Since its inception in 2008, the MPSRC has seen tremendous growth and has made significant contributions to the research community.

Throughout it all, the Collaborative has stayed true to its four initial objectives. Our efforts to conduct, translate and disseminate research are clear: we have produced manuscripts, published in peer-reviewed journals, have several abstract publications, received three Meeting, Planning and Dissemination grants from the Canadian Institutes of Health Research, and much of our work has been presented at national and international conferences.

Our goal to promote evidence-based principles has become an important focus. We have organized and facilitated educational workshops and we have conducted numerous Level 1 studies within the Collaborative. The Collaborative has mentored undergraduate students, pediatric surgery residents and pediatric surgery fellows with more undergraduate students already accepted for next year.

We continue to pursue excellence every step of the way and are proud of our accomplishments thus far. We thank you for taking the time to learn about our Research Collaborative.

Pediatric Surgery

Dr. Karen BaileyDr. Karen Bailey is an Assistant Professor in the Department of Surgery, the Director of the Pediatric Trauma Program and the Pediatric Surgery Clerkship Coordinator. Dr. Bailey joined McMaster University in January 2010 after completing a Pediatric General Surgery clinical research fellowship at The Children’s Hospital of Eastern Ontario. Her research interests include childhood obesity, bariatrics, global health, trauma and intestinal motility.

Dr. Brian CameronDr. Brian Cameron is a Professor in the Department of Surgery and Director of the McMasterInternational Surgery Desk (ISD). Dr. Cameron has done extensive work in Guyana and continuesto play an important role in surgical training in Guyana through his involvement in the CanadianNetwork for International Surgery (CNIS). Dr. Cameron’s research interests include qualitative work inHirschsprung’s disease, surgical clerkship professionalism, as well as quantitative research in international surgical training and program development.

Dr. Peter Fitzgerald Dr. Peter Fitzgerald is the President of the McMaster Children’s Hospital and a Professor in the Department of Surgery. He has been a Pediatric Surgeon at McMaster Children’s Hospital for the past 20 years and is also the Executive Lead of the Women’s Health and Newborn Care Program and the Director of the MPSRC. Dr. Fitzgerald has an interest in minimal access surgery and reconstructive chest wall surgery. His other research interests include evidence-based surgery reviews, assessment of evidence-based practices in pediatric surgery and surgical education.

Dr. Helene FlageoleDr. Helene Flageole is a Professor in the Department of Surgery with a joint appointment in the Department of Pediatrics. She is also the Chief of the Department of Pediatric Surgery as well as the Program Director for Pediatric Surgery. Dr. Flageole joined McMaster University and the Children’s Hospital in 2007 after spending 11 years in the Department of Surgery at McGill University in Montreal. Dr. Flageole’s clinical research interests include pyloric stenosis, congenital birth defects and research education.

OUR STAFFThe MPSRC was formed in October 2008 by a group of pediatric surgeons dedicated to advancing surgical care for children through research activities that are evidence-based, rigorous, accountable, and translatable.

Since then, the Collaborative has grown to include members in the Divisions of Urology and Ophthalmology.

Dr. Luis BragaDr. Luis Braga is an Associate Professor in the Department of Surgery and the Research Director of the Urology Residency Program. He also holds an Associate Appointment in the Department of Clinical Epidemiology and Biostatistics. Dr. Braga joined McMaster University’s Department of Surgery in 2008 after completing a three-year fellowship at the Hospital for Sick Children in Toronto. Dr. Braga has published extensively in the pediatric urology literature and has completed his Master’s Degree in Health Research Methodology. Dr. Braga is interested in studying the effects of antibiotic prophylaxis on frequency of febrile urinary tract infections in infants as well as hypospadias and orchidopexy.

Dr. Jorge DeMariaDr. Jorge DeMaria is a Professor in the Department of Surgery and an Associate member of theDepartment of Pediatrics at McMaster. Dr. DeMaria has an extensive history in Hamilton and withMcMaster University. He has been a leader in pediatric urology and continues to advance the fieldthrough his interest in ongoing refinement of pediatric urological surgical techniques, patient outcomesand surgical education.

Dr. Kourosh SabriDr. Kourosh Sabri is an Assistant Professor in the Department of Surgery. Dr. Sabri joined McMaster University’s Department of Surgery in January 2010 after working as an Assistant Professor at the University of Alberta in Edmonton for 3 years. In 2006, Dr. Sabri completed his fellowship at the Hospital for Sick Children in Pediatric Ophthalmology and Strabismus. His research interests include retinopathy of prematurity, strabismus, telemedicine and resident education.

Dr. Mark WaltonDr. Mark Walton is a Professor in the Department of Surgery and the Assistant Dean of PostgraduateMedical Education for the Michael G. DeGroote School of Medicine. Dr. Walton joined McMaster University in 1993 after completing his residency in Pediatric Surgery at the Children’s Hospital of Eastern Ontario. Dr. Walton completed his Surgical Teaching Fellowship at Izaak Walton Killam Children’s Hospital in Halifax, Nova Scotia. His research interests include medical education, resident evaluation, minimally invasive surgery and quality of life studies.

Julia Pemberton, MSc, PhD(c), Research CoordinatorJulia manages, coordinates and plays a pivotal role in all research activities for the MPSRC. She also coordinates all the interested and current MPSRC Staff and Interns for the Collaborative. Julia joined the Department of Surgery in 2007. She has completed a Master’s degree in Health Research Methodology from the Department of Clinical Epidemiology and Biostatistics and is currently a PhD candidate. Julia is experienced in the design, implementation and coordination of both quantitative and qualitative methods, as well as evidence-based medicine principles.

Jessie Heaman, BSc, Research AssistantJessie plays an essential role in clinical research projects in the Department of Urology, including coordination of the ALPHA trial with Dr. Braga and three clinical trials with Astellas Pharma. Jessie is a graduate of McMaster University and began working for the MPSRC in April 2012.

Virginia Viscardi, BSc, Research AssistantVirginia plays a key role in organizing and conducting clinical research for Pediatric Ophthalmology. Virginia joined the Collaborative in August 2012 after completing a Bachelor of Science in Life Science from McMaster University.

David Kanters, BSc, Research AssistantDavid’s work includes Dr. Braga’sprospective Hypospadias OutcomesLong-term Database (HOLD), as well asclinical research projects in the Divisionsof Urology and Pediatric Surgery. David isa graduate of the University of Waterloowho joined MPSRC in the summer of 2012after completing his degree in BiomedicalSciences.

Dr. Khalid Al-Harbi, Pediatric General SurgeryDr. Khalid Al-Harbi is an associatemember of the MPSRC, currently working at King Faisal Specialist Hospital and Research Centre in Jedda, Saudi Arabia. Dr. Al-Harbi’s research interests include evidence-based surgery reviews, pediatric bariatric surgery, and quality of life research.

Dr. Dan Poenaru, International Pediatric SurgeryDr. Poenaru was a Visiting Professor at McMaster in 2011-2012 and remains a collaborator with the MPSRC. He has worked as a volunteer surgeon in Kijabe, Kenya, where he has started the BethanyKids pediatric surgical unit within the AIC Kijabe Hospital, and now works in Addis Ababa in Ethiopia. Dr. Poenaru has collaborated with the MPSRC on projects studying the global burden of disease and surgical outcomes in low- and middle-income countries.

Current MPSRC Interns

An exceptional group of learners has contributed greatly to the success of current and past MPSRC projects. Many have received recognition for their work in the form of authorship on MPSRC publications and presentations resulting from their work on projects or senior theses.

Our students have the opportunity to be involved in the clinical researchprocess, receive training and take an active role in patient enrollment.Often, students’ responsibilities include collecting, entering, and managingresearch data. Opportunities also exist for students to prepare and deliverresearch presentations and apply epidemiologic knowledge learned in theirundergraduate coursework.

A Commitment to Excellence in Research

International Surgery

Several members of the Division of Pediatric General surgery traveloverseas to conduct research, including Dr. Bailey, Dr. Cameron andDr. Poenaru. Dr. Cameron directs the McMaster International SurgeryDesk, an MPSRC partner organization dedicated to the growth anddevelopment of international surgery. Dr. Poenaru works in Ethiopiaas a full-time volunteer pediatric surgeon and collaborates withthe MPSRC on international research projects such as DAPS. Theongoing effort of these individual surgeons continues to expand theimpact of MPSRC research initiatives.

Education

The Division of Pediatric Surgery will be conducting a number of research studies in the near future on educational topics including assessment of surgical skills, professionalism in surgical clerkship and postgraduate training in evidence-based surgery.

The Division of Pediatric Surgery at McMaster Children’s Hospital is committed to quality patient care, education and research. We are fortunate to take care of a broad range of pediatric surgical conditions.

MPSRC faculty within the Division of Pediatric Surgery conduct a wide variety of clinical research that incorporate several research paradigms including mixed methods, quantitative and qualitative methodologies.

The recent and upcoming presentations and publications resulting fromthe POINTS, CARE, DAPS and ACT NOW. studies showcase the benefits of the rigorous methodology and high level of excellence of research in the MPSRC.

The Division of Pediatric Surgery has been successful in securing funding from the Canadian Institutes of Health Research and the McMaster Children’s Hospital Foundation.

Dr. Helene Flageole is the Principal Investigator for this trial. The objective is to determine the effect of a pre-operatively placed nasogastric tube, compared to no nasogastric tube, on postoperative emesis rates and postoperative length of stay in infants with a primary diagnosis of pyloric stenosis treated with pyloromyotomy. The POINTS trial has completed enrollment with 50 total patients and is currently in the data analysis phase.

Through the POINTS Trial, Dr. Flageole is providing strong evidence for the feasibility of randomized control trials in pediatric surgery.

Dr. Mark Walton is the Principal Investigator for the CAREstudy. The focus of this prospective repeated measurescohort study is to determine the impact of a surgically inserted gastric feeding tube in children on caregiver quality of life.

This study was conducted between November 2009 andMarch 2012 with a total enrollment of 31 patients. Thefindings of the CARE study were recently presented at theCanadian Association of Paediatric Surgeons (CAPS) 2012Annual Meeting.

Dr. Karen Bailey is the Principal Investigator for the ACTNOW project.

This choice based conjoint survey is studying a sample of physicians from across Canada who provide care for obese children. The objective of this study is to understand these physicians’ attitudes, preferences for knowledge acquisition, knowledge of obesity, treatment options, patient demographics, and treatment referral patterns of obese children and adolescents in Canada.

In 2012, Dr. Bailey presented the findings from the qualitativephase of the study at both the American Society for Metabolic and Bariatric Surgery and the CAPS 2012 Annual Meetings.

This study focuses on understanding the global burdenof disease in children in Kenya and Canada by determining Disability Weights for pediatric congenital surgical conditions. Disability Weights are a necessary measurement needed to calculate the Disability Adjusted Life Years (DALY) associated with living in a specific disease state and are used to determine the disability averted by pediatric surgery.

The persons interviewed for this study include physicians,nurses, allied health professionals, and others caring forchildren with congenital diseases.

Worldwide Research

Dr. Braga plans to expand his prospective Hypospadias OutcomesLong-term Database (HOLD) to include participating pediatricurologists across North America, South America and Europe. Dr. Braga also plans to expand the ALPHA trial to includeparticipating centres across Canada and the United States.

Dr. Jorge DeMaria has been practicing pediatric urology at McMasterChildren’s Hospital for over 25 years. Dr. DeMaria’s experience andexpertise are invaluable to the MPSRC. His current research on neonatalcircumcision practice in the community setting promotes the use ofmultimedia educational tools he and his research team are developing to increase patient satisfaction, reduce complication rates, and work to educate community practitioners.

Since his arrival at McMaster, Dr. Braga has raised the bar for clinical research with his groundbreaking studies on treatment for antenatal hydronephrosis and long-term outcomes of hypospadias surgery, in addition to running industry-sponsored clinical trials. Dr. Braga continues to solidify his status as a top researcher in pediatric urology with his innovative work.

The Division of Pediatric Urology has received a number of awardsfor their work. The systematic review “Laparoscopic Pyeloplasty forPatients with Ureteropelvic Junction Obstruction: Effect on OperativeTime, Length of Hospital Stay and Postoperative Complications” waschosen by the prestigious Database of Abstracts of Review and Effects(DARE) of the University of Oxford in 2010. Research from pediatricurology has twice been selected by the Faculty of 1000, an organizationthat identifies and recommends the most important articles and medicalresearch publications. Most recently, a systematic review on “AntibioticProphylaxis for Urinary Tract Infections in Antenatal Hydronephrosis” was recommended as being of special significance.

Urotherapy

Dr. Luis Braga is the Principal Investigator for the ALPHA trial and the project is coordinated by Jessie Heaman. The overall objective of this study is to determine the effect of antibiotic prophylaxis vs. placebo in reducing the number of febrile urinary tract infections in infants with antenatally detected hydronephrosis over the first year of life. The ALPHA trial has recently expanded to include the Children’s Hospital at London Health Sciences in London, Ontario and The Hospital for Sick Children in Toronto, Ontario with funding secured from the Physicians Services Incorporated Foundation. The ALPHA trial is the first placebo controlled trial to be conducted in pediatric urology in Canada.

Dr. Braga supervises Natasha Brownrigg, the Principal Investigator for the Urotherapy trial and a Nurse Practitioner at McMaster Children’s Hospital. The Urotherapy trial studies children with non-neurogenic lower urinary tract dysfunction (NLUTD) and/or dysfunction elimination syndrome (DES). The purpose of this randomized controlled pilot trial is to assess the feasibility of group urotherapy session versus standard individual urotherapy and to evaluate the preliminary effectiveness of this modality. In addition, new and valuable knowledge will be gained on the child’s perception of their condition and the effect of urotherapy on their quality of life.

Welcome to Our Team, Dr. Sabri!

Expanding the Boundaries of Patient Care

Dr. Sabri’s upcoming telestrabismus study will investigate the use oftelemedicine to diagnose and manage strabismus in pediatricpatients. The use of telemedicine in ophthalmology provides ameans for remote communities to access subspecialty services, likeophthalmology, which are unavailable in those areas.

This will ensure patients across Canada receive the same level oftreatment and access to ophthalmologists, regardless of geography.

Dr. Sabri will also be starting a study in the coming year on visionscreening in aboriginal children entitled First Aboriginal Children’sEye Screening (FACES) Study. This project will be funded by theCanadian Institutes of Health Research.

Dr. Sabri is the newest member of the McMaster Pediatric SurgeryResearch Collaborative. Dr. Sabri joined the MPSRC in April 2012, andhas a very successful history in clinical research.

Virginia Viscardi, a McMaster graduate, is the Research Assistantwith Dr. Sabri and the Division of Ophthalmology. Virginia currentlycoordinates Seminal Canadian Recommendations for Evidence-basedExamination of Neonates (SCREEN-ROP) as well as the MPSRCinterns working on research projects in Ophthalmology.

Dr. Sabri has received a number of successful grants for his research.For his SCREEN-ROP study, Dr. Sabri was awarded funding from theCanadian Institutes of Health Research, McMaster University, and theOntario Ministry of Health and Long Term Care.

Dr. Sabri’s excellent research in Ophthalmology is a welcome additionto the MPSRC.

The MPSRC’s newest member, Dr. Kourosh Sabri, is the Principal Investigator of this study, headed by McMaster University and funded by the Canadian Institutes of Health Research.

The primary objective of this study is to develop evidence-based recommendations for retinopathy of prematurity (ROP) screening that minimize the number of patients being screened as well as associated costs and morbidities while still capturing all nascent cases of ROP. This study is in partnership with the Canadian Neonatal Network™ and involves all 30 Level-3 Neonatal Intensive Care Units in Canada. The SCREEN-ROP Study is designed to develop new evidence-based screening guidelines for ROP that will reduce that number of patients screened while still capturing all patients with severe ROP requiring treatment. The ultimate goal of these new guidelines is to reduce the morbidity associated with ROP screening on the patient and parents as well as capture every single patient with severe ROP.

This day and a half workshop in May 2011 was organized by Dr.Cameron and focused on building sustainable research capacityand priorities in Guyana. This workshop brought together surgeonsfrom both Canada and Guyana, surgical education experts and Non-Governmental Organizations to develop a research agenda in Guyana.The objectives were to use high-fidelity simulation for teachingtrauma skills and to evaluate Canadian Network for InternationalSurgery (CNIS) training programs. The impact of this workshop wasprofound. The Guyana-Canada partnership is thriving and throughthis meeting a foundation for future research partnerships wasestablished.

A McMaster surgical team visited Guyana for 3 weeks in November2011 to teach pediatric surgery, run 5 days of CNIS trauma andsurgical skills courses, provide surgical care for over 50 childrenand present papers at the annual Scientific Conference. The CNISCourses were partially funded through a Surgical AssociatesEducation Grant from McMaster University, Department of Surgery.The purpose of these courses was to train personnel at the technical level to apply the principles of trauma management and essential surgical skills at centers with limited resources.

WORKSHOPSThe MPSRC has conducted five educational workshops over the past three years to promote and translate evidence-based surgery principles.

Trauma Team Training (TTT) Update Course

A full day meeting was held on February 18th, 2011 which focused on building an active, meaningful and evidence-based researchcollaborative in pediatric surgery across Canada. The objectives for this meeting were to identify major barriers to increasing the level of evidence in pediatric surgery and strategize solutions. Action plans were developed to address these barriers and a knowledge dissemination strategy was discussed.

A half day educational workshop was held in October 2012, utilizing a previously piloted circumcision video, a podium presentation and a panel discussion to increase clinical knowledge and understanding of neonatal circumcision. The workshop also provided an engaging, multidisciplinary networking opportunity amongst pediatric healthcare providers. The information presented in this workshop is available on the MPSRC website in an online training module designed by Dr. DeMaria.

This workshop was held in October 2010 through collaboration between the McMaster Pediatric Surgery Research Collaborative and the International Surgery Desk. Participants from Canada and Guyana attended this two-day introductory educational opportunity. The goal of this course was to help international surgeons incorporate evidence-based medicine principles into their work and included sessions on finding and evaluating evidence, research methodology, international surgery research, distance learning and web conferencing.

This clinical trial, sponsored by Astellas Pharma Europe B.V, aims to study the long-term safety, efficacy, and pharmacokinetics of Solifenacin Succinate Suspension, a new treatment for pediatric patients with overactive bladder (OAB) or Neurogenic Detrusor Overactivity (NDO). This study is being conducted in three different sub-studies called the Lion, Leopard, and Monkey studies. Each sub-study examines the study drug in a specific population ofpediatric patients. The STEP database, created at the Children’s Hospital Boston, aims to create a web-baseddata registry for all patients worldwide who undergo the serial transverse enteroplasty(STEP) procedure. Analysis of the data from the STEP registry will be used to determine a baseline diagnostic, demographic, anatomical and nutritional profile for patients who undergo the STEP procedure. Dr. Helene Flageole is the Director of thePediatric Surgery Residency Programoffered at McMaster Children’s Hospital.This 2 year program is fully accredited by theRoyal College of Physicians and Surgeons ofCanada.

McMaster Children’s hospital servesa population basin of over 2 million people

in Southern Ontario and is home to one of the largest and most modern neonatal units in Canada.

In the 2 year curriculum, residentsspend 18 months in the pediatric surgery service and 6 monthsin off-service rotations includingneonatal ICU, pediatric ICU andpediatric radiology. Residents alsohave an opportunity to spend 1-2months in clinical research activitiesor other elective training.

From the beginning of their residency, trainees are introduced to the MPSRC. They are encouraged and supported at either joining existing projects or developing new ones. Since 2008, each pediatric surgical resident has completed a research project, with many resulting in presentations at scientific conferences and publication in peer-reviewed journals. The very academic nature of most faculty positions in pediatric surgery fuels our commitment to make quality research a training priority.

This map shows the far reaching involvement of the McMaster Pediatric Surgery Research Collaborative in the scientific community. Each location on the map below is a city where the MPSRC has given a podium presentation, presented an abstract or presented a poster at a conference. In the future, the MPSRC plans to expand our international research collaboration by including pediatric urologists worldwide in the prospective Hypospadias Outcomes Long-term Database (HOLD), expanding the ALPHA trial to centers in Canada and the United States, and continuing our partnership with the International Surgery Desk in our joint research endeavors.

International Research ProjectsThe MPSRC is currently conducting two international research projects. The DAPS study focuses on studying the global burden of pediatric disease in both Kenya and Canada. The Trauma Team Training (TTT) update course was organized and conducted by MPSRC surgeons in Guyana.

The MPSRC was founded through generous support from the McMaster Children’s Hospital Foundation in 2008. Since then, the Collaborative has been successful in securing additional funding from both the McMaster Surgical Associates and Hamilton Health Sciences. Both Dr. Bailey and Dr. Braga were awarded the Hamilton Health Sciences New Investigator Fund. Dr. Sabri has been successful in securing funding for SCREEN-ROP from the Canadian Institutes of Health Research and the Ontario Ministry of Health and Long-Term Care. In 2012, Dr. Braga was awarded funding from the Physicians Services Incorporated Foundation for the ALPHA trial, which has enabled the trial to expand to other pediatric urology centres in Ontario.

The MPSRC relies entirely on research funding. All MPSRC research projects are supported by one of the funding institutions listed below. The MPSRC would like to extend a sincere thanks to each of its supporters.

The McMaster International Surgery Desk (ISD) was launched in June 2010 by Dr. Brian Cameron to facilitate international surgery activity within the Department of Surgery and collaborate with like-minded disciplines and colleagues within McMaster University and the broader Canadian surgical community.

In its short history, the ISD has conducted four educational workshops for international surgeons, hosted two visiting surgeons from Nigeria, given eleven presentations at scientific conferences and has helped six different surgical residents at McMaster find and complete electives abroad.

ServiceThe ISD strives to support and promote serviceto low- and middle-income countries. Twenty-sixdifferent countries have been visited by McMaster surgeons volunteering countless hours performing surgery on both adults and children.

Knowledge GenerationThe ISD is dedicated to its ongoing contributionsto knowledge generation in internationalsurgery. The organization is partnering on threecurrent MPSRC projects and continues to produce high quality research evidence on a range of global surgery topics.

EducationThe ISD has been involved in a number of international surgery education initiatives. Two workshops were conducted in a November 2012 visit to Guyana by Dr. Karen Bailey and Julia Pemberton on Trauma Simulation and Evidence-based Research Methodology.

The Canadian Coalition for Global Health Research (CCGHR) is a not-for-profit organization with an orientation towards health research challenges in low- and middle-income countries. The Coalition includes global health researchers, organizations interested in funding health research, and those actively engaged in applying research to improve global health.

The Canadian Network for International Surgery is an organization that sends doctors to Africa to train essential lifesaving skills to the African people. In addition to sending surgical tools and training to Africa, the CNIS teaches kids conflict resolution, builds labs and sends books and information. The goal of the CNIS is empowering low-income countries to create an environment where the risk from injuries is minimal and all people receive adequate surgical care.

The McMaster Faculty of Health Sciences, Global Health Office (GHO) is a global program that provides students with a solid foundation in global health issues, web-based learning and experience in low- and middle-income countries. The ISD is also developing a new partnership with the Faculty of Health Sciences Master’s in Global Health Program, investigating a possible collaboration with Gulu University in Uganda.

The Bethune Round Table (BRT) is an international round table conference devoted to issues in the provision of surgical care in low resource settings and promoting new ideas for the further development of global surgery. This unique international conference is held annually in Canada and brings together speakers and attendees from around the world to share research, education and clinical experiences in low- and middle-income countries.
