1 Context
Business Context
PEN Clinical Audit Tool, is a clinical audit tool that allows practices to analyse data, devise the
necessary strategies to improve patient care and report on quality improvement activities
undertaken by the practice. In short, it optimises patient health outcomes while enhancing business
capabilities for the general practice.
In order to do this, the practices shall provide the required data captured in their Clinical
Management System to PENCAT.
This document describes the installation of the PENCAT Data extraction tool. This tool helps in
mapping the Data elements of the Clinical Management system and data elements used in the
PENCAT and allow to extract the data in the XML format. This XML files shall be uploaded in the
specified path. After which the XML files will be imported in PENCAT tool to generate the required QI
Measures.

Medtech Evolution PenCAT Extractor Installation & Configuration
1.3.1 Pre-Requisites
1. Run the PenCAT Extractor installer file by double-clicking and follow the below instructions
to complete the installation. Clicking on the Installer file will open the below window.
Click Next in the below window.

2. The installation folder should be pointing to the application installed path.
3. Installation successful message shall be displayed after the completion of Installation.

4. After the installation, “MT2PENCAT.exe” shall be available in the path “<Evolution
installation directory>\Bin\Addins\PenCAT\”.
5. Login to the Medtech Evolution application Click on Utilities Menu – The PENCAT
extractor tool shall be available as shown in the below screen. - Click on PenCAT sub-menu.
1.3.2 Functional Verification
6. The system displays ‘MT2PENCAT’ window with the list of database names available in the
“MTAUEVO.INI” file.

7. Choose the required database click Start System displays “MT2PENCAT Selection”
window
7.1. The window displays instructions for Mapping, Note and Extraction respectively.
7.2. Also, Mapping and Extraction buttons.
8. Click on the Mapping button to view the “MT2PENCAT Mapping” window:

8.1. MT2PENCAT window contains the following tabs: Measurements, Immunization and
Smoking /Alcohol
8.2. In Measurements tab system displays Data provided by PenCAT in the first column.
8.2.1. In the second column, system displays all the Screening Terms available in
the Medtech Evolution database.
8.2.2. In the third column, system displays corresponding Measurement values for
every Screening Term available in the second column.
8.2.3. To map select the required data elements in the first column and second
column. When user selects data elements in the second column the corresponding
measurements are displayed in the third column. Select the required measurement
values.
8.3. Follow the same steps mentioned above (step 9.1) for “Immunisations” and
“Smoking/Alcohol” tabs.

8.4. After completing the mapping click on ‘Save’ button to save the mappings in the
database and click on Exit to close Mapping window
Extraction Process
9. Click Extraction button The “MT2PENCAT Requirements” window displays pre-requisites
checklist.
9.1. Make sure that all the pre-requisites mentioned in the window have been met.
10. Select the checkbox “I confirm that the pre-requisites have been met” and click Next
button.

System displays “MT2PENCAT Extraction” window.
10.1. System displays following details,
Database name is in “Database” field,
Location Code is displayed in the “Location” field,
Extraction Directory field displays the path.
10.2. In Extraction Options group box choose required filters
“Active Patients Only” or “All Patients (Active and Inactive)”.
10.3. In Run Options group box choose required filters.
“Full Extract” or “Test (Only 200 Records)”.
10.4. Click on the Extract Data button to extract the data patients’ data will be
collected and extracted in the form of XML.

10.5. After the completion of the extraction “Extraction Start” time and “Extraction
Finished” time will be displayed click Exit to close PenCAT window.
11. in the specified directory mentioned in the above screen contains the following extracted
XML files:
“MedicalMicrodata-<DB Name_Location Name-Date & Time>-v1_2-link.xml” and
“MedicalMicrodata-<DB Name_Location Name-Date & Time>-v1_15-data.xml”
12. Verify the structure of the Link and Data XML files with the test cases available in Test Link.

PENCS Software & Data Statistics:
1. Install PENCS software from following website:
http://install.pencs.com.au/clickonce/ClinicalAudit/publish.htm
2. After successful installation, launch the “CAT4” software and enter the required login
credential:

3. On the Pen CS CAT4 software, navigate to “Edit Preferences” and configure as below in
the Clinical Audit Tab and Medtech32 Tab

4. After successful configuration, click on “Collect” to populate the statistics.
5. When the data is populated on the PenCS tool then the link and data xml files will be
converted into .bak files in the specified folder as shown in the below figure.
PIP QI Improvement Measures
Below are the screen navigations for different measures:
1.5.1 QIM 1 - Proportion of patients with diabetes with a current HbA1c result
The screen navigation for obtaining the data in Pen CS is as follow:
In Medtech Evolution –Select the required patient.
Click on Module Clinical New Patient Classification screen.
Capture patient classification as ‘Diabetes Type 1’ or ‘Diabetes Type 2’ as shown in
the below figure.

A screening term ‘HBA1C’ (Master Value) shall be created in Setup Recall/Screening
Screening Terms. The description and measurement values shall be added
accordingly.
Once the ‘OK’ button is clicked, the added screening term displays in the ‘Screening
Term’ List screen.
User shall capture HBA1c Value for the patient.
Go to Module Recall/Screening Patient Screening New Screening Entry the
below screen is displayed.

Select the screening term ‘HBA1C’ (which was created under the Screening terms) in
Code drop-down screen. Enter the result in the text box.
On clicking the ‘OK’ button shall save the screening entry and the result shall be
displayed in the ‘Screening History’ screen as shown below.
Now in PENCAT, Mapping and Extraction of data shall be done as shown below.
Once the data is mapped and extracted, the data shall be imported in Pen CS tool.

Login to the PenCAT tool and click on the ‘Collect’ button. Click on HBA1C tab. System displays the
graph as shown below.
1.5.2 QIM 2- Proportion of patient with Smoking Status
A screening term ‘Smoking History’ under Setup Recall/Screening Screening Terms shall be
created as shown below. The description and measurement values will be added accordingly.
Once the ‘OK’ button is clicked, the added screening term displays in the ‘Screening Term’
screen.

User needs to capture smoking status for the patient.
Select the patient and click on Module Recall/Screening Patient Screening New
Screening Entry the below screen is displayed
Select the Smoking History in the Code Dropdown.
The system displays all the measurements created for Smoking History.
Select the required value.
On clicking the ‘OK’ button shall save the screening entry and the result will be displayed in
the ‘Screening History’ screen.
Now PENCAT Extraction tool shall be opened. Map the required smoking status as shown below.

Once the data is mapped and extracted in XML format
Go to PenCAT tool. Click on the Collect Data option.
Select the required filters in the View filters tab.
Click on the Smoking tab. System displays the graph as shown below.

1.5.3 QIM 3- Proportion of patients with Alcohol consumption
A screening term ‘Alcohol’ under Setup Recall/Screening Screening Terms shall be created as
shown below. The description and measurement values will be added accordingly.
Once the ‘OK’ button is clicked, the added screening term displays in the ‘Screening Term’
screen.
User needs to capture alcohol status for the patient.
Select the patient and click on Module Recall/Screening Patient Screening New
Screening Entry the below screen is displayed
Select the Alcohol in the Code Dropdown.
The system displays all the measurements created for Alcohol.
Select the required value.
On clicking the ‘OK’ button shall save the screening entry and the result will be displayed in
the ‘Screening History’ screen.

Now PENCAT Extraction tool shall be opened. Map the required smoking status as shown below.
Once the data is mapped and extracted in XML format
Go to PenCAT tool. Click on the Collect Data option.
Select the required filters in the View filters tab.
Click on the Alcohol tab. System displays the graph as shown below.
