INTRODUCTION
Improving the community health condition is not easy. Many health programs in the precious
years failed because of lack of community participation in every level of program planning. The keys of
success of community diagnosis or community assessment programs are the involvement of the
community (directly through community health need survey and acceptability of action programs, orindirectly through Focus Group Discussion by the involvement of Community leaders) in the
assessment of the program.
This part of module assumes that the health assessment team/committee (your team), after
involving the community participation, has prioritized Osteoporosis as the most important disease to
be controlled in the City. The next essential step is to identify all the potential risk factors of
Osteoporosis in each of the La Londe groups of determinants (behavioural factors, environment
factors, endurance/intrinsic factors and Health Service Factors). In order to get a complete situation
analysis, you should identify the most potential risk factors given.
DETERMINING RISK FACTORS
A risk factor is a variable associated with an increased risk of disease or infection. Risk factors
are correlational and not necessarily causal, because correlation does not imply causation. Risk factors
are evaluated by comparing the risk of those exposed to the potential risk factor to those not exposed.
If an association between exposure to a factor and development of a disease exists, how strong is it?
We can also put the question another way: “What is the ratio of the risk of disease in exposed
individuals to the risk of disease in non-exposed individuals?” This ratio is called the relative risk (RR)=

How do we interpret the value of a relative risk?
If the RR = 1, the numerator equals to the denominator, and the risk in exposed persons equals the
risk in non-exposed persons. Therefore, no evidence exists for any increased risk in exposed
individuals or for any association of the disease with the exposure in question.
If the RR > 1, the numerator is greater than the denominator, and the risk in exposed persons is
greater than the risk in non-exposed persons. This is evidence of a positive association, and may be
causal.
If the RR < 1, the numerator is less than the denominator, and the risk in exposed persons is less than
the risk in non-exposed persons. This is evidence of a negative association, and it may be indicative of
a protective effect.

Therefore, by calculating RR (using above formula), we are also able to determine whether an
exposure or a condition can be considered as a risk factor of a disease or health problem.
CASE CONTROLA case control study is an investigation into the extent to which persons selected because they
have a specific disease (the cases) and comparable person who don’t have the disease (the controls)
have been exposed to the disease’s possible risk factors in order to evaluate the hypothesis that one
or more of these is a cause of the disease.

The odds of having the disease with the risk factor present = a/b
The disease with the risk factor absent = c/d
How do we interpret the value of a relative risk?
If the OR = 1, no association between the disease and risk factor
If the OR > 1, risk factor influenced the disease occurance.
If the OR < 1, indicate the probability that the risk factor may be the protective one.

Center for disease research degenerative arrange survey of risk factors of osteoporosis among
women in a “X” City. The survey was conducted on people who suffer from osteoporosis, as
many as 100 people, whereas as a control as many as 200 people. Data risk factors taken fromthe sample population. The result is shown in table below.

PENDAHULUAN
Meningkatkan kondisi kesehatan masyarakat tidak mudah. Banyak program-program
kesehatan di tahun-tahun yang berharga gagal karena kurangnya partisipasi masyarakat dalam
setiap tingkat perencanaan program. Kunci keberhasilan diagnosis masyarakat atau program
komunitas penilaian adalah keterlibatan masyarakat (langsung melalui survei kebutuhan
kesehatan masyarakat dan penerimaan program aksi, atau tidak langsung melalui Focus Group
Discussion oleh keterlibatan pemimpin masyarakat) dalam evaluasi program.
Ini bagian dari modul mengasumsikan bahwa penilaian kesehatan tim / panitia (tim Anda),
setelah melibatkan partisipasi masyarakat, memprioritaskan Osteoporosis sebagai penyakit
yang paling penting yang harus dikendalikan di Kota. Langkah penting berikutnya adalah
untuk mengidentifikasi semua faktor potensi risiko Osteoporosis di setiap kelompok Londe La
penentu (faktor perilaku, faktor lingkungan, faktor daya tahan / intrinsik dan Faktor Pelayanan
Kesehatan). Dalam rangka untuk mendapatkan analisis situasi lengkap, Anda harus
mengidentifikasi faktor risiko yang paling potensial yang diberikan.
MENENTUKAN FAKTOR RISIKO
Suatu faktor risiko adalah variabel yang berhubungan dengan peningkatan risiko penyakit atau
infeksi. Faktor risiko yang korelasional dan kausal belum tentu, karena korelasi tidak berarti
sebab-akibat. Faktor risiko dievaluasi dengan membandingkan risiko mereka terkena faktor
risiko potensial terhadap mereka yang tidak terpapar. Jika hubungan antara paparan faktor dan
perkembangan penyakit tersebut ada, seberapa kuat itu? Kami juga dapat mengajukan
pertanyaan dengan cara lain: "Apa adalah rasio risiko penyakit pada individu terkena risiko
penyakit non-individu terpajan" Rasio ini disebut risiko relatif (RR) = Rasio Insiden (RI) di
Cohort Study
Relatif Risiko = (Risiko terkena) / (Risiko non terpajan)
Bagaimana kita menafsirkan nilai risiko relative ?
1. Jika RR = 1, pembilang sama dengan penyebut, dan risiko pada orang terkena sama
dengan risiko non-terpapar orang. Oleh karena itu, tidak ada bukti untuk setiap peningkatan

risiko pada individu terpapar atau untuk setiap asosiasi dari penyakit dengan paparan yang
bersangkutan.
2. Jika RR> 1, pembilang lebih besar dari penyebut, dan risiko pada orang terkena lebih
besar daripada risiko non-terpapar orang. Ini adalah bukti hubungan positif, dan mungkin
kausal.

Jika RR <1, pembilang kurang dari penyebut, dan risiko pada orang yang terkena kurang
dari risiko non-terpapar orang. Ini adalah bukti hubungan negatif, dan itu mungkin
menunjukkan efek perlindungan.

BAGAIMANA KITA MENGHITUNG RISIKO RELATIF ADALAH SBB:

o Oleh karena itu, dengan menghitung RR (menggunakan rumus di atas), kita juga
dapat menentukan apakah suatu paparan atau kondisi dapat dianggap sebagai faktor
risiko dari penyakit atau masalah kesehatan.
KASUS KONTROL
Sebuah studi kasus kontrol merupakan penyelidikan sejauh mana orang-orang yang
dipilih karena mereka memiliki penyakit tertentu (kasus) dan orang yang sebanding yang
tidak memiliki penyakit (kontrol) telah terkena faktor resiko kemungkinan penyakit, untukmengevaluasi hipotesis bahwa satu atau lebih dari factor tsb adalah penyebab dari penyakit
tsb.

KASUS:
Pusat penelitian penyakit degeneratif mengatur survei faktor risiko osteoporosis pada
wanita di kota "X". Survei ini dilakukan pada orang yang menderita osteoporosis,
sebanyak 100 orang, sedangkan sebagai kontrol sebanyak 200 orang. Data faktor risiko
yang diambil dari populasi sampel. Hasilnya dapat dilihat pada tabel di bawah ini.
