1. Executive Summary

NES Engagement with the Third Sector and Voluntary Organisations

Greater integration of public services in Scotland is a key driver of the Public Service Reform agenda. Through partnership, collaboration and effective local delivery by statutory and third sector organisations, it is expected that Scotland will achieve the 2020 Vision of sustainable quality in healthcare. This requires the full engagement of the third sector in the planning, design and delivery of public services.

The Scottish Government asked each NHS Board to identify and agree with third sector colleagues, high impact areas of collaborative improvement and to complete an ‘Engagement Matrix’. This Engagement Matrix tool intended to facilitate the mapping of current engagement and partnership working with the third sector, to identify gaps in provision and to plan for further developments.

In consultation with NES leads, this report presents the NES engagement with the third sector, and offers some potential ways forward for further engagement.

NES engagements – of varying breadth, quality and collaboration – with approximately 40 third sector organisations have been identified. These were identified over the following five groups:

1. Policy and strategy development

2. Planning and commissioning

3. Service delivery

4. Capacity building

5. Learning exchange

It is acknowledged that the third sector contributes to the Scottish healthcare system, and there is a number of existing successful NES partnerships with third sector organisations. For example, there is a strong partnership with Alzheimer Scotland to deliver training, education and workforce development in relation to Scotland’s National Dementia Strategy.

However, our involvement and engagement with third sector organisations is variable and there is indeed potential for future developments and partnerships to be initiated, strengthened and sustained as appropriate.

There is currently no strategy for identifying further potential partnerships; however, some directorates have embedded third sector representation in Programme Boards and Advisory Forums (as part of programmes and work-streams).

In strengthening and sustaining the work of the third sector, a number of recommendations are suggested, and these include:

1. A formal strategic engagement process.

2. Embedding third sector engagement in educational governance.

3. NES promotion, learning exchange and awareness raising.

4. Internal and external communications.

5. A NES-wide accessible contacts database of the third sector.

6. Partnership working and shadowing.

7. Continuation of existing and successful partnerships.

2. Introduction

2.1. Strategic Landscape

The Healthcare Quality Strategy for NHSScotland1 aims to support the NHSScotland workforce in the delivery of safe, effective and person centred care. In 2010, the Quality Alliance Board2 (QAB) considered the importance of the third sector in the implementation of the strategy, and in 2011 recommendations for more effective engagement and coordination between NHS Boards and third sector organisations were endorsed. Full engagement of the third sector suggests that they “must be treated as partners in the planning, design and delivery of public services”3.

In addition, the Public Service Reform agenda suggests that “public, third sector and private organisations must work more effectively in partnership with communities and with each other to design and deliver excellent public services which meet the needs of local people”4.

This agenda rests upon four pillars5:

Prevention – Reduce future demand by preventing problems arising or dealing with them early on.

Performance – To demonstrate a focus on continuous improvement of the national outcomes, such that services are consistently well designed and based on the best evidence.

People – To unlock the full creativity and potential of people at all levels of public service, and empowering them to work together in innovative ways.

Partnership – The development of local partnership and collaboration, bringing public, third and private sector partners together with communities to deliver shared outcomes.

Similarly, the Public Bodies (Joint Working) (Scotland) Bill6 proposes greater integration between health and social care services, and aims for more efficient services and improved outcomes for individuals.

In line with the above, the Scottish Government envisions that by 2020, everyone will be able to live longer, healthier lives at home or in a homely setting. The healthcare system is envisioned to involve an integrated health and social care infrastructure, with a focus on prevention, anticipation

and supported self management7.

In supporting the workforce, the Everyone Matters: 2020 Workforce Vision8 sets out the values that are to be shared across Scotland’s health services:

We will respond to the needs of people we care for, adapt to new, improved

ways of working, and work seamlessly with colleagues and partner

organisations. We will continue to modernise the way we work and embrace

technology. We will do this in a way that lives to our core values.

Together, we will create a great place to work and deliver a high quality

healthcare service which is among the best in the world.

2.2. Third Sector Engagement in NES

In developing strategic engagements, each NHSScotland Board Chief Executive designated a Lead Officer, and the NES Third Sector Strategic Lead is Sonya Lam, Director of Allied Health Professions.

2.3. The Engagement Matrix

The Scottish Government9 asked each NHS Board to identify and agree with third sector colleagues high impact areas of collaborative improvement and to complete an ‘Engagement Matrix’ for each area. This Engagement Matrix tool intended to facilitate the mapping of current engagement and partnership working with the third sector, and to identify gaps in provision10, 11.

In consultation with named leads (Appendix A – Consultation Discussions, p. 18), a report12 of the NES engagement with the third sector was drawn up, and submitted to the Scottish Government in October 2013.

Claire Tester, Strategic Lead for Third Sector Engagement & Partnership at the Scottish Government, discussed the NHSScotland wide response at the NHSScotland Chief Executive’s meeting on 5/6 November 201313.

It is expected that the completed matrices from across NHSScotland will contribute to a national picture of joint working with the third sector, and will support planning for further engagement.

2.4. Discussion with a Third Sector Interface

Third Sector Interfaces (TSIs) represent third sector organisations. There are approximately 40,000 third sector organisations in Scotland, and the 32 TSIs represent these organisations in each area. In providing an interface between statutory and third sector organisations at a national level, it is recommended to initiate discussion with these TSIs14.

National intermediary bodies include the Scottish Council for Voluntary Organisations15 (SCVO), Voluntary Health Scotland16 (VHS), Health and Social Care Alliance Scotland17 (the Alliance), Voluntary Action Scotland18 (VAS) and Coalition of Care and Support Providers Scotland19 (CCPS).

In order to develop high impact areas of collaborative improvement, NES is in discussion with the SCVO, an umbrella body (i.e. TSI) for Scotland’s charities, voluntary organisations and social enterprises.

Specifically, discussion is underway with Lucy McTernan, Deputy Chief Executive and Paul White, Director of Networks.

2.5. Report Outline

This report is based on the NES Engagement Report submitted to the Scottish Government, and discusses:

1. Information on the Engagement Matrix tool used to map current NES engagement and gaps in provision, p. 6.

2. The methodology employed in researching the NES evidence, p. 8.

3. The Engagement Matrix

The Quality Alliance Board (QAB) endorsed proposals to strengthen strategic engagement between NHSScotland Boards and the third sector. In light of this, the ‘Engagement Matrix’ was co-developed by the third sector, NHS and the Scottish Government, and was approved by the QAB for use by NHS Boards and partners20. It was launched at ‘The Gathering’21 on 28 February 2013, by the then NHSScotland Chief Executive, Derek Feeley.

The Engagement Matrix aims to assist NHSScotland Boards and third sector organisations to map existing engagements, identify any gaps, and to agree potential future developments. It covers five functional themes – ranging from Learning Exchange to Policy – across five levels of engagement22.

4. Methodology

Further to the request by the Scottish Government, the Engagement Matrix was used throughout NES to review current partnership working and to map potential ways forward with the third sector.

Directors were asked to nominate NES leads24 for follow up discussions, and if applicable, to indicate whether their Directorate was a ‘nil return’25.

The process was then completed via one to one consultation with the named leads (Appendix A – Consultation Discussions, p. 18), in which joint completion of the matrices was central to the discussions. The process, facilitated by a discussion template, also allowed for in-depth exploration of current levels and quality of engagement with third sector organisations, as well as discussions around potential future strategies for engagement.

Reference was also made to the Executive Team paper of 07 May 2013, ‘Strengthening NES partnership with the Third Sector and third sector organisations’, by Sonya Lam and Jane Davies26.

5. NES Engagement Matrices

A narrative review of the NES engagement with the third sector can be found in Appendix B – Summary of NES Interaction with the Third Sector, Table 7, p. 19. This outlines engagements over the following areas:

Scottish Council for Voluntary Organisations (SCVO)

NES Dentistry

NES Educational Development Directorate (EDD)

NES Nursing, Midwifery and Allied Health Professions (NMAHP)

NES Pharmacy

NES Psychology

It can be seen that approximately 40 voluntary organisations have been identified, with interactions ranging from learning exchange, to co-creation of learning resources, through to strategic engagements.

The individual matrices developed with NES leads are available in Appendix C – Individual NES Engagement Matrices, p. 27.

Using the Engagement Matrix, the levels and nature of these engagements are clearly evident.

Table 4, overleaf provides a collated summary of the NES engagement matrices. Here, the numbers refer to the specific projects and work-streams outlined in Table 7, p. 19. In addition, for simplicity, Table 5, p. 11 provides an accumulated numerical summary of these projects/work-streams.

It can be seen that the most comprehensive coverage of engagement occurs at the collaborative level, i.e. Level 4 – and this occurs across all the identified themes. In addition, there are also some strong examples of where NES actively involve and consult with partners.

However, gaps in provision are also evident, most notably at the empowerment level.

In developing further strategic engagements, the next section of this report outlines suggested ways forward with the third sector and voluntary organisations.

This would support staff who wish to engage with the area. For instance, one nominated interviewee who was tasked with developing the work within her Directorate highlighted that she found the consultation exercise to be fruitful, as it offered her a beneficial starting point. Whilst there is third sector representation across NES Advisory Forums and Programme Boards, it was suggested that a dedicated database outlining contacts would be beneficial. There was a perception that some engagements were ad hoc (and through known contacts) and it was recommended that a more robust method for engagement was required. Noted that some charities contacted NES so that individuals could sit within their committees.

This work-stream involves the development of a training programme to select and recruit volunteers to deliver part of the service.

The service aims to embed and sustain a service to help self-management and the normalisation of feelings around grief, bereavement and loss.

Chaplains recruit, train and supervise volunteers (who are recruited from local volunteer networks).

Volunteers are provided with travelling expenses (and the training is funded by the Scottish Government).

It is expected that the use of volunteers will be embedded across Scotland by 2015 – as informed by the current pilot work.

Programme of work is aligned against the co-production

work-stream of the Scottish Government’s Person Centred

Agenda.

It is expected that the use of volunteers will be embedded across Scotland by 2015 – as informed by the current pilot

work.

Chaplains provide a listening service in GP practices. The service aims to embed and

sustain a service to help self-management and the

normalisation of feelings around grief, bereavement

and loss.

This work-stream involves the development of a training

programme to select and recruit volunteers to deliver part of the service. Chaplains recruit, train and supervise volunteers (who

are obtained from local volunteer networks). Volunteers are

provided with travelling expenses (and the training is funded by the

Scottish Government).
