You will be responsible for dealing with the surveillance, prevention and control of communicable disease; provision of specialist advice and support locally to Primary Care, hospitals, other relevant organisations, local authorities and other local organisations. This post is part of a team of 3 WTE health protection consultants, programme managers, nursing and support staff. The post holder will be involved in the investigation and management of a full range of health protection incidents/outbreaks and will be expected to contribute to co-ordination, support and monitoring of local implementation of certain key national programmes. Appointment: This is a full time replacement post (10 PAs or 37.5 hours per week, AfC) for a consultant in public health medicine/public health (health protection) within the Public Health Protection Unit of NHS Greater Glasgow and Clyde, West House, Gartnavel Royal Hospital, 1055 Great Western Road, Glasgow G12 0XH

1. Job summary

The post holder will be responsible for dealing with the surveillance, prevention and control of communicable disease. S/he will provide specialist advice and support locally to, primary care, hospitals, and other relevant organisations, and local authorities and other local organizations and staff and agree with them how health protection should be delivered locally. S/he will investigate and manage a full range of health protection incidents (including outbreaks of diseases like meningitis and food poisoning), and will carry out

surveillance, co-ordination, support and the monitoring of local implementation of certain key national programmes.

In discharging these responsibilities, the postholder will maintain and develop effective working relationships with the wide variety of individuals and organisations involved with protection of the health of the population. In particular, s/he will need to work closely with others in the local, regional and national health protection partnerships including HPS, local authority environmental health departments, the NHS (particularly public health and microbiology colleagues) and the emergency response services. However it is essential that effective working relationships are developed with the wide range of others with roles in health protection, including for example, Scottish Water, Food Standards Agency, veterinary services, hospital control of infection teams, occupational health services, hospital medical staff, health service managers, primary care, schools, nursing homes, prisons and voluntary agencies.

1.1 Other dutiesThere may be opportunities for the post holder to take on other remits within the public health directorate (i.e. within health improvement or health services). This would be discussed and agreed with the post holder and the job plan would reflect these agreements.

There will also be occasions when Board and Directorate priorities require the post holder to undertake non-health protection duties under the direction of the Director of Public Health.

2. The employing organisation

NHS Greater Glasgow and Clyde provide services to almost a quarter of the entire Scottish population. The public health issues within the community are significant and challenging.

NHS Greater Glasgow and Clyde is one of the largest National Health Service providers and employers in the UK. In partnership with 6 local authorities we are responsible for the health needs of a population of 1.2 million people; Glasgow itself has a population of nearly 600,000. The geographical area covered by NHS Greater Glasgow and Clyde is diverse; it covers both urban and rural locations in the Glasgow and Clyde area. As such it presents considerable challenges in ensuring that the health needs of the population are met.

3. Public Health Protection Unit

3.1 Current staffingThe Public Health Protection Unit within Greater Glasgow and Clyde NHS Board comprises of a head of section (CPHM), a further 2.1 whole time equivalent CsPHM, one staff grade doctor, three health protection nurse specialists, six TB nurse specialists, three programme managers and support staff. The postholder will have shared managerial responsibility for support staff within PHPU.

The public health directorate within NHS Greater Glasgow and Clyde comprises four divisions - health services, health improvement, directorate services and the public health protection unit.

3.2 Training and CPD arrangementsNHS Greater Glasgow and Clyde supports continuous professional development for all staff and attaches considerable importance to its staff development strategy. The postholder is expected to:

participate in CPD and is encouraged to take part in events designed to widen knowledge and expertise in all areas, not only those relating to their own area of work;

contribute to the training of specialty registrars and other junior doctors within the PHPU;

contribute to the postgraduate and undergraduate teaching at the University of Glasgow.

4. Management arrangements

The post holder is accountable to the health board via the Head of Section of the Public Health Protection Unit and the Director of Public Health. An initial job plan will be agreed with the successful candidate prior to that individual taking up post. This job plan will be reviewed as part of the annual job planning process.

The post holder will be a Competent Person under the provision of the Public Health Act etc (Scotland) Act 2008.

5. Professional obligations

The post holder will be expected to: participate in the organisation’s staff appraisal scheme and departmental

audit, and ensure appraisal and development of any staff for which s/he is responsible

contribute actively to the training programme for Specialty Registrars in Public Health Medicine as appropriate, and to the training of practitioners and primary care professionals within the Board;

pursue a programme of CPD, in accordance with Faculty of Public Health requirements, or other recognised body, and undertake revalidation, audit or other measures required to remain on the GMC Specialist Register or other specialist register as appropriate;

practise in accordance with all relevant sections of the General Medical Council’s Good Medical Practice and Faculty of Public Health’s Good Public Health Practice.

6. Key areas of responsibility

The job description will be subject to review in consultation with the postholder and in the light of the needs of the employing organisation and the development of the speciality of public health and any wider developments in the field of public health.

The key result areas relate to the public health/ public health medicine contribution to health protection for all of Greater Glasgow and Clyde. In each of the key areas listed below, the post holder will be required to provide a consultant level service, in accordance with appropriate professional standards. UK standards for public health/ public health medicine practice are currently specified by the Faculty of Public Health, and will be subject to periodic amendment. For some areas of practice, additional standards are specified at a national or local level.

Surveillance and assessment of the population’s health and well-being To maintain and develop an effective system for the surveillance of

communicable disease and environmental hazards in the designated area, and ensure that the Board’s surveillance system feeds into the national surveillance system.

To design, develop and utilise information and intelligence systems to underpin public health improvement and action across disciplines and organisations.

To write and/or contribute to national and local policy setting reports on the health of the population of Greater Glasgow and Clyde.

Assessing the evidence of effectiveness of health and healthcare interventions, programmes and services

To provide expert public health advice and leadership to support and inform an evidence-based approach to communicable disease control and dealing with non-infectious environmental hazards, working in potentially contentious and hostile environments where barriers to acceptance may exist.

To be responsible for leading on service development, evaluation and quality assurance governance of services to reduce the impact of communicable diseases and environmental hazards to health and for preparing and adjusting action plans in line with changing needs and changing geographical boundaries.

Policy and strategy development and implementation To provide public health medical advice and perform the duties of the

Competent Person for the six local authorities within the Board area To supervise and co-ordinate activities for the prevention of communicable

disease and non-infectious environmental hazards, leading on the local implementation of national programmes as appropriate, such as those for the prevention and control of individual infectious disease such as tuberculosis and hepatitis C.

To be a member of Control of Infection Committees within NHS Greater Glasgow and Clyde and play a leading role ensuring the preparation of contingency plans for the control of major outbreaks and incidents both in hospitals and community settings. To discharge the responsibilities for NHS emergency public health responses as specified and agreed between local agencies, and to take the responsibility for the preparation and revision of all major emergency plans as they relate to health protection.

Leadership and collaborative working for health To identify problems, define objectives, prepare plans and set targets for the

control of communicable disease and non-infectious environmental hazards in Greater Glasgow and Clyde, taking into account the need to improve health and reduce health inequalities.

To lead the development of infection control advice for the wider community (schools, nursing homes, prisons, etc) working closely with infection control specialists and leads at local level, including primary care, hospitals, control of infection and public health nurses, local authorities and relevant national standard setting bodies.

To be a member of, and contribute to, relevant liaison groups in communicable disease control and health protection within the area and region as required

To assist communicable disease control, health protection and public health colleagues in neighbouring areas during major communicable disease or other health protection incidents.

Health improvement To work with local communities in helping them in addressing the issues of

communicable disease control and where appropriate non-infectious environmental hazards, using community development approaches as appropriate.

To assume overall responsibility for the control of outbreaks of infectious disease. This will entail co-ordinating control of major outbreaks and working closely with the infection control doctors and other relevant staff in hospitals and primary care, and those involved in the local, regional and national health protection arrangements.

To contribute to non-infectious environmental hazard incidents. This will entail working closely with those involved in the local, regional and national health protection arrangements.

To co-ordinate, monitor and support the implementation of national immunisation programmes and the development of the local immunisation policy for the designated area, working with the Director of Public Health, immunisation co-ordinator and other relevant partners at local level.

To provide expert advice and support to the Integrated Pollution and Prevention Control (IPPC) and other legislative processes and planning issues which could have significant health impacts.

To contribute to the provision of an emergency 24 hour rota for health protection covering NHS Greater Glasgow and Clyde. (Overall frequency currently 1 in 9 with prospective cover, mainly first on call but occasionally second on call covering StRs).

The post holder will be expected to undertake duties in relation to emergency planning.

The post holder will be expected to attend port health call-outs at Glasgow International Airport and the port of Greenock.

Service improvement To support and encourage relevant activities to manage risk and reduce

hazards (including clinical governance) with respect to the control of communicable disease and non-infectious environmental hazards within Greater Glasgow and Clyde.

To provide advice as required, given local arrangements, to NHS commissioners on the provision of services within Greater Glasgow and Clyde for the prevention, diagnosis, treatment and control of communicable diseases.

Public health intelligence To analyse and evaluate quantitative and qualitative data and research

evidence from a range of sources to make recommendations and inform decision making which has long term impacts.

To compare, analyse and interpret highly complex options for running projects identified as key health protection priorities, and communicate this information across organisations and the local community.

To lead on, plan and design agreed aspects of the assessment of health needs, health inequalities, and health impact assessment with respect to communicable disease control and non infectious environmental hazards, to identify areas for action within the local population based on the best available evidence, and to be responsible for short and long term planning and for providing advice on the treatment of groups of populations.

To work with others to develop the research evidence base required to support the control of communicable disease and the health protection function, both initiating and collaborating with research as appropriate and linking with relevant academic institutions.

To undertake and commission literature reviews, evaluative research surveys, audits and other research as required to inform equitable service and reduce health inequalities. This may involve taking the lead on R&D public health and related activities for NHS Greater Glasgow and Clyde.

To assist with education, training and competency development and assessment in health protection for the wider health protection and the specialist public health workforce within the Board area.

GENERAL CONDITIONS

Terms and conditions of serviceThe post is subject to NHS Terms and Conditions.

Those candidates who meet the requirements for appointment as a Consultant in Public Health Medicine will be eligible for the current NHS Consultant Contract salary scale and on-call availability supplement (currently 5%). This post is for a full time (10 PAs) consultant. The job plan consists of 9 PAs of direct clinical care and 1 PA of supporting activity.

Those candidates appointed as Consultants in Public Health will be eligible for NHS Agenda for Change Band 8d.

Base The post is based at West House, Gartnavel Royal Hospital, 1055 Great Western Road, Glasgow, G12 0XH but will involve travel within the health board area for which appropriate mileage rates will be paid. The postholder will be expected to be available for on call for health protection and to participate in the communicable disease and environmental hazards control and emergency planning arrangements within the Board area. The post-holder will be required to live (or reside while on-call) within 40 minutes travelling time of Glasgow Airport.

IndemnityAs the postholder will only be indemnified for duties undertaken on behalf of NHS Greater Glasgow and Clyde the postholder is strongly advised to ensure that he/she has appropriate professional defence organisation cover for duties outside the scope of NHS Greater Glasgow and Clyde and for private activity within NHS Greater Glasgow and Clyde.

Short listing notes The Faculty of Public Health advises that in order to be short listed for a consultant post applicants who are not yet on the GMC Specialist Register/GDC Specialist List in dental public health/UK Public Health Register (UKPHR) must provide verifiable signed documentary evidence that an application for inclusion on one of these specialist registers is in progress as follows:

1. Applicants in training gradesPublic health Specialist Registrars and Specialist Trainees in a recognised UK public health training scheme must provide evidence to confirm that they are within SIX months of award of their certificate of completion of training (CCT) and inclusion in the GMC Specialist Register/GDC Specialist List in dental public health/UKPHR at the date of interview (i.e. the expected date of award of their CCT must fall no more than six months after the date of interview). Either a RITA Form G (Final Record of Satisfactory Progress) or a letter from the postgraduate dean (or Faculty Adviser) specifying the expected date for completion of training (which must be not more than six months after the date of interview).

2. Applicants in non training grades

2.1 Doctors (i.e. medical practitioners)

Doctors outside recognised UK public health training schemes fall into a number of categories:

those who have trained outside the UK, who may have specialist training and qualifications which they are seeking to have recognised by the Postgraduate Medical Education & Training Board (PMETB) in order to gain registration with the GMC: these doctors may be short listed according to the following 2005 guidance from the Department of Health and Scottish Executive which indicates that There will be some instances (for example when considering applicants trained outside the UK) where an AAC may choose to interview a candidate prior to [GMC] Specialist Register entry. In these circumstances, it will wish to be satisfied that subsequent Specialist Register entry is likely. Employers should ask the applicant to provide documentary evidence that he/she has submitted an application to the PMETB which is eligible for consideration at the time of application (for short listing).

those who have not completed specialist training in the UK who are seeking entry to the GMC Specialist Register through Article 14 of the European Specialist Medical Qualifications Order (ESMQO), which allows the PMETB to consider not only training but also relevant experience: these doctors may have trained in or outside of the UK. Again, employers should ask the applicant to provide documentary evidence that he/she has submitted an application to the PMETB which is eligible for consideration at the time of application (for short listing).

Employers are advised that individuals should not take up consultant in public health medicine or consultant in public health posts (including DPH posts) until such point as they have gained entry to the GMC Specialist Register/GDC Specialist List in dental public health/UK Voluntary Register for Public Health Specialists. Although applicants will be able to provide documentary evidence that an application is in progress, no guarantee can be made as to the outcome of an application to the GMC/GDC/UKPHR specialist registers.

In the course of this activity, other agreed public health consultant level inputs to aspects of health promotion, health intelligence and other corporate public health activities and public health networking may be undertaken.

Health protection: Provision of communicable disease and environmental health advice in response to enquiries from the public, partner agencies and professionals. Management of CD & EH incidents and participation in the daytime public health first on duty rota.

Other agreed public health consultant level inputs to aspects of health promotion, health intelligence and other corporate public health activities and public health networking may be undertaken.

Participation in the out of hours on-call rota for Public Health incidents and emergencies, on weekdays from 17h00 to 09h00 and on weekends and public holidays. On-call duties include responsibility for communicable disease and environmental health incidents, including chemical, biological and nuclear incidents that might affect the health of the population of NHS Greater Glasgow and Clyde, as described in c) below. This may involve provision of assistance to neighbouring NHS Boards under memoranda of understanding signed by respective Directors of Public Health.

A significant proportion of all public health work (described in section a) above), is dictated by the agenda of other agencies including local authorities, the Scottish Government Health Department and other NHS. This work is often necessarily conducted at varying times away from West House. The nature, extent and timing of my involvement is subject to managerial agreement. It is described under other

c) Activities during Premium Rate Hours of Work e.g. hours outwith 8am-8pm Monday to Friday

Participation in the public health on-call rota, noted in b) above. The main duties include:

Statutory duties as a Competent person for each of our 6 local authorities Responding to cases of certain communicable diseases or outbreaks and

certain environmental hazards Response to serious incidents potentially affecting the health of the population

of NHS Greater Glasgow and Clyde. Port health at sea ports such as Greenock Ocean Terminal and airport port

health at Glasgow International Airport Assistance to neighbouring boards’ public health departments in case of

major public health incidents. This would include co-ordination of surge capacity responses.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

GRADE AND SALARY Consultant£ 75,249 - £ 101,451 per annum (pro rata)AfC Band 8d

New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.

HOURS OF DUTY Full Time 40 hours or 37.5 hours per week AfC

SUPERANNUATION New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme.  Should you choose to "opt out" arrangements can be made to do this via: www.sppa.gov.uk

REMOVAL EXPENSES Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.

EXPENSES OF CANDIDATES FOR APPOINTMENT

The rehabilitation of Offenders act 1974 allows people who have been convicted of certain criminal offences to regard their convictions as “spent” after the lapse of a period of years. However, due to the nature of work for which you are applying this post is exempt from the provisions of Section 4 of the Rehabilitation of Offenders Act 1974 by virtue of the Rehabilitation of Offenders Act 1974 (Exceptions Orders 1975 and 1986).
