Mogale and Zulu were suspended in February 2017. According to a statement from the NHLS board chairperson, Eric Buch, the CEO and CFO were charged with irregularities and failure to fulfil their duties in relation to three procurements. The charges related to exceeding delegations of authority, irregular payments made to service providers and irregular appointment of service providers. The United States administration’s backtracking on a plan to cut US funding for South Africa’s HIV programme by 30% (R2.9-bn) is “a major victory for people living with HIV”, said non- governmental organisations (NGOs). Citing the poor performance of SA’s HIV programme, the US President’s Emergency Plan for AIDS Relief (PEPFAR) earlier threatened to slash funds to the country. The about-turn, decided on during the second round of PEPFAR’s planning meetings in Washington DC, will now see SA receiving up to $730-m in the 2019/20 financial year. Clinics hit by vaccine shortages Due to a stock scarcity of the DT (diphtheria and tetanus) vaccine, many South African parents now have to have their children immunised privately. According to Kopano Klaas, case manager for the Stop Stockouts Project, reports of shortages and stockouts continue nationwide. A study of global vaccine use found that one in three World Health Organisation member states experienced at least one stockout of one vaccine for at least a month every year. Immunisations against DT are available at all primary healthcare clinics and centres. The DT vaccine provides protection against three diseases: diphtheria (which can cause fatal swelling in the throat area), pertussis (whooping cough) and tetanus (a condition of severe muscle spasms caused by contamination of dirty wounds). Special News READ MORE Accountability needed to counter medical negligence Pre-existing contingent liability resulting from medico-legal claims in the public health service that cannot be avoided, and which is now impacting on the Government’s fiscal liability, is a direct manifestation of multi-faceted governance failures in the system. “A future contingent of medical liability will continue at the same scale unless something changes,” Prof Alex van den Heever, Chair of Social Security Systems, Administration and Management Studies at the University of the Witwatersrand, warned in a recent SA Medico-Legal Association (SAMLA) Presidential Presentation. Van den Heever’s presentation was one of several on pressing medico-legal issues video-recorded by SAMLA for submission to President Cyril Ramaphosa’s office. Key among these governance failures, he pointed out, was the absence of strategic accountability frameworks. These comprise of independent supervision, performance requirements, transparency and consequences at both a meta- and micro-governance level. READ MORE Financial News Netcare squeezed by prescriptive medical aids On May 13, hospital group Netcare experienced the biggest one-day drop in its share price in almost two months after it said profits will be squeezed by increasingly prescriptive medical aids, reported Business Report (14 May). The schemes restrict members to specific hospitals, allowing them to “shift market share in return for price discounts”, Netcare said. According to Sentio Capital portfolio manager, Imtiaz Suliman, medical aid schemes were “going to put more and more pressure on the hospital groups to be more selective in admitting patients”. Netcare warned investors that margins in its hospitals and emergency-services divisions would shrink between 50 and 80 basis points in the second half of the financial year. Describing the squeeze as the result of medical schemes offering cheaper options to members, Netcare CEO Richard Friedland said it is a “new fact of life”. Latest Global Credit Rating for Transmed The latest Global Credit Ratings (GCR) affirmed the national scale claims paying ability rating assigned to Transmed Medical Fund of BB+(ZA), with the rating outlook accorded as Negative. Pharmaceutical News Is SA’s top contraceptive putting women at risk? "Is it better to have an unwanted pregnancy, complications in pregnancy or an increased risk of contracting HIV and tuberculosis?” wrote Katharine Child in Times Select (29 April 2019). “This is the question SA policymakers and birth control experts around the world are asking as evidence mounts against the most popular contraceptive in sub-Saharan Africa." Depo Provera, a three monthly contraceptive injection with the synthetic hormone Depo- Medroxyprogesterone (DMPA), has long been suspected of increasing the risk of HIV. One major study suggests the risk could increase by 40%, but other studies suggest there is no risk. UCT researchers have now shown the active ingredients in the Depo injection stops the immune system from fighting TB, the country’s biggest killer. However, scientists have cautioned that this was a study of cells in a laboratory and not in people. The Depo Provera DPMA injection is popular because it is taken every three months, can be used secretly and requires fewer visits to a clinic to access contraception than other options. As a result, 16.5-m women in sub-Saharan Africa use it. READ MORE IN ATTACHMENT: DEPO PROVERA ‘Breakthrough’ slimming pill A “breakthrough” slimming pill - that expands in the stomach - was twice as successful as a placebo, according to research presented at the European Congress on Obesity. Of those taking the drug, already given regulatory approval in the US, 59% achieved at least 5% loss of body weight, with average weight loss of 10kg in six months. The treatment, called Plenity, is said to have no serious side-effects and could become “the treatment of choice” for millions. The treatment involves taking three pills before main meals, with two glasses of water. The capsules contain tiny particles of super- absorbent hydrogel, no bigger than a grain of salt, which then expand in the stomach forming a gel, so that the person feels full. Amazon acquires online pharmacy Online shopping giant, Amazon, has recently acquired an online pharmacy in the US called Pillpack. “In the wake of this move, local pharmacy experts have been considering the possibility that e- pharmacies could be a massive step forward for people who struggle to access medication in South Africa,” reported Saturday Star (11 May). Erik Roos, chief executive of Pharma Dynamics, said selling medicine online could have life- saving benefits for patients living in rural areas. By supplying medicines online, e-pharmacies are able to warehouse large quantities of stock, which reduces the cost of medicines. Chief executive of the Pharmaceutical Society of South Africa (PSSA), Ivan Kotze, says e- pharmacies would not be in the best interest of patients as it is essential that patients discuss medicine with a pharmacist before taking it. No pressure, but perhaps check if you are hypertensive Ahead of World Hypertension Day on May 17, medical experts have urged people to take a blood pressure test - the first step in the fight against the condition, which one in two SA adults have. Two years ago it has been decided that May be declared “Measurement Month” by the International Society of Hypertension (ISH), the Southern African Hypertension Society and the National Health Department, among others. The screening campaign has already tested more than 2.7-m people since its inception. In SA, an estimated 53 men and 78 women older than 30 die from the impact of hypertension every day. General News UN report warns of disastrous consequences of antibiotic resistance According to a new report, released by the United Nations Interagency Coordinating Group on Antimicrobial Resistance (IACG), medical progress is threatened by worldwide antimicrobial resistance. It concluded that drug-resistant diseases could cause 10-m deaths each year by 2050. The report follows a South African study that has exposed very serious shortcomings with the way doctors and nurses are prescribing antibiotics. Currently, at least 700 000 people die each year worldwide due to drug-resistant diseases, including 230 000 people from multidrug resistant tuberculosis. The report can be found at: https://www.who.int/antimicrobial-resistance/interagency-coordinationgroup/ Pilot Blood Test to flag Chronic Fatigue Syndrome People suffering from a debilitating and often discounted disease, known as Chronic Fatigue Syndrome, may soon have scientific proof of their ailment. Researchers at the Stanford University School of Medicine have created a blood test that can flag the disease, which currently lacks a standard, reliable diagnostic test. According to an article in Medical Brief (8 May), Dr Ron Davis, professor of biochemistry and of genetics; Dr Rahim Esfandyarpour, a former Stanford research associate; and their colleagues have devised a blood-based test that accurately flags all Chronic Fatigue Syndrome patients and none of the healthy individuals. The diagnostic platform could even help identify possible drugs to treat Chronic Fatigue Syndrome. News on Medical Aids 'Big shift' to cheaper medical care According to Medical aid broker Alexander Forbes the number of corporate clients who reviewed their plans for this year had doubled. For the first time, more people had downgraded than upgraded, reported Sunday Times (5 May 2019). Profmed principal officer, Craig Comrie, said 2019 was the first time the scheme introduced low-cost options and about 3 500 of its 70 000 members had switched. But medical aids warn spending less means higher premiums ultimately, because less is paid into the pool for all to share. Discovery Health CEO, Jonathan Broomberg, said a more important trend than downgrading was “that members joining Discovery Health Medical Scheme are tending to join at lower plan levels than they have in the past”. Bonitas principal officer, Gerhard van Emmenis, said new members preferred cheaper plans. While 94% of members had not changed their plans this year, 10,949 had downgraded and 7,345 had upgraded.
