The following set of QWB-SA questions are about ACUTE HEALTH PROBLEMS experienced over the

previous 3 days *Note: For purposes of NHMS responses of “don’t know” or “refused” for the following questions were counted as “no”. This is based on the assumption that if the participants did not recall or want to mention the problem, then it did not cause enough discomfort to negatively affect their quality of life. This assumption was made to avoid very confusing coding and multiplying number of data elements as we would not know which day a “don’t know” or “refuse” referred to. QWB_VISPROB_1

HUI 2/3 QUESTIONS The Health Utilities Index, which is proprietary to Health Utilities Inc., Dundas, Ontario, has 40 questions in its interviewer-administered form. Not all questions are asked of all respondents since there are skip patterns. However the content of the question stems and responses, the skip patterns, and the mapping of responses into the HUI2 and HUI3 domain levels are kept confidential under terms of our license from Health Utilities Inc. Accordingly they are not included in the public NHMS data set. Users of HUI2 and HUI3 should be aware these two indexes are correlated in part because they are both scored from one set of responses to the same 40 questions. Mapping from the 40 questions to HUI3 requires all 40 questions. The HUI2 sensation domain variable is created from three HUI3 domains, vision, hearing, and speech (the three of which are created from 15 of the 40 questions) using a logical mapping algorithm.
