Nurse Practitioners and Midwives – Accessing the Medicare Benefits Scheduleby Martin Bowen

Effective this month, eligible nurse practitioners and midwives will be able to access speci c items in the Medicare Bene ts Schedule (MBS).

This reform has been a long time coming, and is expected to increase affordable access to health professionals for patients across Australia. The Australian Nursing Federation’s assistant secretary Yvonne Chaperon has said that the community has voted with their feet in support of this reform, and cited a nurse-led clinic in Canberra which is staffed by nurse practitioners and advanced practice nurses, and which saw 2,400 patients in the rst two months of operation.

To provide MBS services and prescribe speci ed PBS subsidised medicines, Nurse Practitioners and Midwives need to apply for a medicare provider number and a PBS prescriber number. You can only receive a Medicare provider number if you are in private practice. For more information and to access the requisite forms, visit the Medicare Australia website.

Rural health advocates have highlighted the importance of allied health workers in the proposed establishment of a network of primary health care organisations known as ‘Medicare Locals’.

Planned as part of the new Australian health and hospital system, the network is part of the wider national health reform, whose remit includes greater support for allied health professionals including dentists, podiatrists, nutritionists, Aboriginal Health Service workers and physiotherapists.

Minister for Health Nicola Roxon con rmed it was expected Medicare Locals would be independent legal entities with formal linkages to Local Hospital Networks.

“Medicare Locals will have strong links to local communities, health professionals – including GPs, practice nurses and allied health professionals – and service providers, enabling them to respond more effectively to the needs of local communities,” she said.

“They will provide more coordinated care, improve access to services

and drive integration between GPs, hospitals, allied health, mental health and aged care.”

The National Rural Health Alliance (NRHA) con rmed a greater emphasis on allied health workers’ contribution to rural healthcare was one of its central concerns. The alliance’s submission on Minister Roxon’s Medicare Locals discussion paper, which dead-lined on 12 November, said rural and remote Medicare Locals “must be small enough in area for meaningful relationships to be maintained among individual clinicians, and between them and local hospitals, other service providers and community leaders. They should be based on existing communities of interest – which are de ned in part by the current ows of people to the services they access.”

Medicare Locals would not work well in rural and remote areas if they spanned long geographic distances or dif cult terrain or ignored natural communities of interest, such as was purportedly proposed in NT, Tasmania and South Australia.

“In areas of this size there are not the professional contacts nor the sense of local ownership and control needed to make the new entities work,” the submission said.

“Although there is always some tension between the search for economies of size or scale, and the search for the degree of localisation required for effectiveness, the Alliance believes that populations as small as 50,000 are suf cient for the effective undertaking of the tasks envisaged for these new primary care organisations. These

tasks include assessing local health needs, putting in place plans to meet those needs, and supporting the programs and personnel once they are in place.”

Speaking to NCAH, NRHA chair Dr Jenny May said there was no doubt that Medicare Locals promised greater opportunities for allied health professionals, with particular areas of the sector such as dentistry reporting high demand in rural and remote areas. However, such opportunities also came with caveats attached, such as the importance of effectively distributing and incentivising the workforce.

“The opportunities exist, but the question is, how well will [the system] work in delivering them?”

Issues still to be addressed included mitigating the social isolation and lack of continuing professional development opportunities experienced by allied health professionals working through Medicare Locals in remote areas, May said.

by Martin BowenStaff shortages and technical challenges (such as patient lifting and moving) means there is a present and growing incentive to design, develop and implement robotic technology in the delivery of health care. The reliance on robotic technology in surgical elds has been a reality for decades, but recent trends and developments indicate that the emergence of technology and even robotic technology in the delivery of primary healthcare is a growing phenomenon.

The NCAH website features a number of videos of robots used in healthcare. For example:

• Lifting Robot – is intended to assist nurses particularly in the aged care setting to lift or move patients. The obvious bene t being that there is no need for nurses to compromise their own back-health in the process of assisting their patients. The use of a robot, as opposed to a mini-lifting machine is intended to make the process a bit less intimidating for the patient.

• ‘Stan the Man’ – which is used to train nurses and health professionals. The robot is designed to respond to various treatments applied by the trainee nurse.

• Actroid-F – the life-like nature of this robotic nurse can only mean it is intended to mimic real nurses and despite the fact that this is clearly a signi cant technological achievement, it does raise signi cant questions about the role and limitations of technology in the delivery of primary healthcare.

Machines have been used in the delivery of primary healthcare for as long as adjustable beds have been around, but the line between machine and robot and indeed the ethics of their use becomes blurred as the machine / robot is designed to replace rather than assist nurses and health professionals in the delivery of healthcare.

NSW Nurses have said that they are prepared to escalate their campaign for mandated nurse-to-patient ratios if the NSW Government is not prepared to meet their demands.Nurses in NSW and indeed across the country are determined to force the NSW Government to implement nurse-to-patient ratios in order to meet fundamental patient safety requirements.Many nurses have rejected the government offer for a pay rise in place of ratio’s pointing to work place morale and patient safety as the fundamental issue that needs to be addressed.One possible explanation for the NSW government not being prepared to commit to ratios is that they know they won’t be able to meet the commitment. Staff shortages are the critical issue facing health ministries across the country. And with retirement rates set to dramatically increase in the aftermath of the global nancial crisis, which saw many nurses put off their retirement to sure up their

nancial security, the problem over the next three to ve years is going to get much worse.Perhaps a rst step is for governments to create the necessary places for graduate nurses to enter

the workforce next year and receive the necessary training to ultimately

ll the void that will be left as the exodus of many experienced nurses begins.
