CUES NURSING DIAGNOSIS ANALYSIS GOALS & OBJECTIVES NURSING INTERVENTIONS RATIONALE EVALUATION Subjective: The patient verbalized, “hindi ako makapagpadede kasi masakit utong ko.” Objective: Cracks on nipples tenderness swelling VS - Bp = - Rr = - Pr = - Temp. = Ineffective breastfeeding related to interruption in breastfeeding as manifested sore nipples with cracks. Immediate cause: Interruption in breastfeedin g. Intermediate cause: Sore nipples with cracks. Root Cause: Exposure of nipples to factors that dries up the nipples which results to cracks and sore. GOAL: After 8 hours of nursing intervention , the patient will be able to have appropriate knowledge on how to avoid sore nipples and cracks. OBJECTIVES: The client will be able to promote moisture on her nipples. SUPPLEMENTAL Assess patient’s vital signs and pay particular attention to the nipples. DEVELOPMENTAL Discuss to the client the factors that affect skin To obtain baseline data from the patient’s condition.
