MEDICAL RELEASE FORM INFORMATION

• When you complete the online registration, you will be sent a confirmation e-mail from registration@ag.org.

• This registration will include two release forms that MUST be submitted when you check in on-site. Your student will not be considered checked-in until both forms (completed and signed) are submitted upon team check-in or faxed in prior to the event.

need to be completed for students and participants over the age of 18. These forms will be kept on file for each participant.

NATIONAL BIBLE QUIZ FAQS

Q. What are the registration deadlines this year? A. Participants and officials (all quizzers, head coaches, assistant coaches) must register by May 24, 2019. General attendees must register by June 14, 2019.

Q. I missed the postmark deadline of May 24, 2019. Can I still register? A. After May 24, no participants, coaches, or assistant coaches can register. General attendees can still register until June 14, 2019.

Q. I completed my registration and then realized I forgot a ticket. What do I do? A. Call the NBQ office at 417.862.2781 ext. 4033 immediately. Any missed fees may be paid for with a credit card over the phone or by mailing a check. No changes to Participant registrations will be accepted after May 24.

Q. I can't make it and I've already paid for my registration. Can I get a refund? A. There are no refunds for any registrations. Monies can be transferred to someone else attending in their place.

Q. What does the registration fee include? A. General Attendee registration covers admission to all of the quizzing rooms, morning devotions, a National Bible Quiz Finals program, the Saturday ice cream fellowship, and Thursday night fellowship. All Participant registrations include the same items as the General Attendee registration as well as the opportunity to participate in all quizzing activities throughout the week and the Friday night banquet.

Q. What is the deadline for hotel reservations? A. The hotel reservation deadline for National Bible Quiz Finals is June 1, 2019.

Q. What is the parking policy for Bible Quiz Nationals 2019? A. Parking is available at North Central University. See maps section for maps to assist with parking.

Q. I’m a parent/leader/guest. What form do I fill out? A. All non-participants should register as a General Attendee. Click here to register as a General Attendee online or to access the General Attendee paper registration. Q. I’m a parent/leader and I’m also the assistant coach. What ticket do I choose? A. If you are serving as the team’s assistant coach, simply mark the box that says “Assistant Coach” on the registration form.

ONSITE REGISTRATION FAQS

Q. I just want to watch quizzing. Do I have to register? A. Every non-participant must complete an individual BQ Nationals Registration form and check “General Attendee.” Those who are 11+ must pay the minimum of the $15 General Attendee registration fee as well. Q. When is On-Site Registration? A. On-Site Registration will begin on June 29 at 5:00 pm. All Bible Quiz participants must complete check-in during this time. Coaches may check in their team on-site. General attendees must complete On-Site Registration before gaining access to quizzing and morning services. Q. Can I register at On-Site Registration? A. Only general attendees may register during On-Site Registration. However, it is recommended that all general attendees pre-register with the church they are coming to watch. All students and coaches participating in National Bible Quiz must meet the registration deadline of May 24. Q. My travel plans do not allow me to be at On-Site Registration by 5:00 pm on June 29. What do I do? A. Please have another responsible adult who will already be on-site complete registration for you. Participant registration must be completed before Sunday’s activities. Q. Can I add any tickets on-site? A. There are certain tickets you may add once you are on-site. Below is a list of these tickets and when you’re able to add them: • T-shirts: Will be available for purchase on-site beginning Monday, July 1, for those who did not pre-register for one. • Banquet tickets: May be purchased on-site until Tuesday morning, July 2, as we must turn in a meal count at this time.

All other tickets, including lunches and Senior Shoot-out, must be purchased or selected at the time of pre-registration.

Q. Does every registrant from my church/group have to attend On-Site Registration? A. No. It is recommended that only the coach(es) complete On-Site Registration for each church. Coach(es) will need to check in all participants, officials, and general attendees who have pre-registered. Q. Do I need to bring anything with me to On-Site Registration? A. This year, all participants and general attendees MUST submit a release form. If someone in your group has not submitted the appropriate release form, these will need to be submitted upon check-in. An individual will NOT be considered fully registered/checked-in until the release forms have been received. It is also helpful if the coach(es) checking in have a list of everyone who has pre-registered (participants and general attendees), but it is not required. Please also bring a typed list of any on-site cancellations or substitutions that must be made.
