In accordance with Part III.D.6 of Permit AR0050296 and your 11/8/13 email, on behalf of El Dorado Water Utilities please find attached the Engineer’s Report regarding the 11/7/13 release from the Ouachita Joint Pipeline. The report contains information on the steps taken to verify the integrity of the repair, plus the Engineer’s certification the line is ready to resume operations. We appreciate your assistance in this matter, and request your prompt review of the document and the Department’s determination on resumption of operations for the pipeline. 1. Comply with Standard Condition 6 of your permit 2. Fully remediate the site including the removal and disposal of all wastewater.   3. Fully assess and determine the cause of the line failure including a review of the entire pipeline to determine 

if additional portions of the line are susceptible to the same failure. 4. Fully repair and recertify the damaged section. 5. Provide a report to the Director detailing the work completed in items 1‐4 above. 6. Continue to cease operation of the pipeline until such time as the work above has been completed and 

approval to restart operation of the pipeline has been received from the Director.

From: Mark Smith [mailto:mark@eldoradowater.com] Sent: Friday, November 08, 2013 10:06 AM To: Shafii, Mo; Uyeda, Craig; Benefield, Ryan Cc: 'Vince Blubaugh'; 'Chuck Campbell' Subject: pipeline failure  Dear Mr. Shaffi:  The purpose of this email is to inform ADEQ of a line failure in the Joint Pipeline near Crain City Road.  The failure was discovered around 8:00 pm last night and all pumping to the Joint Pipeline ceased as soon as all the parties could be notified.   It is not known whether the failure of the line resulted in the release of treated effluent to waters of the State due to the depth of the line in that particular location.  Nor has the cause of the failure been determined at this time.   Repair actions are underway and we anticipate completion within 48 hours.  We will be sending a full report to you at such time as we have more information.   If you have any questions do not hesitate to contact me at (870) 862‐6451.
