NEW PATIENT WELCOME LETTER AND INTAKE Welcome to Windsor Community Acupuncture! Please take a moment to read through this important information so you can get a sense of what to expect. What's Different About Windsor Community Acupuncture? The clinic is filled with comfortable recliners clustered in groups in a large quiet soothing space. Treating patients in a community settmg has many benefits: it's easy for friends and family to come in for treatment together; many patients find it comforting and relaxing which makes the healing process so much better. The style of acupuncture practiced at WCA allows patients to keep their needles as long as they want and the "right" amount of time varies from patient to patient. Most patients learn after a few freatments when they are "done"; this can take anywhere from twenty minutes to over an hour! There is a sliding scale of $15-$35 Because of the sliding scale, insurance cannot be billed (that's the insurance company's policy). The sliding scale ranges from $15-$35. You decide what you can pay within that scale. It is completely up to you. You will NEVER be asked for any personal information regarding your income. First visits to the clinic please include a $10 intake processing fee. This means that the ver>' first time you come in for a treatment you will pay a minimum of $25 for your treatment. All subsequent visits to WCA fall on the sliding scale of $15-$35. Payment is due at the time of your visit. Payment Box Box is clearly marked, located against the south wall. You may pay either before or after your treatment. It's up to you. Please feel free to make payment in the box without supervision as the acupuncturist may be busy in the clinic. Check: simply drop your check into the box. Please make them out to "Ben Arnold" Cash: place cash in one of the available envelopes, write your name on the envelope and drop it into the box. (This information is also available near the box to help remind you). ,.,you're done! Bathroom Feel free to use the bathroom down the hall. It is accessed through the door in the corner of the room with a sign on it. Be sure to take the key with you. Please be sure to use the bathroom before you begin treatment as it will take over 30 minutes. Commitment To You I want to make it possible for you to receive acupuncture regularly enough and long enough to get better and stay better. I want to give you the tools to take care of your own health so that you will not need to rely on Big Insurance or Big Pharmaceuticals for costly, high-tech interventions. I am deeply committed to sustainability at every level of this practice.

What's Needed From You: * Responsibility Windsor Community Acupuncture does not provide primary care. Acupuncture is a wonderful compliment to Western medicine, but it is not a substitute for it. I f you think you might have a serious medical problem or i f you want someone to go over the details of your medical history, you need to see a primary physician (ND, MD, or DO). We can provide complimentary care for conditions which also require a physician's attention. Acupuncture is often very helpful for side effects of chemotherapy, Lyme disease. Hepatitis, etc. but you must also take responsibility for your own health.

* Commitment to yourself Acupuncture is a process. More often than not, a series of treatments is required in order to resolve a problem. On your first treatment you will be given a treatment plan to take home. That is where you must ultimately decide and make best use of the sliding scale without hesitation.

• TURN OFF YOUR C E L L PHONE OR LEAVE IT AT HOME • Show up on time (every minute counts when scheduling 10 minute intervals) • Cancel within 12 hours (or else you may be charged $15, as with no-shows) • Do NOT wear perfume, after shave, smoke cigarettes or any other smoke before

coming to WCA (many are chemically sensitive) • Be very brief and to the point about your chief complaint. There is little time to talk
