EDITOR’s LETTERAyurveda is one of the world’s oldest holistic (“whole-body”) healing systems. It was developed more than 3,000 years ago in India. It’s based on the belief that health and wellness depend on a delicate balance between the mind, body, and spirit. Its main goal is to promote good health, not fight disease. But treatments may be geared toward specific health problems. Ayurveda is a remarkably individualized system of medicine. The tradition is rooted in the idea that each of us is born with a completely personal blueprint for optimum health. From birth onward, it is this reference point—your constitution—that, in many ways, defines who you are. No two constitutions are exactly alike; therefore, no two human beings can have precisely the same expression of ideal health. In Ayurveda, everything is medicine it all depends on the context of who you are, and what patterns are currently at play in your particular system. Ayurveda is fundamentally opposed to one-size-fits-all remedies. While there are certainly some practices that are considered to be beneficial for most everyone, Ayurveda places the focus firmly on you, the individual concerned. Ayurveda has a long tradition of supporting and promoting pregnancy. Infertility is one of the leading diseases around the world. Many couple are not getting their progeny because of lifestyle changes, and other factors. So before the treatment, modification of lifestyle, cleaning of the body and mind is needed, this can be achieved from Ayurveda principles. In fact, Ayurveda emphasizes the preconception period just as much, if not more, than the pregnancy period itself. The preconception time offers a golden opportunity to provide the best of the both of you to your future child. Ayurveda preconception care takes advantage of our body’s natural ability to cleanse and renew. No matter what you’re past experiences with fertility are, Ayurveda offers a complete approach to fertility that embraces both the male and the female roles as equally important. The lens of Ayurveda broadens our vision to see that fertility is more than the physical act of becoming pregnant. Fertility is first about your shift into parenthood, as a couple, which comes well before conception. It begins with your first true desire to begin a new family. This desire serves as an invitation to the new soul. In this way, understand that fertility is equally spiritual and emotional as it is physical. Appreciate that achieving fertility is also about achieving the healthiest child possible on all dimensions so that the soul may fulfil its purpose to the highest degree.

Polycystic ovary syndrome (PCOS) is a condition that affects a woman’s hormone levels. Many women have PCOS but don’t know it. In one study, up to 70 percent of women with PCOS hadn’t been diagnosed. PCOS affects a woman’s ovaries, the reproductive organs that produce estrogen and progesterone — hormones that regulate the menstrual cycle. The ovaries also produce a small amount of male hormones called androgens (eg: Testosterone). The ovaries release eggs to be fertilized by a man’s sperm. The release of an egg each month is called ovulation. Follicle-stimulating hormone (FSH) and luteinizing hormone (LH) control ovulation. FSH stimulates the ovary to produce a follicle — a sac that contains an egg — and then LH triggers the ovary to release a mature egg.

Symptoms:1) Acne2) Sudden weight gain3) Irregular periods4) Hair fall5) Infertility6) Abnormal hair growth especially on face7) Depression or Mood swings8) Insomnia

Although these are the symptoms associated with PCOS, please know that, these are also the symptoms for few other disorders too, so diagnosis should be done only with the help of a doctor.

Complications:o Infertilityo Gestational diabetes or pregnancy-induced high

blood pressureo Miscarriage or premature birtho Nonalcoholic steatohepatitis — a severe liver

inflammation caused by fat accumulation in the liver

o Metabolic syndrome — a cluster of conditions including high blood pressure, high blood sugar, and abnormal cholesterol or triglyceride levels that significantly increase your risk of cardiovascular disease

o Type 2 diabetes or pre diabeteso Sleep apneao Depression, anxiety and eating disorders

o Abnormal uterine bleedingo Cancer of the uterine lining (endometrial cancer)

Causes:There are no proven causes of PCOS, But the leading theories that causes PCOS are these:

1) Hyperinsulinism Insulin is the hormone that helps Glucose to

pass through cell membrane for metabolism, for this Glucose and Insulin needs to bind. But if the visceral fat layer (belly fat or intra-abdominal fat) of our body secretes insulin like growth factor binding protein (IGFBP) this may leads to hyperinsulinism. To avoid this the best way is to reduce visceral fat in our body. When insulin rises in our body this may cause increased production of androgens in our body, causing PCOS.

2) Pituitary-Thyroid-Ovary axis hypothesis Ovulation cycle is regulated directly by pituitary

gland through the secretion of Luteinizing hormone (LH) and Follicle-stimulating hormone (FSH). These are secreted by Anterior Pituitary Gland. If pituitary gland secretes law amount of FSH then it will lead to PCOS. There is another gland at work here, Anterior Pituitary Gland is in turn regulated

by Thyroid gland through negative feedback mechanism. So if thyroid is not functioning properly that will affect Pituitary in turn ovulation. This could be why most of the time PCOS and Hypothyroidism are associated.

3) Genetic Although not proven scientists

believe genetic factors also could be one of the reason for PCOS. If your family members suffer from PCOS, studies shows that, then there is higher chance for you to get PCOS.

Treatment:As the disorder treatments for PCOS should also be multi factorial.

Diet:These would be the key feature on PCOS diet,

IodineOrganic Iodine is a key part for normal thyroid to functions. Even though iodized salt is available in market, this may not be the ideal solution for our body’s iodine requirement. So it would be good if take some food with rich iodine content. Some of the popular iodine rich food common in India are Curd, Berry fruits (Strawberry, Blackberry, etc), Potato, Sea food, etc.

Anti-inflammatory foodsAnti-inflammatory diet for PCOS should be focusing mainly on the essential fatty acids such as Omega 3, Omega 6. Essential fatty acids have proved to be very effective in case of inflammatory conditions, foods rich in essential fatty acids are Flax seeds, Flax seed oil, Fish liver oil, Rice bran oil, Walnut, Almond, Coconut oil, Coconut milk, etc.

Anti-oxidantsAnti-oxidants can reduce the levels of oxidization in our body, these are especially beneficial to reduce the negative effects of visceral fat layer. Many vitamins and other essential nutrients have anti-oxidant property. Some foods rich with anti-oxidants are fresh fruits and vegetables, green leafy vegetables, coconut oil and olive oil, almond, walnut, fig, dates.

YogaTo reduce oxidization in body and to reduce inflammation in body pranayama practices such as Anulom-Vilom, Nadi-Shudhi and Surya Bhedana.

Also Practices like Jalandhar Bandha and Moola Bandha are very effective for PCOS Management.

Strict follow up of diet and yoga for at the very least 3 months have shown significant improvements in the PCOS condition, clinical trials shows menstruation returning to regular cycle and even regular ovulation.

Mucuna pruriens is a slender, herbaceous, twinning, annual with hairy branches. The plant is used as food as well as medicine by humans and the tender leaves are used as fodder for animals. It is a potent aphrodisiac, anti deppresent and geriatric tonic which is also useful in neuropathy, nephropathy and general debility. The demand of this drug in India and in International market has increased many folds as it has L-DOPA which is made utilized in the treatment of neurological disease mainly Parkinsonism.

In Ayurveda the drug kapikachu is used for its properties like balya, Brahmana, vrisya and vata samana. It is a main ingredient in preparations like Amrita prasa ghrita,Maha mayura ghrita, Kapikachu choornam, satavaryadi choornam ,somaraji thailam, aswagandhadhi lehyam and vidaryadi kashayamThe drug has found to be mentioned in classics as follows:

• Caraka-madhura skanda,balya mahakashayas, purisa virajaniya varga,vamanopaga.

• Susrutha- vidaryadi gana &vata samana gana, vrishya.

• Vagbhata- Durvadi gana.

Synonyms of the plant includeKapikachu, Kachura, Kapiroma phala etc, which indicates the plant is hairy vine, which causes severe itching.Atmagupta, swayam gupta, dusparsa & durabhigraha indicates that the plant is capable of self-protection on account of its hairs which is highly irritant not only to monkeys but also to human beings.Brahmani, vrisya indicating its brihmana and vajeekara property.

DISTRIBUTION:• It is distributed all over India up to 1000 m in

Himalaya & in Andaman and Nicobar Island, commonly seen in Bengal and Assam.

MORPHOLOGY:• HABIT- An annual twinning climber of 5-8 m long

having slender hairy branches • LEAVES-Trifoliate, stipulate, ,petioles 6-11 cm

long: leaflets- thin, densely silvery hairy beneath ,terminal one rhomboid, laterals unequal sided with dilated lower margins.

• INFLORESCENCE- axillary racemes of 15-30 cm long.

• FLOWER- Purple coloured :Calyx- Silky outside, companulate with upper 2 teeth focused to form a triangular lobe with lateral ones deltoid &lower tooth lanceolate and longer than tube:corolla- 2.5- 4 cm long, vinaceous

• FRUITS- A sigma shaped turgid pod, explosively dehising of 6-9 cm long,2 cm broad, longitudinaly ribbed, curved densely clothed with persistent pale brown irritant bristles .

• SEEDS- 5-6, Smooth, ellipsoidal, about 1 cm long, blackish brown, generally variegated.

• ROOTS- long, soft woody flexible roots of 7mm thickness or more: outer surface-dark brown to black with protruding or prominent lenticels and few rootlets.

PARTS USED: Root, seeds, pod &hairs.

CONSTIITUENTS:• Seeds are naturalized sources of amino acids,1,2,3

dihydroxy phenyl alamine L-DOPA,the direct precursor to the neuro transmitter dopamine .It also contain ether alkaloids like mucuadine, mucuadinine, along with maucunin and small amount of nicotine various amino acids, carbohydrates and indole alkyl amines and serotonin.

• Seeds oils-Palmitic, stearic, oleic acids, acids & Sterol.

• Pod-trichomes- 5 – hydroxytryptamine.

RASAPANCHAKA:• Rasa- madhura tikta.• Guna- guru snigdha.• Vipaka- madhura• Virya- ushna virya(roots), sheetha virya (seeds)• karma: Tridosha shamaka(SN),Vata samana(su.sa),

Vata- pitta hara (DN) ,Vrishya, Balya, vajeekarana, brihmanam.

INDICATION: vatavyadhi, atisara, yoni roga, klaibya, swasa(su.sa)

DOSAGE:Seed powder: 3-6 gm.Root dec- 50-100ml.

THERAPEUTIC USES:• Neurological disorders: decoction of atmagupta

roots is beneficial in apabahuka vyadhi.(CD)• Roots processed with oil mitigates pain due to

arthritis.• Decotion 0f roots is useful in diseases of nervous

system such as hemiplegia,falcial palsy.• Decoction of roots can also be used to induce

mensturation.• Seeds boiled in milk is used with ghee and wheat

act as aphrodisiac.• Compound powder of seed and fruits of gokshura

equal parts with sugar and milk also act as vajeekarana.

• Intake of seed powder with honey and ghee is useful in swasa roga and dyspnoea.

• Researches prove that oral administration of 5 g m of seed powder taken along with hot water once in a day is beneficial for men with decreased sperm count and motility.

Dr. P. Ram Manohar is the Research Director at Amrita Centre for Advanced Research in Ayurveda. He has been contributing in the field of Ayurvedic research since the last 24 years. He has to his credit more than 60 publications with research papers published in SCI research journals as well as contributions in other journals and chapters for books. The Amrita Centre for Advanced Research in Ayurveda (ĀCĀRA) at Amrita School of Ayurveda focuses on paving a strong scientific approach towards Ayurveda. The centre’s attention is to cultivate this research culture in an Ayurveda academic environment, improving the quality of postgraduate and doctoral research programs and to generate high quality research in herbal medications.ĀCĀRA seeks entreat collaboration between modern scientific researches and Ayurveda researches to gain a better understanding of Ayurveda treatment’s basic concepts, mechanism, and clinical outcomes. ĀCĀRA further aims to develop a platform to facilitate integration between practitioners of modern medicine and Ayurveda, and to establish a model centre for collaborative, integrated health care. Gaining global acceptance for Ayurveda can only be achieved through collaborative research with scientific organizations and universities worldwide. This is ĀCĀRA’s mission and Amma’s vision when she proposed the concept of ĀCĀRA.Dr. Ram Manohar, the Research Director at ĀCĀRA delivered talk at Ayurveda College, Trivandrum on 1st of July. In the middle of July along with the Medical Director at Amrita School of Ayurveda, Br. Sankara Chaithanya he visited Arya Vaidya Nilayam, Madurai.ĀCĀRA and Amrita centre for Nano science and Molecular Medicine togetherly hosted Amrita Samyogam 2017 on 6th August 2017 at Amrita Institute of Medical Sciences, Kochi. Amrita Samyogam 2017, an International Conference on Integrative Ayurveda and Modern Medicine was attended by more than 1000 delegates representing Ayurveda, Modern Medicine and Modern Science. The focus of the conference was primarily Clinical Integration of Ayurveda and Modern Medicine by identifying areas for cross referrals as well as multimodal combined treatment in specific diseases. In order to strengthen the clinical integration, the conference also proposed to discover biological mechanisms through basic science studies and Integrating new technologies for effective delivery of treatment at point of care.The Special Secretatry of Ministry of AYUSH, Dr. Rajesh Kotecha, The Advisor of Ministry of AYUSH, Dr. Dinesh Katoch, Scientist from Office of the Principal Scientific Advisor to Government of India, Dr. Ketaki Bapat, Director of National Institute of Cancer Prevention and Research, Dr. Ravi Mehrotra, the Director of All India Institute of Ayurveda, Dr. Abhimanyu

Kumar and Director of NIMHANS, Dr. B. N. Gangadhar were amongst the top government officials who attended the conference. About sixty resource persons took part in the deliberations of Amrita Samyogam 2017 conference from United States, Germany, Italy, Latvia, Argentina and India. About 100 Institutions were represented including 22 States from India. Academic sessions of the conference included a Pre-conference Workshop on Scientific Writing and a Pre-conference Meeting on Integrative Oncology. The major areas for iintegrations that was taken up for discussion during Amrita Samyogam were Cancer, Diabetes, Neurodegenerative Diseases, Musculoskeletal Diseases and Mental Health.

Dr. Ram was a part of Panel Discussion on Integrative Medicine on FICCI HEAL 2017: “Indian Healthcare: A Patient’s View” on 18th August at New Delhi. He also delivered a talk on Traditional Veterinary Medicine during 19th International Ayurveda Symposium at European Akademie For Ayurveda at Birstein, Germany and attended panel discussion on Integrative Medicine at the Symposium on 9th September 2017 Evening Session At Rosenberg Europäische Akademie für Ayurveda.

ĀCĀRA organized a Continuous Health Education program for a group of Scholars from Argentina (August5 to August30).In the same month a memorandum of understanding (MOU) was signed between ĀCĀRA and Fundacion Villa Luz for Human Development and Quality of Life, Argentina

On September, he made a talk on improving Clinical Research in PG Curriculum during the National Ayurveda Summit at Gandhinagar with Prof. M.S. Baghel, Prof. Anand Chaudhary, Prof. Umesh Shukla, Dr. Brijesh Mishra at Mahatma Mandir, Gandhinagar. He also attended SATAPATHAM - Regional Seminars conducted by Kottakkal Arya Vaidya Sala in connection with Centenary Celebrations, SN Ayurveda College, Puthur. Following this on October 6th, he delivered a talk on “Ayurveda as a Knowledge System: Epistemological and Ontological Premises” on a National Seminar on Indian Intellectual Traditions: Concepts and Concerns conducted by the Department of Sanskrit, Calicut University with the support of Kerala Sahitya Academy. On October 17th, the National Ayurveda Day (Dhanwantari Jayanti) Dr. V. Rajmohan delivered a talk on Pain Perspectives at Amrita School of Ayurveda on special invitation of ĀCĀRA. Following this Dr. Ram made a visit to Argentina to impregnate an idea of simplicity and benefits of practicing Ayurveda rather than other medications. After return, on 30th October he delivered a talk on Ayurveda - The Future of Integrative Medicine on a seminar on Ayurvedic approaches.

On November Dr. Ram attended the programme related to first International Research Project funded jointly by Italian Ministry and Govt of India. It is regarded as a milestone for Amrita School of Ayurveda and Dr. Ananthram Sharma was ready to take the final small step into the aircraft and he made a giant leap to University of Milan, Italy. On November NOV27 & 28, ĀCĀRA conducted a workshop on Research process- From Question to Publication. The workshop was a great success where lectures were delivered by Dr. Girish Thillu, Dr.Kishore Patwardhan and Dr.Supriya Bhalerao.

Dr. Ram Manohar had delivered lectures on Ayurveda Seminars from 2017 December 2nd to December 10th at Novosibirsk, Siberia, Russia. Also he made discussions with Prof. Aifman Roman, Novosibirsk State Pedagogical University, Russia along with Dr. Oleg and Prof. Mikhail about possible collaborative research and MOU with Amrita University during his Russian visit. He attended a five day seminar introducing the basics of Ayurvedic approach to health and disease management organised by National Ayurveda Medical Association of Russia with Biokvant under the leadership of Oleg Sorokin and was completed with about 50% participants being medical professionals. 15 cities were represented in the same including neighbouring countries like Czech Republic, Finland and Kazakhstan which was held at Mira Healthy Centre, Novosibirsk, Russia.

After returning from Russia, Dr.Ram had made arrangements for inaugurating a well equipped lab named ‘Amrita Centre for Advanced Research in Ayurveda’ laboratory which is established under his supervision. Thereby, Swami Amritageetananda Puri inaugurated ĀCĀRA Research Laboratory by lighting the lamp on December 15th.

Her Holiness Sri Mata Amritanandamayi Devi (Amma), the Chancellor of Amrita Vishwa Vidyapeetham had a vision of uniting modern and traditional medical approaches for the sake of humanity. She recognized that mainstream medicine would not accept traditional medicine unless its outcomes and medications were analyzed and scientifically documented. The stories of successes in ayurveda or herbal approach is not accepted worldwide unless it is based upon scientific evidence. If there were statistical and scientific proof of the efficacy of traditional healing methods, this can published in international journals and take attention of people and can strive in the area of therapies where modern medicine was believed to be the only solution and also it can fight with allopathic medicinal issues like resistance to drugs, side effects, and escalating costs. This was the mission under establishment of such a Research laboratory exclusively for Ayurveda. Following the ceremony, an International Symposium on Ayurveda in Neuromuscular and Neurodegenerative Diseases was conducted at Amrita School of Ayurveda on the same day.

On Dec21st, Dr. Ram made lectures on recent trends in Ayurgenomics in Continuing Medical Education (CME) programme on Genomics at Govt. Ayurveda College, Trivandrum.

In this above verse, Acarya Caraka telling that one who obeys his duties regarding own body is an INTELLIGENT. To convey that he has given 2 beautiful examples for both internal and external factors by

1. Nagari :- Means Officer in charge or king, as we know officer or king always think about the happiness of his society. He always chooses the right way to control the negativities in his surroundings. As the law of Karma says that every good or bad action has a definite result, sooner or later in life. If the officer or king serves better for the people, they also give the same response. If the officer or king is injudicious in their duties, then people will also indulge in unlawful acts. The position of king or officer will reduce day by day, finally they will get demotion. Here some people from the same place only will harm the society. Similarly for us, we are the king of our beautiful body. It is our duty to know what is needed and what is not to maintain the perfect healthy status of our body but then also day by day, we are becoming more and more negligible towards our daily lifestyle. Not we ??????

2. Rathi - means the Charioteer, who controls the chariot specially the horses. They are meant for the racers. But the charioteer controls their racing mind and moves the chariot and reaches the people safely to the destination. He is always an alert and attentive while riding the chariot, to protect it from the obstacles like humps, stones etc. on the road which are external factors. If he is not alert then the chariot, horses, charioteer himself and the people may also get injured. Similarly in our life the MANAS which is called to be cancala (always wandering), like uncontrolled horses only. Many external factors will divert the mind, but if we have the perfect hold on it, then we can achieve the desired goal. Otherwise we have to face many ups and downs in our life. For an example we cannot concentrate on both works together for a long time, even though while going on the road will hear songs by using headphones in high volume. Is it good????

After observing these examples given by Acarya Caraka, it can be conclude that if the person can control his emotions, if he thinks and act wisely, can avoid all the obstacles and finally he can achieve the optimum health

Message

INFERTILITYInfertility is the most distressing condition affecting a couple’s life. The treatment for this condition is complex and multidimensional. According to Ayurveda the healthy reproductive status is rooted at the deepest level of physical and mental health. Ayurvedic pre-conceptional care with appropriate diet and regimen nourishes the requisites for a healthy conception which finally paves way towards this ultimate goal. Psychological aspects related to fertility is a very burning issue to the medical professionals worldwide. This in turn creates a vast scope for the alternative systems of medicine, especially Ayurveda. This approach aims for a proper shareera shudhi and mana shudhi (Physical and mental purity). The neuro functional domains of mind in relation to fertility potential is now receiving increased interest which explores the concept ofsoumanasya as told by our acharyas. Despite the technological advances in the field of assisted reproductive techniques like IVF etc, the success rate still remains obscure particularly in those cases with idiopathic infertility with a genetic predisposition. This has turned out to create a paradigm shift towards an integrative medical approach incorporating the holistic concepts of Ayurveda to improve the preganancy outcome. This approach will surely help the society in large. Hope Ayurvedic community is able to respond to this situation!

International Research Exchange Visit – Dr Anandaraman Puthanmadam Venkittarama Sharma visited the University of Milan and Fondazione I.R.C.C.S. Ca’ Granda Ospedale Maggiore Policlinico, Milano, for the continuations of discussions, research idea exchange and planning for developing an integrated approach to management of neuromuscular diseases like Duchene Muscular Dystrophy and Spinal Muscular Atrophy. The project An integrated approach to the treatment of Duchene Muscular Dystrophy and Spinal Muscular Atrophy is Jointly Funded by Italian Ministry of Foreign Affairs and Department of Science and Technology (DST), Government of India.
