    As a JPIAn, how would you revolutionize the entire business industry?

    Develop on the matter through any of the following: portraying an artwork, writingcomposition, or taking a photograph. Choose the best spot you are good with.

    PORTRAIT. Submit an 8.5 x 11 artwork using any medium of your choice. It could be pepainting, water color, etc. It must be originally made by the applicant and has not won previouslin any contest. On a separate sheet of paper, write a short description on the award. Thdescription should be not more than 100 words.

    COMPOSITION. Your essay must be unique and has not been reproduced to any periodicalsubmitted as a requirement for any university event. Accomplish a one-page, 500-word,typewritten essay. Those who choose this category must also email a soft copy of the essay [email protected] .

    PHOTOGRAPH. The shot must be unique and has not been printed to any periodical. Digretouching is allowed as long as the quality of the picture will remain. Simple retouching suchdensity, contrast, burning, fill flash, strobist is allowed. Submit a printed 8 x 10 photograph ana CD containing the soft file of the picture in jpeg format. On a separate paper attached to tpicture, write a brief description of the photograph. The description should not exceed 100 words.

    Choose one (1) evaluator with a deep knowledge of the personal history of the applicant. Thletter of the recommendation must specify the reasons, circumstances, and impressionssurrounding the applicants life story that the evaluator thinks are vital to his/her recognition asan outstanding student.

    Indicated below are the guiding questions for the evaluators.

    1. How did you know the applicant?2. What are his/ her unique values?3. What are the strengths and weaknesses of the applicant? What are his/ her fears?4. Does he/she deserve this award?5. Feel free to add all necessary information that would help our dear evaluators to know moabout the applicant such as his/ her qualities, how he/she works as a leader and some otheinformation which you believe would be value-adding to this screening process.

    The evaluators answer must be written on a one-page letter. On the professional letterhead,

    information below must be included:

    The candidate must properly secure the following documents in hard copy.

    1. Duly signed endorsement letters from the College Dean, Incumbent JPIA Adviser andIncumbent JPIA Local Chapter President.2. Fully accomplished application form.3. Certified True Copy of Grades or Transcript of Records with Grading System4. Comprehensive Resume with two 2x2 colored picture.5. Summary of Top Achievements (including past years as long as he/she is a JPIAn)6. Photocopy of all corroborating certificates supporting all claims and assertions made in thebid. (It must be in chronological order). Photocopies or pictures of Medals and Plaques are noaccepted. It must be numbered and arranged chronologically (for it is needed on the referencepage part of summary of achievements).7. Certificate of Good Moral

    Qualifications:1. Must be a bona fide member of NFJPIA FY 2012-2013.2. No non-passing grades in all subjects taken including non-academic subjects.3. GWA of 85% or above or its equivalent. No dropped subjects. Major subjects retaken wilbe given 6 units allowance. Units in excess will be grounds for disqualification.4. Subject initially marked as incomplete would be fine as long as the completion grade ipassing.5. The candidate must not be involved in any criminal case brought upon by any competentcourse of jurisdiction.
