For more than two decades, Congress and the federal gov-ernment have wrestled with how to pay physicians fairly while controlling spending in the Medicare program. The

Sustainable Growth Rate (SGR) formula failed to achieve either of these goals, and it was repealed by the Medicare Access and CHIP Reauthorization Act (MACRA) of 2015. Instead of annual threats of cuts in physician payments, MACRA stabilizes physi-cian fee levels and creates two new approaches to paying physi-cians that are designed to control cost and improve quality.

The default program under MACRA is the Merit-Based Incentive Payment System (MIPS), which retains the basic fee-for-service payment structure for physicians but requires adjustments in the amounts of payment based on physicians’ performance in improving quality and controlling costs. However, MACRA also creates significant incentives for physicians to participate in alter-native payment models (APMs) such as patient-centered medical homes, bundled payments, and accountable care organizations. Physicians who meet minimum levels of participation in alterna-tive payment models will be exempt from MIPS, receive bonus payments, and receive higher annual updates in their fees, in ad-dition to the greater flexibility in payment that is possible through APMs. In addition, Congress created mechanisms to encourage the development and implementation of more physician-focused payment models in the Medicare program.

Many choices must be made about how to implement the provi-sions of MACRA, and these choices will have profound impacts on the future of healthcare. Although the provisions of MACRA are directed specifically at physicians, the incentives and accountabil-ity required under MACRA will affect every healthcare provider—hospitals, nursing homes, home health agencies, etc.—and it will affect health plans and employers that contract with physicians.

The MACRA Summit seeks to provide deep insights into the choices under MACRA and their implications, and it will feature many examples of innovative approaches to healthcare pay-ment and delivery that can help achieve the goals of MACRA.

INTELLECTUAL PROPERTY POLICYUnauthorized sharing of Conference content via Webcast access through the shar-ing of user names and passwords or via alternative media (Flash Drive) through the sharing of said media is restricted by law and may subject the copyright infringer to substantial civil damages. The Conference aggressively pursues copyright infringers. If a registrant needs the ability to share Conference content within his or her organiza-tion, multiple Conference registrations are available at discounted rates.

The Conference will pay a reward for information regarding unauthorized sharing of Conference content. The reward will be one quarter (25%) of any recovery resulting from a copyright infringement (less legal fees and other expenses related to the recovery) up to a maximum reward payment of $25,000.
