of Surgeons in Scotland.Prof Emeritus Dato’ Dr Lian Chin Boon is the founding president and currently is still presiding over the College of Dental Specialists Malaysia.Post retirement, he found himself doing voluntary work in several organisa-tions, which include the Spastic Childrens’ Association of Selangor & Federal Territory, National Council for Spastic’s Children Associations in Malaysia, and Alzheimer’s Dis-ease Foundation Malaysia.

Summary of plenary session:

My Left Foot: A Volunteer’s Perspective.

Inspired by the Oscar winning movie in 1989, based on the life of an inspiring Irish individual who overcame his debilitating cerebral palsy, this session would be a gen-eral overview of Cerebral Palsy (CP) and the scenario in the local context. In keeping with the conference theme of ‘Providing Holistic Support: The Way Forward’, the fo-

cus will be the various therapies employed currently, and to look at what future alter-natives would be feasible. CP is the most common motor disability in childhood,

affecting the brain (cerebral) and the usage of muscles (palsy). It is a heterogeneous group of clinical syndromes that describe permanent disorders of movement and pos-

ture. As there is no cure, children with CP have problems with muscle tone, which affects their balance, posture, and their ability to walk and move.Population-based

studies from around the world report prevalence estimates of CP ranging from 1.5 to more than 4 per 1,000 live births or children of a defined age range. In short, around 2

or 3 children for every 1,000 babies born develop cerebral palsy. Another 764,000 people currently live with cerebral palsy, with 500,000 of those being children and teens. Each year, 8,000 to 10,000 infants are diagnosed with cerebral palsy. And

these figures continue to grow annually.

PHOTODENTISTRY- AN INNOVATIVE APPROACH TO IMPROVE STUDENTS’ EMPATHY AND EDUCATIONAL EXPERIENCE IN SPECIAL NEEDS DENTISTRY

Introduction: Development of empathy and training in Special Needs Dentistry (SND) is essential to prepare future dental professionals for managing patients with special needs. The study investigated the impact of an innovative educational activ-ity using visual arts in improving dental students’ level of empathy and educational experience in SND. Methods: All Year 4 undergraduate dental students (n=69, Re-sponse rate= 100%) were invited to participate in the ‘Photodentisty’ module, de-veloped by education experts in dentistry and arts. Participants presented their interpretation of photographs portraying different scenarios and images of individ-uals with special needs, followed by group and expert discussion about manage-ment of these people. A survey was done on participants pre- and post- test using the Toronto Empathy Questionnaire (TEQ), followed by quantitative analysis via paired T-test. Students also reported their learning experience via a written quali-tative survey, which data was then analysed via thematic analysis. Results: Partici-pants demonstrated an improved level of empathy, with significant increase in a majority of items from the TEQ questionnaire. From the qualitative survey, stu-dents cited that their learning activity was beneficial in improving their skills in observation, communication and critical thinking, which were perceived as crucial in caring for patients with special needs. They also reported improved attitudes towards caring for these individuals. Additionally, participants perceived to have acquired better understanding of comprehensive treatment planning and manage-ment of elderly people, patients with disabilities and those with complex medical conditions. Conclusion: The ‘Photodentistry’ module was an effective innovative method in improving dental students’ empathy and learning experience in Special Needs Dentistry.

ORAL HEALTH STATUS, BEHAVIOUR AND ORAL HEALTH-RELATED QUALI-TY OF LIFE (OHRQOL) OF SPECIAL NEEDS ADULTS AT PUSAT LATIHAN

Objective: To determine oral health status, behaviour and OHRQoL of special

needs adults living in PLPP, Bangi. Methods: This was a pilot study involving special

needs adults in PLPP. All participants that meet inclusion criteria were invited to

participate in the oral health examination and self-administered questionnaire

(n=70). Two set of questionnaires on oral health behaviour and Malay version of

OHIP-14 (OHRQoL) were used in this study. Dental examination was conducted by

two trained examiners using DMFT and Oral Hygiene Index-Simplified. Data were

analysed using SPSS version 23. Results: Sixty-seven students consented for the

study. Of those consented, 59.7% of them with physical disability and 40.3% with

learning disability. Most of them (67.2%) were Malay male. Mean DMFT was 1.87

(SD±2.9) and majority had gingivitis (88.1%). Although 75% reported that they

brushed their teeth twice daily, only 43.6% presented with good oral hygiene.

Most of them never use floss (85.9%). About 41.7% were unsure whether they are

using fluoridated toothpaste or not. Around 25.9% snacking >4 times per day and

37.3% reported smoked. Overall, the mean severity of impact using OHIP-14 was

12.75 (SD±14.30), indicating low oral health impact experienced by the partici-

pants. The most impacted domain was psychological discomfort with mean score

of 3.02 (SD±2.27), followed by functional limitation 2.10 (SD±2.15) and physical

pain 2.08 (SD±2.06). The least impacted domain was social disability, which score

1.02 (SD±1.84). Conclusion: The participants’ dental caries experience was low.

However, majority of them suffer from gingivitis. Although some reported prac-

Objective: To assess parental oral health practise and knowledge towards their

ASD children. Methods: A 13-item of self-administered questionnaire of World

Health Organization (WHO) Oral Health Questionnaire for Children (Annex 8) is

administered to parents from four different special education program centre

named Pusat Permata Kurnia Kuala Lumpur, Pusat Pemulihan Dalam Komuniti

Puncak Alam, Qamara Sensory Therapy and Intervention Centre Sungai Buloh and

Pusat Jagaan & Latihan Insan Istimewa Subang Jaya. Data were analysed using SPSS

version 23 for descriptive analysis. Results: Eighty-seven parents were recruited.

All children were diagnosed with any kind of ASD during the data collection. The

age range of the children is between 3 – 17 years old consist of 68 boys and 19

girls. Equal percentage of parents reported that both their child’s teeth and gum

health were excellent with 4.6%. 44.3% reported as good followed by 42% as aver-

age and 9.2% stated as poor for teeth while 4.6% poor for the gum. 35.6% parents

stated that they had never brought their child to seek for dentist care within the

past 12 months. However, 77% parents do clean their child’s teeth at least once a

day. In terms of diet practise, majority of the parents choose to give their children

sweet snacks such as biscuits, cream cakes and buns (95.4%) and more than half of

them do give fruits as a snack at least once a week (56.3%). 33.3% of the children

are caries free. Conclusion: The oral health care practises and knowledge of par-

ents with ASD are considered as above average. However, there is a space for im-

provement for empowerment pertaining dental visits.

ORAL HYGIENE STATUS OF A GROUP OF VISUALLY IMPAIRED SCHOOL CHILDREN IN KUALA LUMPUR.

Background: Children with visual impairment are said to be at higher risk in oral health perspective. These children are challenged every single day in learning or observing the basic skills of life. For this reason, they might have experience of facing higher levels of dental diseases. Aim: The aim of this study was to assess the oral health status of a group of visually im-paired school children in Kuala Lumpur. Methodology: A total of 91 visual-ly impaired students (44 blind, 47 low vision) with age group of 13 to 17 years old from a Special Education School in Kuala Lumpur participated in this study. A clinical oral examination was conducted to determine the oral health status, namely DMFT and simplified oral hygiene index (OHI-S) of the students and baseline data was recorded in a WHO assessment form which was modified. Result: Poor oral hygiene status was noted within both low vision and blind participants without no significant difference in gender. The mean OHI-S score and DMFT score of total students were 1.68 and 0.80. Conclusion: By providing regular oral check-ups dental problems can be detected initially. Further, a study on a larger population of both chil-dren and adolescents need to be conducted in Malaysia to obtain a result that will differentiate statistically.

PARENTAL EXPERIENCES IN THE MANAGEMENT OF CHILDREN WITH SYN-DROMIC CRANIOFACIAL DEFORMITIES

Objectives: This cross-sectional study aimed to assess the experiences of parents who have children diagnosed with syndromic craniofacial deformi-ty (CFA) in terms of their: 1) feelings in caring for the children 2) perceived social stigmatisation experiences and; 3) perceptions of the support gained from health professionals and community. Methods: The sampling frame was all parents of CFA children (aged 18 years and below) who have regis-tered at the Combined Oro-Craniomaxillofacial Deformities Clinic, Univer-sity of Malaya Medical Centre. The study instruments comprised of a vali-dated self-administered questionnaire which used a Likert scale of 1 to 4 (strongly disagree to strongly agree) to assess the first and third objectives and a scale of 1 to 3 (almost never, sometimes, often) to assess the second objective. Descriptive analysis of the data was conducted using SPSS. Re-sults: A total of 30 parents participated in the study (response rate: 75%). In terms of the respondents’ feelings, about 90% felt anxious whenever their child had to undergo an operation despite having positive percep-tions on the outcome. Most parents had never had negative stigmatization experiences, where about 60% reported receiving ‘positive comments’ and were ‘treated with respects’ when they were with their CFA child. Majority of the respondents agreed that they obtained the appropriate and ade-quate support from health professionals and community with regards to caring of their child. Conclusion: Most parents of CFA children in Malaysia had positive experiences in caring for their child, had no significant social stigmatization experiences, and were provided with the necessary profes-sional and community supports.

A RETROSPECTIVE AUDIT OF PAEDIATRIC DENTAL TREATMENTS UNDER GENERAL ANESTHESIA OVER A SIX-YEAR PERIOD

Objective: This study aimed to perform a retrospective audit of dental treatments provided to paediatric patients under general anesthesia (GA) over a six-year period at University Malaya Specialist Centre (UMSC). Methods: The records of paediatric patients (≤18 years) who received dental treatment under GA as a day-care patient at UMSC during the time period January 2012 to December 2017 were reviewed. Information such as pre-operative diagnosis, type of treatment, duration and cost of proce-dure was recorded in the data sheet. The data were analysed using SPSS version 23.0. Results: A total of 304 patients (168 males and 136 females) were included in the study. The mean age of patients at the time they re-ceived GA was 5.5 (S.D: 2.9) years. Patients were mostly diagnosed with multiple caries (86.5%), followed by dental trauma (5.6%) and supernu-merary teeth (4.6%). Most of the dental procedures performed under GA on paediatric patients were a combination of restorative and extraction (79.9%), followed by surgical procedures (6.9%) and extraction only due to caries (6.9%). The mean treatment duration was 79.4 (S.D:33.3) minutes with children less than 6 years of age had longer treatment time (p<0.016). The mean cost of treatment was MYR 3659.67. Conclusions: Majority of the patients were diagnosed with multiple caries. Most of the dental pro-cedures performed under GA on paediatric patients were restoration and extraction. Children less than 6 years of age had longer treatment time. Although GA is necessary for some children, it should be complemented with a preventive programme and behavioural changes to avoid having to repeat the use of GA.

Objective: To assess the oral health behaviour and preventive dental care experiences of patients with special health care needs (SHCN) who attend-ed the Special Care Dental (SCD) Clinic at University of Malaya. Methods: Data were collected from 31 patients using a self-administered question-naire which consisted of questions pertaining to their oral health behav-iours and experiences on receiving preventive dental care (PDC). For pa-tients who were unable to answer due to existing health conditions, par-ents or primary caregivers were approached as proxy. Descriptive analysis was conducted and reported based on percentages to identify the oral health behaviour and PDC of the individuals. Results: Majority brushed their teeth at least once a day (84%), used fluoridated toothpaste (87%) and oral hygiene aids (58%). However, almost half of them (48%) required assistance from parents or caregivers during tooth brushing and consumed sugary snacks in between meals (58%). About 80% had received PDC where the type of care received was mainly oral hygiene advice (77%). Slightly more than 80% preferred the one-to-one demonstration method during PDC. Most gave positive evaluation on their PDC experience espe-cially on items pertaining to the dentist’s attitude. Conclusion: Whilst most of the SHCN patients in this study had positive perception on the PDC that they received and practised good brushing behaviour, their dietary habits need to be improved. Thus, regular preventive dental visit amongst SHCN patients is important for appropriate and comprehensive dental care in maintaining oral health.
