A complex process that involves the spread of a cancer or tumor to distant parts of the body from its original site is called metastasis. However, this is a difficult process. Before it becomes a clinically detectable lesion cancer cell must complete a series of steps to successfully colonize a distant area in the body .The steps are as follows: • separation from the primary tumor • penetration of their basement membranes and invasion through tissues around the initial lesion • survival within blood and entry into the blood vessels • Hematogenous spread is spread via blood vessels. • entry into peritoneal or lymphatic’s cavity - spread via lymph channels is called lymphatic spread • Reaching the distant organ like lungs, brain bone, liver, etc. • angiogenesis-formation of a new lesion along with new blood vessels feeding the tumor Routes of metastasis The cancer can spread by four possible routes such as: • spread via lymphatic channels – most carcinomas are favored by this. • spread via blood vessels – this is favored by some carcinomas and sarcomas that originate in the kidneys • spread via body cavities – the cancer cells seed onto pleural (covering the lungs), peritoneal (covering the gut and stomach and other abdominal organs), membranes ,pericardial (covering the heart) or subarchnoid spaces (covering the brain) • transplantation of the cancer – this occurs by carriage of fragments of tumor cells via needles or surgical instruments to other parts of the body during surgery and diagnostic procedures Treatment of any form of cancer is dependent on whether or not it has spread to distant organs. It may decrease a patient's likelihood of survival if the cancer spreads to other tissues and organs. Choosing treatment: When cancer has metastasized, it may be treated with radiation therapy, surgery, chemotherapy, hormonal and biological therapy, or a combination of these. The choice of treatment generally depends on factors like: • Type of primary cancer. • Location and size of the metastasis or secondary tumor. • General health of the patient.
