95921 - Testing of autonomic nervous system function; cardiovagal innervations (parasympathetic function) includes the following: Valsalva ratio and 30:15 ratio.

95922- Includes measurement of beat-to-beat blood pressure and R-R interval changes during valsalva maneuver and at least 5 minutes of passive tilt. This testing involves continuous heart rate recording and requires photoplethysmographic beat-to-beat blood pressure recording. Analysis of heart rate and blood pressure responses reflects the integrity of vasomotor adrenergic innervations.

95924 - Testing of autonomic nervous system function combined parasympathetic and sympathetic adrenergic function testing with at least 5 minutes of passive tilt.

95943 - This code has been established to report when an autonomic function testing does not include beat to beat recording, or for testing without the use of a tilt table. This is a simpler, automated procedure compared to the autonomic codes. Simultaneous, independent, quantitative measures of both parasympathetic and sympathetic function, based on time-frequency analysis of heart variability concurrent with time-frequency analysis of continuous respiratory activity with mean heart rate and blood pressure measures during rest and paced (deep) breathing. Valsalva maneuvers and heads up postural change.

Why perform Autonomic Nervous System & Sudomotor function assessments

Galvanic skin / Sympathetic skin response to electrical stimulation acts as a predictive tool for early signs of distal peripheral neuropathy in people with diabetes by measuring the Sudomotor function.

Diabetes is shown to be the primary cause of small fiber neuropathy and the American Diabetes Association recommends including Sudomotor function assessment in the current panel of diagnostic tests for the detection of peripheral neuropathies.

Autonomic Nervous System testing helps to differentiate progressive diseases and serves as a means of monitoring disease progression and response to treatment.

It can also distinguish primary from secondary autonomic disorders, pure autonomic failure (PAF) or postural orthostatic tachycardia syndrome (POTS).

25% to 50% of patients with symptomatic autonomic dysfunction are likely to die within 1 to 5 years.

Test is performed in less then 10 minutes and is completely non-invasive. Recording includes a base line phase where the patient is relaxed and a testing phase where the patient is asked to perform breathing exercises.

       What is Oxygen Saturation (SpO2)Pulse Oximeter Score

Oxygen saturation is defined as the ratio of oxy-hemoglobin to the total concentration of hemoglobin present in the blood (ie Oxy-hemoglobin + reduced hemoglobin).

Hemoglobin is an iron-containing protein bound to red blood cells and makes up nearly all the oxygen presence (there is a minute amount dissolved in the plasma). Hemoglobin is responsible for transporting oxygen from lungs to other parts of the body, where the oxygen can be used by other cells. Oxy-hemoglobin (HbO2) is the bright red hemoglobin that is a combination of hemoglobin and oxygen from the lungs.

A hemoglobin molecule can carry a maximum of four oxygen molecules. 1000 hemoglobin molecules can carry a maximum of 4000 oxygen molecules; if they together were carrying 3600 oxygen molecules, then the oxygen saturation level would be (3600/4000)*100 or 90%.

Autonomic function testing is covered as reasonable and necessary when used as a analytical tool to evaluate symptoms indicative :

1. To evaluate the presence of autonomic neuropathy in a patient with signs or symptoms suggesting a progressive autonomic neuropathy.

2. To evaluate the severity and distribution of a diagnosed progressive autonomic neuropathy.

3. To differentiate between certain complicated variants of syncope from other causes of loss of consciousness.

4. To evaluate inadequate response to beta blockade in vasodepressor syncope.

5. To evaluate distressing symptoms in a patient with a clinical picture suspicious for distal small fiber neuropathy in order to diagnose the condition.

6. To differentiate the cause of postural tachycardia syndrome.

7. To evaluate change in type, distribution, or severity of autonomic deficits in patients with autonomic failure.

8. To evaluate the response to treatment in patients with autonomic failure who demonstrate a change in clinical exam.
