Name (Individual, ward or stake)
Individuals will use this form to communicate to their ward canning specialist those item
they wish to order. Ward canning specialist will use this form to communicate their total ward order to the stake canning
specialist. Stake canning specialist will use this form to communicate the total stake order to the Mesa Home Storage
Center. Submit form as hard copy or save as computer document and then send as an attachment via e-mail.Only those who participate in canning the products on their assigned stake canning day may purchase the canned
on Stake Canning Day.
