Principles of Inpatient Care

1. The safety and wellbeing of our employees is paramount

2. We will protect our non-COVID inpatients from transmission, especially our most vulnerable patient cohorts (immunocompromised and elderly)

3. We will equip our employees with the resources they need to provide care (PPE, IT systems, equipment, training, psychosocial support)

4. Where demand exceeds our capacity to provide care, care will be prioritised based on agreed clinical policies

Phase 1: Demand is Below Capacity (current phase)

Trigger for Phase 1: First COVID-19 case presents to Monash Health

Clinical Impact:

Cases may present to ED via Fever Clinics, GP or direct presentation

Confirmed and suspected COVID cases managed in negative pressure rooms (single rooms if no negative pressure)

Goals of Care remains as per usual policy

Operational Impact:

No change to configuration of inpatients clinical areas

Workforce Impact:

No change to configuration of inpatient staff

Training and Upskilling of inpatient staff to prepare for subsequent phases

Phase 2: Demand is At or Approaching Capacity

Trigger for Phase 2: Capacity reached for negative pressure / single rooms for COVID confirmed or suspected cases at any acute site

Clinical Impact:

Suspected and confirmed COVID cases will be cohorted in designated wards

Non-COVID patients will be cared for in non-COVID wards and clinical areas

Goals of Care may be altered for some confirmed COVID patients

All Elective Surgery cancelled

Additional MET call teams

Operational Impact:

Current wards will be repurposed for Suspected and Confirmed COVID wards

Suspected cases will move to the Confirmed COVID ward as soon as case is confirmed

Dedicated COVID bedcards will be activated at all acute sites

Confirmed COVID patients from Kingston to transfer to acute site if indicated

Workforce Impact:

Dedicated COVID medical teams

Dedicated COVID nursing teams on Suspected and Confirmed COVID wards

No mixing of COVID and non-COVID care teams

Trigger for Phase 3: Capacity reached for ICU – unable to accommodate COVID-19 cases for intubation

Clinical Impact:

COVID Intubation Prioritisation activated

Goals of Care will be altered for some confirmed COVID patients

Operational Impact:

Will need increased capacity for Suspected and Confirmed COVID wards at all acute sites

All possible areas where patients can be safely accommodated may be commissioned

Will need increased Mortuary capacity

Workforce Impact:

Will need increased numbers of COVID medical and nursing teams

COVID teams may stay in residence at their acute site (accommodation, cleaning and catering needs)
