MyBenefits – Open Enrollment User Guide This guide will help you navigate MyBenefits, the University’s online benefits enrollment application. All benefit-eligible faculty and staff must actively enroll in a University healthcare plan or waive coverage between October 17 th and November 3 rd , 2017. Let’s get started! What You Will Need Your netID, password, and your election decisions. Name and address of your life insurance beneficiaries. If adding a spouse or other dependent(s), you’ll need each person’s full legal name, date of birth and social security number. If enrolling in a flexible spending account, you’ll need to provide banking information, including name, routing and account numbers. Dependent verification documentation (see note below). MyBenefits – Open Enrollment User Guide

This guide will help you navigate MyBenefits, the University’s online benefits enrollment application. All benefit-eligible faculty and staff must actively enroll in a University healthcare plan or waive coverage between October 17th and November 3rd, 2017. Let’s get started! What You Will Need

• Your netID, password, and your election decisions. • Name and address of your life insurance beneficiaries. • If adding a spouse or other dependent(s), you’ll need each person’s full legal name, date of birth and social

3. Landing Page – If you have a qualifying event that will result in a change of benefit coverage for the remainder of

2017, click the link that says “Initiate QE during OE workflow”. This will allow you to complete your 2017 elections before completing the 2018 Open Enrollment. Otherwise, review the welcome message, then select Continue.

4. Dependent Verification Process – Review the dependent verification requirements, then select Continue.

5. Summary Plan Description (SPD) Opt Out - Review the SPD Opt Out message, then select Continue.

6. Demographics – Review your personal demographic information and select your Marital Status.

If any personal demographic information is incorrect, please contact the askHR Customer Service Center at [email protected] or 574-631-5900. Employees are unable to edit their own demographic information in the online application. Select Continue to proceed to the next step in the enrollment process.

7. Spousal Information If you are single or your current spouse’s information is already displayed, select Continue to proceed to the next step in the enrollment process. To enroll your spouse, click Add to enter their biographical information.

Complete the highlighted fields, then select Save & Continue.

Check the appropriate box and answer the addition questions if selecting “yes”, then select Save & Continue.

9. Child Information If you do not have dependent child/children or their information is already displayed, select Continue to proceed to the next step in the enrollment process. To enroll dependent child/children, click Add to enter their biographical information.

Complete the highlighted fields. If entering one child, select Save & Continue to proceed to the next step in the enrollment process.

To enter additional children, select Add Another Child. Repeat this process until all children have been added, then select Save & Continue to proceed to the next step in the enrollment process.

10. Enroll Now – Medical Insurance Step 1. Select the check box next to each dependent you wish to cover on your Medical Plan within the “Members Covered” section.

Step 2. To elect your Medical Plan, select Enroll within the plan window you are selecting.

To view additional information for each plan, including a benefits overview and links to search a list of in-network providers, select Benefit Description.

To waive Medical Plan coverage, select Enroll within the Medical Waiver window.

11. Enroll Now – Dental Insurance Step 1. Select the check box next to each dependent you wish to cover on your Dental Plan within the “Members Covered” section.

Step 2. To elect your Dental Plan, select Enroll within the plan window you are selecting.

12. Enroll Now - Vision Insurance Step 1. Select the check box next to each dependent you wish to cover on your Vision Plan within the “Members Covered” section.

To view additional information about the vision plan, including a benefits overview and links to search a list of in-network providers, select Benefit Description.

To waive Vision Plan coverage, select Waive this benefit in the upper right corner of the page.

13. Enroll Now - Base Life Insurance The University provides a $25,000 base group term life insurance policy through Securian to eligible faculty and staff. You will be automatically enrolled in this plan. Select Continue to proceed to the next step in the enrollment process. To view additional information about the base life policy, select Benefit Description.

14. Enroll Now – Supplemental Life Insurance Supplemental life insurance is purchased in multiples of your annual salary, from 1 to 10 times up to a maximum of $1.5 million. You may increase your supplemental life insurance coverage by one times your annual base salary without completing an Evidence of Insurability (EOI) form, if your elected total amount is under $750,000. Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the supplemental life policy, select Benefit Description.

To waive Supplemental Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

15. Enroll Now – Spousal Life Insurance You may elect either $12,500 or $25,000 in life insurance coverage for your eligible spouse. Note: If both you and your spouse are employed by the University, you may not be double-covered. That is, if you are covered under the University’s base life insurance plan, you may not also be covered as a spouse.

Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the spousal life policy, select Benefit Description.

To waive Spousal Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

16. Enroll Now – Child Life Insurance You may elect either $5,000 or $10,000 in life insurance coverage for your eligible child(ren). Note: If both you and your spouse are employed by the University, one (not both) may insure your eligible child(ren). Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the child life policy, select Benefit Description.

To waive Child Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

17. Accident Insurance Step 1. Select the check box next to each dependent you wish to cover with Accident Insurance within the “Members Covered” section.

Step 2. To elect Accident Insurance, select Enroll within the plan window you are selecting. To view additional information about Accident Insurance, including a benefits overview, select Benefit Description.

To waive Accident Insurance, select Waive this benefit in the upper right corner of the page.

18. Critical Illness Insurance Step 1. Select the check box next to each dependent you wish to cover with Critical Illness Insurance within the “Members Covered” section.

Step 2. To elect Critical Illness Insurance, select Enroll within the plan window you are selecting. To view additional information about Critical Illness Insurance, including a benefits overview, select Benefit Description.

To waive Critical Illness Insurance, select Waive this benefit in the upper right corner of the page.

19. Enroll Now – Health Care Flexible Spending Account The health care flexible spending account allows you to set aside pre-tax dollars (up to $2,550.00 annually) to pay for eligible health care expenses.

Enter the annual coverage amount, then select Enroll to proceed to the next step in the enrollment process. To view additional information about health care flexible spending accounts, select Benefit Description. Note: the amount shown under “Employee Cost” is prorated based on the number of months left in the current calendar year. This will be the amount that is deducted from your pay check each month.

To waive health care flexible spending, select Waive this benefit in the upper right corner of the page.

20. Enroll Now – Dependent Care Flexible Spending Account The dependent care flexible spending account allows you to set aside pre-tax dollars (up to $5,000.00 annually) to pay for eligible dependent care expenses so that you or your spouse can work. Enter the annual coverage amount, then select Enroll to proceed to the next step in the enrollment process. To view additional information about dependent care flexible spending accounts, select Benefit Description. Note: the amount shown under “Employee Cost” is prorated based on the number of months left in the current calendar year. This will be the amount that is deducted from your pay check each month.

To waive dependent care flexible spending, select Waive this benefit in the upper right corner of the page.

21. Providing Direct Deposit Information

If you enroll in either flexible spending account, you are required to provide direct deposit information for reimbursement of these funds. Complete the required fields in red, along with selecting the type of account from the “Account Type” drop down menu.

After completing the required fields, select the appropriate box, then select Save & Continue.

22. Uploading Documents

If enrolling new dependents for the first time, such as a spouse or child, you must present documentation to the Office of Human Resources to verify each dependent’s relationship on or before December 9th, 2016. Traditionally these documents were to be provided to the Office of Human Resources in person, but now they must be submitted through MyBenefits. Please review the University’s dependent verification document at https://hr.nd.edu/benefits/group-insurance/dependent-verification-for-university-benefits for a list of acceptable documents. To upload a document, click on the Browse button to locate the scanned document on your local computer. Select the document, then click OK.

Once the document appears in the white box in the middle of the page, click Upload to upload it to the system. You can view the documents that were successfully submitted by clicking the View Uploaded Document(s) button at the bottom of the page. Once complete with your documents, select Save & Continue. Note: Documents will still be accepted in person at the Office of Human Resources.

23. Employee Beneficiary Designation

You are required to designate beneficiaries for your life insurance policies. If you entered your dependents’ information earlier, they will automatically be provided as options for designation within the drop-down menu. If you do not have a spouse or child(ren), or wish to name someone else as a beneficiary, you can select the relationship from the available options and provide their contact information.

You have the ability to designate multiple primary beneficiaries. Click on the “+” icon to add another primary beneficiary. Percentages must equal 100% to continue. You may also designate multiple secondary, or contingent beneficiaries. The process for adding these is the same as adding primary beneficiaries. To begin, select Add Secondary Beneficiary.

If you wish to apply the same beneficiaries from your base life plan to your supplemental life plan, you can select Assign same beneficiaries to all plans. All beneficiary designations will be replicated for you automatically. When beneficiary designations are complete, select Save and Continue to proceed to the next step in the enrollment process.

24. Confirmation Statement

This page allows you to review both your current and future benefit elections prior to submitting the enrollment request to the Office of Human Resources for approval and processing. The Future Enrollment Summary will reflect your 2018 benefit elections. The future enrollment summary does not include the Health Screening medical plan credit. This amount will be displayed on your confirmation statement in December 2017. Once you have confirmed your elections are correct, select Finish in the bottom left hand side of the page. Congratulations, your online enrollment is complete!
