For every problem there are multiple solutions, some better than others. The HMO must identify all of the possible solutions for each problem and then decide which solution or solutions best fits the circumstances of the particular incident.

In planning a response, some solutions might be eliminated due to lack of equipment, supplies, on scene capabilities, safety concerns and/or PPE limitations. Some solutions will not be possible because of time constraints.

As an example, consider Personal Protective Equipment (PPE). For each response option the proper PPE must be selected for the task or tasks involved. The PPE needed for a response will be dependent on the levels of PPE available or on scene, the compatibility of the suit and glove material given the incident conditions, as well as the experience and training of the responders working in the PPE. For any one given option there may be more than one level of PPE needed to perform the separate tasks involved.

The Incident Action Plan is a very important task for which the HMO is typically responsible. Emphasize that an IAP can be a simple verbal agreement or can be complex written document when multiple jurisdictions across more than one operational period are involved. In a large scale incident in which a Planning Chief has been named, the Planning Section Chief is responsible for writing the IAP. It can get complicated if the Planning Chief requires the HMO to do write the tactical planning section for inclusion in the IAP. Explain the difference between strategy and tactic as these concepts are used throughout the remainder of this lesson by reviewing the following definitions and examples:

Incident Objective- what needs to be accomplished Example: Eliminate the flammable vapors

Strategy – The general plan or direction for accomplishing an Incident Objective Example: Use foam to blanket product

Tactic – The specific assignments that specify how the strategy will be accomplished

Example: Have Engine 4’s crew apply a 3% concentration of AFFF (Aqueous Film Forming Foams) foam to the spill.

Once the tactical objectives have been approved by the IC, documentation must be made as to the assignments, who will perform the tasks involved, and in what level of PPE. This documentation becomes part of the Incident Action Plan (IAP). The IAP contains the goals, strategies and tactics of how the situation will be handled. The IAP can be as simple as a verbal

agreement, such as the action taken by an engine company to extinguish a car fire, or it can be a complex written document. On common emergencies the IAP is not written however when a situation involves multiple jurisdictions or multiple agencies, if more than one operational period of time is needed to conclude the incident, or when a large number of the ICS positions are being filled, it becomes necessary to document the IAP in a written form.

The IAP becomes the road map to arrive at the desired outcome as well as the history book of what took place on the trip. The stated problem, the incident objectives, the strategy and tactics used to achieve those objectives, the personnel assigned to each task as well as the equipment used, PPE worn, and communications channels utilized become part of the IAP. Under NIMS the federal government publishes the ICS 200 forms that are useful in developing a written IAP.

When developing the IAP, one must understand the steps involved. Review the “Planning P” steps for developing an IAP as taught in the ICS 300 course. Remind the participants that the ICS Form 215 noted in the content is used to communicate the decisions made concerning resource needs for the next operational period. It provides the Incident Commander, Command and General Staff with the means to identify Division or Group assignments, develop specific tactics, and identify available and needed resources. The safety concerns will be discussed in the HMSO module in relation to the ICS Form 215A.

The process used to develop the IAP is taught in the ICS 300 course. This process is part of the “Planning P” and includes five major steps. The steps are:

As you can see these are similar to the same basic steps that NFPA utilizes in establishing the competencies of this course. The ICS 215 form can be used for documenting the objectives and strategies as well as the groups/divisions assigned to each task. A copy of the ICS 215 form is included below and a full copy can also found in Appendix A. Under the strategic goal of isolation there is the tactical objective of public protective actions. These actions are steps taken to protect the surrounding population from the HazMat spill. Basically evacuation and/or shelter in-place are the two considerations for public protective actions. The Emergency Response Guidebook (ERG) provides the suggested initial distances for evacuating the area and provides information and guidelines on sheltering in place. Factors such as the characteristics of the hazardous materials (vapor density, IDLH levels etc.) or wind direction, speed and even the time of day will cause the elimination or adjustment of the options. For example, sheltering in place is not considered a good option when dealing with vapor clouds of flammable gases.) Plume modeling software such as the CAMEO/MARPLOT/ALOHA suite can be utilized to determine downwind evacuation zones. Air monitoring can be used to verify the hazardous areas.

Part of the IAP involves ongoing assessments to determine the success of the plan. Tasks must be evaluated constantly to determine if adjustments need to be made to the strategy or tactics being employed. Setting benchmarks will help in evaluating the progression of the IAP. Supervisory skills will be needed to determine if a failure to complete a task is due to equipment issues, staffing issues, training issues or other issues that will need to be corrected before the next operational period. The written IAP must remain dynamic and be changed or adjusted at any time. To help the HMO to coordinate and asses the IAP he/she will need to assign certain positions within their branch or group. As taught in the Ohio HazMat/WMD technician course there are ICS positions that will typically fall under the direct supervision of the HMO. The HazMat Branch/Group and each of the assigned ICS positions must coordinate their activity with the other branches, groups, divisions or teams operating on the scene. Coordination will be necessary with:

Mutual aid providers Law Enforcement officers performing public protective actions Fire suppression teams Groups assigned to perform defensive measures used to control, contain and confine the

hazardous material EMS groups or branches. They will need chemical information from the Research Officer

and may be providing the HazMat branch/group with the EMS services standing by for entry and back-up teams

Decontamination Units Environmental Remediation Services trained in cleaning-up the spill, and Evidence preservation and sampling teams if the incident involves criminal activity

Coordination requires communication. The HazMat branch will need to be in constant communication with the IC and the other branches, groups, and divisions in order to have successful coordination of efforts. Regularly scheduled meetings with the IC and other team leaders will help in this process.

As the team leader it is important to supervise the positions assigned to you and track their productivity. As problems arise that effect the progress of the incident they must be identified and steps taken to resolve the shortcomings. On incidents that span a longer period of time, regular meetings should be held to update and brief the IC and your branch/group on its’ progress. Your team must always be aware of your expectations and requirements. They must understand the entire mission and know their part in accomplishing the strategic goals.

The safety briefing is a quick meeting to present the basic facts about the chemical, container and the environment surrounding the incident. It is the time to discuss safety hazards and concerns for each group or division assigned to perform a task. The elements found on the ICS Form 208 become the basis of the “Safety Briefing”. ICS Form 208HM (included below and in Appendix A) is a suggested “Incident Site Safety and Control Plan” (SSCP). This plan should be filled out in coordination with the HMSO. The ICS 208 will allow the HMO and HMSO to document many of the same things found in the IAP, however, it is focused more on the safety issues of the incident. The SSCP becomes a part of the documents included in the IAP. OSHA’s HAZWOPER standard (29CFR 1910.120(b)(1)(ii)(C)) requires that a site safety plan be written for every HazMat incident. The 208 form will be reviewed in detail and utilized in the HMSO module.

A concise summary of this lesson is included in the first paragraph under the slide which may be helpful for the participants to review before facilitating the Instructor-Led example which is a continuation of the example started in Lesson 1.

Highlight the following key points from the example incident: Based on the analyses done in Lesson 1, one of the goals would be to eliminate the flammable vapors. The following objective might be decided upon, written and approved. “Cover the spill of gasoline with foam to eliminate the production of flammable vapors and maintain the coverage for the remaining operational periods.” This objective would require the following tactics to be determined and assignments made in order to complete them:

Deployment of hose lines

Establishment of a water supply

A supply of foam concentrate

A decision on what concentration to be used

Firefighters in full fire protective clothing and SCBA to apply the foam

Air monitoring teams in PPE to verify that indeed the concentration of gasoline vapors is reduced and maintained below IDLH levels.

All activity to be supervised and placed in a chain of command

Designated titles and communication channels

Documentation of the entire process

After analyzing the incident, as the HMO your next task is to plan the response which includes suggesting solutions to the problem. Those suggested options, if approved by the IC, will become the objectives for your branch/group of the HazMat incident. As the HMO, you must select the tactics, choose the PPE, determine the decontamination plan, assign personnel including entry and back-up team requirements, and implement EMS responsibilities for the responders on your team. All of these things must be documented as part of the Incident Action Plan.

INSTRUCTOR – LED EXAMPLE: The example of the gasoline tanker incident continues. We have analyzed the incident and based on that analysis we can begin to develop a plan of attack. Multiple strategic goals can be suggested and depending on the amount of responders on the scene we must prioritize each tactic. One response objective might be to control the potential of a fire. The response option to perform this would be the tactical objective to “foam the spill and maintain the foam blanket to keep the concentration of flammable vapors below the IDLH level for the remaining operational time frame”. This will require the following tasks to be coordinated:

Deployment of hose lines Establishment of a water supply A supply of foam concentrate A decision on what concentration to be used Firefighters in full fire protective clothing and SCBA to apply the foam Air monitoring teams in PPE to verify that indeed the concentration of gasoline vapors is

This is just one of several objectives that need to be accomplished in this example. Additional objectives, which would be simultaneously completed, could include: evacuation of the hazard zone, traffic being detoured around the site, and plans being made for a HazMat team entry into the hot zone to stop the release.
