Rabies
• ไมควรเยบแผลทนท• Tetanus prevention & ABO
วคซนปองกนโรคพษสนขบาทใชในปจจบน
Post-exposure immunization
• Intramuscular• Intradermal
Intramuscular
• ฉดวคซน HDCV, PCECV, PDEV 1 ml หรอPVRV, CPRV 0.5 ml เขากลามเนอตนแขน(deltoid) หรอถาเปนเดกเลกฉดเขากลามเนอ
หนาขาดานนอก (anterolateral)
Intradermal
การฉดแบบ 2-2-2-0-2• ฉดวคซนจดละ 0.1 ml โดยฉดวคซนเขาในผวหนง
บรเวณตนแขนซายและขวาขางละ 1 จด
Intradermal
• มประสทธภาพในการปองกนและระดบภมคมกนทดเทยมกบทใชวธฉดเขากลามเนอ
• สามารถใชไดกบวคซน PVRV, CPRV, PCECV, HDCV
• ไมแนะนำาใหใชวธการฉดในผวหนงแบบ 8 จด และ วธการนไมสามารถทดแทนการฉด RIG ได
• ไมควรใชในกรณตอไปน– ถาผสมผสโรคอยระหวางกนยา chloroquine หรอสา
รอนๆ ทมฤทธกดภมคมกน หรอมการตดเชอ HIV
ขอพจารณาพเศษในการฉดวคซน
• กรณผสมผสโรคไมมาตามกำาหนดวนนดหมาย เชน อาจมาคลาดเคลอนไปบาง 2-3 วน ใหฉดวคซนตอ
เนองตอไปโดยไมตองเรมฉดวคซนใหม• การฉดวคซนในเดกและผใหญใหใชขนาดเดยวกน• หญงมครรภไมมขอหามในการฉดวคซนและอมม
โนโกลบลน• กรณผตดเชอ HIV หรอภาวะภมคมกนเสอม หรอ
กำาลงไดรบยากดภมคมกนใหยดหลกปฏบตตามปกต
ขอพจารณาพเศษในการฉดวคซน
• วคซนทใชอยในประเทศไทยในขณะน มคณภาพประสทธภาพ และความปลอดภยใกลเคยงกน ในการฉดเขากลามสามารถ
ใชทดแทนกนได หากหาวคซนทใชอยเดมไมได• ในกรณทฉดเขากลามเนอหรอในผวหนง ใหฉดวธเดยวกน
ตลอดจนครบชด ไมควรเปลยนวธการฉด• เนองจากสวนใหญการฟกตวของโรคพษสนขบาใชเวลาไม
เกน 1 ป เมอมผสมผสโรคมาขอรบบรการหลงสมผสโรค ภายในเวลาไมเกน 1 ป ใหพจารณาปฏบตเชนเดยวกบกรณท
เพงสมผสโรคใหมๆ ในกรณทมารบการรกษาหลงสมผสโรค เกน 1 ป ใหพจารณาเปนรายๆ ไป
การฉดวคซนกระตนสำาหรบผสมผสโรคทเคยไดรบการฉดวคซนมากอน
• ผสมผสโรคทเคยไดรบวคซนมากอน: ผสมผสโรค ทเคยไดรบวคซนแบบกอนสมผสโรคครบ หรอ ได
รบวคซนแบบหลงสมผสโรคดวยวคซนเพาะเลยง มากอนอยางนอย 3 เขม ใหฉดวคซนกระตนโดย
ไมตองให RIG
การฉดวคซนกระตนสำาหรบผสมผสโรคทเคยไดรบการฉดวคซนมากอน
เทคนคการฉด RIG
• หากไมสามารถหา RIG ไดในวนแรก เมอจดหาได แลวควรรบฉดโดยเรวทสด แตถาฉดวคซนเขม
แรกไปแลวเกน 7 วน จะเรมมภมคมกน ไมใหฉดRIG เพราะจะกดภมคมกนทกำาลงสราง
• ถาใช HRIG ไมตองทดสอบการแพ แตถาใชERIG ตอง intradermal skin test กอนใช
การทดสอบการแพ ERIG ทผวหนง
• เจอจาง ERIG 1:100 ดวย NSS• ฉด 0.02 ml เขาในบรเวณทองแขนดวย
tuberculin syringe จนเกดรอยนนเสนผา ศนยกลาง 3 มม.
• ฉดนำาเกลอเขาในผวหนงบรเวณทองแขนอกขางหนงเพอเปรยบเทยบ
การทดสอบการแพ ERIG ทผวหนง
การอานผล• รอ 15-20 นาท จงอานผล• ถาจดทฉด ERIG ม wheal เสนผาศนยกลาง 10 มม.ขน
ไป และม flare ลอมรอบ แตจดทฉด NSS เปรยบเทยบ ไมมปฏกรยาน แสดงวาผลทดสอบเปนบวก
• กรณผลทดสอบทผวหนงเปนบวก– ตองเปลยนไปใช HRIG– ถาไมม HRIG ควรให ERIG ดวยความระมดระวงเปนพเศษ
• หลงฉด ERIG ตองใหผปวยรอ เพอเฝาระวงอาการแพ อยางนอย 1 ชม.
การฉด RIG ใหไดผล
• ฉด RIG รอบแผล โดยแทงเขมลกเขาใตบาดแผลคลายกบวธฉดยาชาเฉพาะท
• ถาม RIG เหลอใหฉดเขากลามเนอในตำาแหนงทไกลจากจดทฉดวคซน
• ไมใช RIG ขนาดสงกวาทแนะนำา เพราะจะไปกดการสรางภมคมกนจากการฉดวคซน
• กรณทบาดแผลกวาง หรอหลายบาดแผล แต ปรมาณ RIG ทคำานวณไดมจำากด แนะนำาใหเพม
ปรมาณดวยการผสมกบ NSS ประมาณ 2-3 เทา จนไดปรมาณทตองการ เพอฉดไดครบทก

• Majority: within 60 min• biphasic phenomenon
– 3-20% of patients– caused by a second phase of mediator
release, peaking 8-11 hours after the initial exposure and exhibiting itself clinically 3-4 hours after the initial clinical manifestations have cleared

การพจารณารบผปวยไวรกษาในโรงพยาบาล
1. ผปวยทมอาการและอาการแสดงซงบงวาไดรบ พษเขาสรางกาย (systemic envenoming)
2. ผปวยเดก โดยเฉพาะเดกเลก3. ผปวยทมอาการเฉพาะทรนแรง4. ผปวยทมอาการแสดงทวไปอนๆ เชน เปนลม
หมดสต ความดนโลหตตำา หรออาการแพพษง
การเฝาสงเกตอาการ
• งทมพษตอระบบประสาท: ควรรบผปวยไวในโรง พยาบาลเพอสงเกตอาการ 24 ชวโมง
• งพษตอระบบโลหต: สงเกตอาการจนครบ 3 วน หลงถกกด pt ควรไดรบการตรวจรางกาย CBC
VCT ( หรอ PT) อยางนอยวนละครง• งแมวเซา: ควรรบผปวยไวในโรงพยาบาลเพอให
สารนำา อาจมอาการไตวาย ควรตรวจ CBC VCT เปนระยะ ตดตาม urine output

ซำาไดอก จน VCT ปกต หลงจากนนควรทำา VCT ซำาอกประมาณ 24 ชวโมง โดยเฉพาะในผปวยทม

– usually occurs after strenuous physical activity in a hot environment
• Nonexertional heat stroke– more commonly affects chronically ill or
debilitated patients and persons at the extremes of age, especially during a prolonged heat wave

Electrical Injuries
• Low voltage (≤1000 V)• High voltage (>1000 V)
• Standard household electricity is low voltage alternating current (AC), power lines are high voltage.
Clinical Features
• Mechanism of injury– direct tissue damage from electrical energy – thermal damage from heat created by tissue
resistance– mechanical injury induced by a fall or tetanic
muscle contraction• Severity of injury and tissues
– voltage, duration of contact, tissue resistance, and path of current
Clinical Features
• Cardiac complications– arrhythmias and QT prolongation– high voltage injuries
• Temporary loss of consciousness is common.
Clinical Features
• Severe burns: contact with high voltage lines
• The size of the skin injury does not correlate well with internal injuries.
• Traumatic injuries frequently accompany electrical injuries.
Emergency Department Care and Disposition
• ABC• Monitor V/S• Monitor urine output with a Foley catheter.• Monitor for rhabdomyolysis, compartment
syndrome and renal failure. • Treat rhabdomyolysis with aggressive fluid
rehydration aiming for a urine output of 2 mL/kg/h.

Drowning
CLINICAL FEATURES
• aspirate water into their lungs have washout of surfactant
diminished alveolar gas transfer, atelectasis,
ventilation perfusion mismatch, and hypoxia
CLINICAL FEATURES
• Noncardiogenic pulmonary edema• Mental status: normal-comatose• hypothermia
DIAGNOSIS AND DIFFERENTIAL
• Respiratory acidosis metabolic acidosis• Early electrolyte disturbances: unusual• A CXR is usually obtained but is frequently
normal in patients who are otherwise asymptomatic.

• Data do not support routine antibiotic prophylaxis for pulmonary aspiration.

Paraquat and Diquat
• toxic with lethal oral doses of 20% solution– 10-20 mL in an adult – 4-5 mL in children
• Both agents are toxic via inhalation, dermal exposure, or ingestion.
• Due to their caustic properties, ulceration of skin and mucous membranes and gastrointestinal corrosion can occur.
• result in – renal– cardiac– hepatic failure– pulmonary fibrosis

• Toxicity from these alcohols results from the formation of toxic metabolites, which produce a significant anion gap metabolic acidosis.

• Symptoms of methanol toxicity may not appear for 12-24 h after ingestion until toxic metabolites accumulate.
• Signs and symptoms– CNS depression– visual disturbances (classically, a complaint of
