Need a wellness exam? Visit a Primary Care Physician. Then go to STEP 1.
Already had a wellness exam in the current calendar year?
Go to STEP 1
STEP 1 Register an Account & Take your Personal Health Assessment (PHA)
Go to www.bluewellnessla.com
Click REGISTER NEW ACCOUNT and complete the registration form
Complete the Terms of Use (Click “Accept” if you agree to terms) and
then SUBMIT.
Go to “Get Started” to take your Personal Health Assessment (PHA).
PHA Completed; print an individual report letting you know your top
risk factors and giving suggestions to lower your risks.
STEP 2 Visit your Primary Care Physician (PCP) for an Annual Wellness Exam
Free Annual Wellness Visit offered to employees and spouses
enrolled in Century Rehab’s BCBSLA’s Insurance Plan. Specify
“wellness visit” when scheduling your appointment.
Log your visit at www.bluewellnessla.com
o Enter Username and password
o Click incentives then “Healthy Habit” to log the visit.

To take a Workshop: Click “2-Take a workshop” to participate in a workshop
To log all other activities: Click “Incentives” tab then click “Healthy Habit” tab. Click “Complete” and give activity name and date of activity. Click “Update” then verify information to be accurate by clicking “OK”.
To track completed activities: Click “Incentives” tab then click “Incentive Summary” to track completed activities.

Already Registered: Enter your
username and password and click “Go”
First Time Users: Log on to www.bluewellnessla.com And click “Register New Account”. Create username and password. Enter location code: 27613FF2
