Foreword by the ChairI am pleased to present North LincolnshireSafeguarding Adult Board’s 2017/18 AnnualReport. The report demonstrates the success ofthe Board in meeting our objective of effectivesafeguarding practice across the NorthLincolnshire area.

As Chair of the Board I would like to thankmembers for their commitment and hard workthroughout the year and congratulate them on theprogress that has been made in meeting theBoard's strategic priorities. I am particularlypleased with the completion of our newSafeguarding Board website, devised inpartnership with the 'expert by experience' group;a co-production that has ensured the site providesuseful information that is accessible by all. It hasalso proved to be a very strong year forprofessional development. Safeguarding AdultsBoard representatives delivered a full week ofworkshops that raised awareness and increasedknowledge around such important safeguardingsubject areas as: Mate Crime, Modern Slavery,Making Safeguarding Personal, Mental Capacityand Doorstep crime. Practitioner attendance wasvery high, and feedback indicated the workshopswere well received providing a valuable increase inknowledge and skills in these particularsafeguarding areas.

It has also been encouraging to note that qualityassurance of our safeguarding activity, andscrutiny of our safeguarding intelligence dataduring 2017 has indicated a consistent level ofgood safeguarding practice in North Lincolnshire.The number of adult at risk enquiries is concurrentwith the national average picture, and particularskills have been demonstrated in the completionof mental capacity assessments and themaintenance of the 'making safeguardingpersonal' approach to all safeguarding concerns.

North Lincolnshire Safeguarding Adults Boardcontinues to carry out learning and safeguardingadult reviews into cases when adults have died orbeen seriously injured and there is a concern thatpartners could have acted more effectively.Lessons from the reviews have been highlighted tothe Quality Assurance and training groups toimprove inter agency safeguarding practice.

I look forward to our Safeguarding Adult Boardconference in 2018 with a focus on SafeguardingAdults reviews and what good safeguardingpractice looks like. I welcome the opportunity forthe Board to build upon the strengths of its currentsuccess during the coming year.

About the Safeguarding Adults BoardIn accordance with the Care Act 2014, the objective of the Safeguarding Adults Board is to ensure effective co-ordination of services to safeguard and promote the welfare of local adults who may be at risk of abuse andharm. The Board is made up of senior officers nominated by each member agency. Members have sufficientdelegated authority to effectively represent their agency and to make decisions on their agency’s behalf. Theyhave access to those responsible for making the decision for which they do not have delegated authority. Ifthey are unable to attend board meetings for any reason they send a nominated representative of sufficientseniority.

The Board promotes a ‘Making Safeguarding Personal’ approach as a golden thread that runs through allstrategic and operational adult safeguarding work in North Lincolnshire. Making Safeguarding Personalplaces emphasis on the individual and the outcomes each person wishes to achieve to stay safe. MakingSafeguarding Personal is ever present in the Safeguarding Adults Board strategy and the work that the Boardundertakes.

During 2017 /18 the Safeguarding AdultsBoard Executive group, Police, NorthLincolnshire Council, ClinicalCommissioning Group, met every six weeks,the full board met quarterly. Actions completed by the Safeguarding Adults Board to meet its objectivesCommunicationand engagementCommunity Engagement with those who useservices, those who deliver them and the widerpublic.

Information, advice and engagement are essentialcomponents to enable the Safeguarding AdultsBoard to realise its vision for safeguarding in NorthLincolnshire. The Safeguarding Adults BoardCommunication and Engagement Groupreformulated its membership and strategy to meetthe Board priority of raising safeguardingawareness. The membership of the groupincorporates communication representatives fromPolice, Humberside Fire and Rescue, NorthernLincolnshire and Goole NHS Foundation Trust,Healthwatch, North Lincolnshire Council.Consultation takes place, with other partnerorganisations and with adults who experienceservices, on a regular basis in relation to specificindividual communication events.

The group understand the importance of using themethods of communication preferred by mostpeople, surveys have been completed withpractitioners and the local community that haveidentified social media and leaflets as the prioritymeans of raising safeguarding awareness. During2017 the Safeguarding Adults Board launched itsindependent website at www.northlincssab.co.uk

The site provides a range of information aboutwhat adult abuse is, how to report a concern,safeguarding news and resources, a professionalpractice area and a calendar of events facilitatedby the Safeguarding Adults Board and itsindividual partners.

The Communication Group also launched itsSafeguarding Adults leaflet. The leaflet has been

co- produced with North Lincolnshire Councilexperts by experience group. It offers advice andguidance about how adults can combat doorstepcrime in the area.

Leaflets can be downloaded and printed asinformation by accessing the website and can beused any organisation or distributed within thecommunity as needed.

In addition to the website, and leaflets a number ofarticles about safeguarding and Board activityhave been published in local media outlets thathave a wide established circulation. Informationin relation to the Board, the Strategic Plan andMaking Safeguarding Personal has been publishedin various newsletters for Healthwatch NorthLincolnshire, the North Lincolnshire Council AdultInformation Service and Community Hubs.

Throughout the year the Board and its partnershave promoted safeguarding awareness via anumber of events within the locality. TheSafeguarding Adults Board were represented atthe Public Health conference Mental Health Day(May 2017) and celebrated the 10 year anniversaryof the Mental Capacity Act with participation in apublic event, engaging members of thecommunity in conversations about keeping safe.The Board have also engaged with members of thecommunity at a number of local events includingRescue Day (July 2017) and the Voluntary Alliancefor North Lincolnshire (October 2017).

The SAB have supported local interest groupsincluding the Seniors Forums and Experts byExperience, in which members have participated inthe co-production of safeguarding information forthe public and the SAB business plan to meet itspriorities. Engagement with the community hasresulted in the completion of 150 questionnairesrepresenting people’s views of safeguarding.

People wanted to know where they could accessinformation about safeguarding and who tocontact should they require help. The majority ofpeople told us that they would access the internetto try and find this information. During 2017 the

Safeguarding Adults Board website was launchedand we continue to use this medium to raise theprofile of Safeguarding amongst professionals andpeople within the community.

Workforce trainingand DevelopmentA skilled and knowledgeable workforce.

To keep people safe at home and in health andcare settings, it is important that professionalsdelivering services have sufficient skills andknowledge to carry out their safeguarding duties.The Safeguarding Adults Board led thesafeguarding training and development across theworkforce through its Workforce DevelopmentGroup, chaired by Sarah Glossop of the ClinicalCommissioning Group. A review of the workforcedevelopment plan was completed to ensure it is fitfor its purpose of equipping professionals,volunteers and carers with the safeguardingknowledge they need.

The workforce Development group also reviewedthe content of the basic level 1 SafeguardingTraining to ensure that all organisations aredelivering the same knowledge to an agreedstandard across the area of North Lincolnshire. Safeguarding Adults Reviewsand learning DevelopmentA culture of continuous learning and improvementacross the organisations

The Safeguarding Adults Board is committed to ashared culture that values and facilitates learningto improve safeguarding practice. During 2017/18the Safeguarding Adults Review and LearningDevelopment group, chaired by Mark Kelk,Humberside Police, completed one SafeguardingAdults Review, identified a further SafeguardingAdult Review to take place and agreed a learningreview. The group also considered a briefingfollowing an important review of financial harmthat took place in a neighbouring council.

Learning Review L

L was a gentleman who died 2017. He experiencedmental health difficulties and had received servicesfrom a number of partner agencies. A review of theevents leading to his death led to revision of theagency referral protocol to the Local AuthoritySafeguarding Adults Service.

Lincolnshire Council Financial SafeguardingAdults Review

Lincolnshire Council completed a SafeguardingAdults Review into the theme of financial harm thattook place within the county over a period of 10years with multiple victims. A seven minute briefingwas shared with the group to highlight the issuesinvolved in the review and agree dissemination toBoard partners.

The SAR group have completed an audit timetableto check that organisational partners have enactedrecommendations from previous SafeguardingAdults Reviews that took place during 2016. Thereis a close link across all four subgroups, ensuring

that information and learning from practice isdisseminated to all partner organisations toenable them to make changes or improvementsthat will keep adults safer in the future.

7 Minute Briefing - lincolnshire SAR

1 – Background The LSAB received informationfrom the Police relating to 34 adults who were

the victims of financial exploitation in aLincolnshire market town. It was

believed the individuals had beentargeted due to their

vulnerabilities. The policeled an operation.

2 – Why was a SAR Commissioned?The LSAB commissioned a SAR to see if there were any lessons to be learnt regarding how agencies worked together to safeguard individuals.

Section 6 There weresubstantial blocks in multi-agency

working. Agencies routinely did not record detailsof alleged perpetrators which severely impaired policeefforts to gather evidence and secure prosecutions.

There were occasions when safeguarding referralswere made through the safeguarding process butdecisions were made to not take any further action. This created missed opportunities to reduce the risks to the individual and the wider community.

4 – What the reviewidentified The review found there

wasn’t a hidden picture and the abuse was knownto the agencies working with the individuals.

The circumstances of all the cases were complexand the adults were often unwilling and unable

to accept help.

The majority of individuals had MH needs.Drug and alchol dependency often left

individuals in highly vulnerable situations.

Section 7 The views of the individuals made valuable contributions to the learning.

The review highlighted many committedPractitioners/agencies. The majority of peopleinvolved found the review process positive.

The police investigation began when oneofficer began to piece together informationin relation to the individuals, the officerwas very proactive in raisinghis concerns.

3 – SAR Information The reviewconsidered a seven year period with a focus

on the most recent 18 months. The SARlooked in detail at the experience of 10people. Three of the 10 were willing to

contribute their views to the review.

Quality Assuranceand performanceSafeguarding responses are timely,effective and personal.

The primary aim of the Quality Assuranceand Performance group is to monitor andevaluate the effectiveness of partners tosafeguard adults in North Lincolnshire.The group has established a safeguardingdata dashboard which provides quarterlyperformance information used tohighlight any emerging themes in relationto safeguarding, contributing to theformulation of any actions required toimprove practice. The group consider theinformation in the data dashboard andthe Safeguarding Adult Collection (Agovernment requested return ofsafeguarding information), completing ananalysis of safeguarding performance.Further action is undertaken to exploreparticular themes in more detail and toidentify areas of good practice. Themed

audits are carried out to look closely atinformation provided to reassure theBoard that appropriate action is beingtaken to protect adults from abuse.

During 2017 666 safeguarding concernswere received, 281 became safeguardingenquiries and 298 safeguarding enquirieswere completed during the year.

There was a high level of performance inrelation to risk identified action taken,consistent with the previous year. Thisapproach has ensured adults were madesafe. In cases where a risk remained, thiswas in accordance with the wishes of theadult subject to the enquiry. The MakingSafeguarding Personal (MSP) approachhas also remained consistent with theprevious year demonstrating MSP hasbeen embedded into safeguardingresponses. Of those adults involved inenquiries, 96% of individuals were askedtheir views and wishes in relation to theS42 enquiry, in comparison to 86% duringthe previous year.

The safeguarding data indicates a strong response to both the issues of self-neglect and domestic abuse. A high proportion of enquiries elicited a goodmulti agency response to meet the needs of the adult at risk.

A profile analysis of the quality assurance data indicates that white Britishfemales are most in receipt of a safeguarding response. These adults arepredominantly over the age of 65 years, with physical needs identified as theprimary support reason.

Individual profileThe highest number of referrals came fromresidential care homes. This should be viewedin the positive context that care homes arevigilant about recognising and addressingsafeguarding issues. It should be seen as acontributory factor to the high quality care

provider market that currently exists withinNorth Lincolnshire. This is endorsed by CQC asduring 2017/18, 84% of care homes were ratedas either good or outstanding and 100% ofhome care providers were rated as good.

Development SessionsIt is a key function of the Board to develop goodsafeguarding practice across the area; it is with thisaim in mind that the Board ensures safeguardingdevelopment sessions take place at each meetingto keep members informed about particularsignificant safeguarding issues and updates. It isexpected that each member will disseminate thisknowledge within their individual organisations.

Quality Assurance line of sight to practice

In June 2017 the Board completed a developmentsession centred on a line of sight to practicelooking at two safeguarding cases in detail. Oneincluded a good practice case of MakingSafeguarding Personal involving the police, theother a case of challenge that involved aresidential care home. In both cases theinformation was fully explored with highlights ofwhat actions were successful and how challengeswere addressed to ensure adults were safe.

Safeguarding Adults Reviews

A second development session was completedwith the Board in October 2017, focusing on thetopic of Safeguarding Adults Reviews. This sessionwas led by North Lincolnshire Council, with Boardmembers taking part in a discussion about therelevance of Safeguarding Adults Reviews forimproving organisational safeguarding practice.

trading Standards and Doorstep Crime

In accordance with Care Act 2014, StatutorySupport and Guidance, the Board undertook afurther development session in December tostrengthen links between the Safeguarding AdultsBoard and its Trading Standards partners. Boardmembers received information about local issue ofdoorstep crime and scams, with guidance abouthow agencies can work with Trading Standards toreduce and prevent this area of harm to adults.

Assurance of SafeguardingpracticeA key objective of the Safeguarding Adults Board isto assure itself that local safeguardingarrangements are effective, and partners act tohelp and protect adults in its area. TheSafeguarding Adults Board has a strategic role thatis greater than the sum of the operational duties ofcore partners. It oversees and leads adultsafeguarding across the area and will be interestedin a range of matters that contribute to theprevention of abuse and neglect. In 2017/18 theSafeguarding Adults Board had oversight of twopartner’s inspections and action plans to improvesafeguarding. Details and outcomes of partnerinspections are presented to the Board Executivegroup, and kept under review to gain anunderstanding of the progress of anyrecommendations made during inspection. Twoinspection outcomes have been monitored by theBoard during 2017.

Assurance of safeguarding arrangements alsoextends to scrutiny of the safeguarding datainformation both locally and nationally. TheSafeguarding Adults Board review thesafeguarding picture quarterly monitoringprogress via the agreed safeguarding qualityassurance dashboard.

Safeguarding Adults BoardSelf-AssessmentThe Board completed an annual self-assessmentprocess with partners to discover how well itsorganisations were carrying out safeguardingresponsibilities, and to review their contributoryrole to the Safeguarding Adults Board.

Members of the Board were requested to completethe agreed Yorkshire and Humber self -assessmenttemplate that captures information about theorganisations safeguarding structure, practice andprocedures.

Organisations were invited to attend a self-assessment panel to discuss the content of theircompleted template. The self-assessment panelconsisted of: Moira Wilson SAB Chair, Karen PaveyDirector Adults and Community Well Being, GeorgeDucker voice of the service user, Barbara StarnsSAB Manager.

Self-Assessment paneloutcomes1. All agencies had in place robust internal

safeguarding strategies that includeduser/carer involvement. Systems forsafeguarding prevention, reporting andlearning were also in place.

2. Organisations believed their leadership,management and workforce were clear aboutsafeguarding roles and responsibilities.

3. In relation to partnership, organisations wereable to demonstrate regular attendance at theBoard and support to the progression of theBoard strategy and business plan within thesub groups.

4. Commissioners were confident that highquality services were available in NorthLincolnshire, and where there had beeninadequacy in inspection reports qualityassurance processes were in place to overseeimprovement.

5. Several partners highlighted a lack ofunderstanding in relation to MCA/Dols in someareas of their service. MCA/Dols has beenadded to the Board Strategic plan and will be afocus for the training strategy going forward.

6. Several organisations rated their assuranceregarding audits of the quality andeffectiveness of their safeguarding training asamber. An action has been passed to the SABtraining group to focus on this area of workwith members.

executive GroupThe Clinical Commissioning Group took theopportunity at the panel discussion to provide anupdate regarding new governance arrangementsand a new leadership team. Representatives werealso able to assure the panel that the CCG issighted on adult safeguarding, in particular theHead of Safeguarding requirement that asafeguarding report is submitted to the CCGQuality Board quarterly and the production of asafeguarding section of information within the bimonthly Public Quality Report.

The panel welcomed the commitment of the CCGto lead involvement of the Primary Care Voice inthe Board going forward.

North Lincolnshire Council took the opportunity toupdate the panel about reorganisation of services,highlighting the positive benefits of housing theAdult Safeguarding Team, Dols, and ProviderDevelopment together. This co location buildsstrong safeguarding relationships and helps aunified approach. It was also noted that theClinical Commissioning Group safeguarding nurseis now embedded in the Adult Safeguarding Team.

Humberside Police emphasised to the panel theircommitment to safeguarding indicating thatvulnerability is a police priority that runs as agolden thread through the organisation. Howeverthe ‘vulnerability’ term used by the police has awider definition than that used by the Care Act inits definition of safeguarding. The DetectiveSuperintendent of Humberside Police wouldwelcome further discussion about how this worksin practice.

Humberside Police have reorganised to a morelocalised north and south bank approach whichwill enable the Board representative from thePolice (Detective Superintendent) to focusattention to both North and North EastLincolnshire. The Police continue to fully supportthe Board, and a representative is chair of the SABSafeguarding Adults Review Group. In response toa query from the Expert by Experience on thepanel the Police confirmed that updates would beprovided regarding the progress of ‘safe places’and the Herbert Protocol.

Wider Board MembersSeveral organisations gave examples of howservice users were involved in their services, notingareas such as co-production of systems andinformation. Recording of individual responses toa service and representation on governancegroups. Ongo housing association, in particular,gave details about minority representation withinits organisation, including a Syrian spokespersonto represent Syrian refugees in supportedaccommodation. Tenants are also represented onits governing board. NLaG assured the panel thatregular liaison and feedback to patients and carerstakes place, family and friends’ questionnaires areembedded within the Trust. A patient Experiencereference group has been developed and involvesthe Safeguarding Adults Team.

Organisations provided information and discusseddevelopments in their own services, thatcontribute to the wider safeguarding agenda, andwhich are linked to the SAB Business plan andstrategic priorities.

RDaSh described the new Mate Crime initiativethat they are undertaking in the area. The Trustare also in the process of implementing a oneservice IT system which will enable more effectiveworking arrangements. Humberside Fire andRescue service provided an update in relation toits strategic response to cases of significanthoarding and self-neglect to agree across agencysolutions. Probation informed the panel ofchanges to the service nationally with particularrelevance to early intervention work in prisonsprior to prisoner release. This is viewed as a

positive change enabling offenders to have agreater degree of support and preparation forrelease. Nick Hamilton Rudd from Probationwould also like to highlight a good practice multiagency case for the full Board meeting in June.

NLaG confirmed to the panel that followingsafeguarding issues raised by CQC at a recentinspection, a great deal of work had taken place toaddress areas of risk, particularly in relation tosuicide risks within the A & E Department. It wasrecognised that improvements would not takeplace over night but that the NLaG Boardrepresentative would keep the Board updated withprogress.

NHS England outlined the process by which theBoard will receive assurance that improvementsare made in health organisations such as NLaG.Arrangements were confirmed regardingcommunication between NHS England and therisk summit process which is put into place whenthere is deemed to be high risk as a result oforganisational failures. An area of focus is theLeDeR programme and a process of review istaking place to identify good practice in this area.This work with link with North Lincolnshire SAB‘Appreciative Enquiry’ programme to identify whatworks well in safeguarding practice.

I have found it very interesting to sit on the

Safeguarding Adults Board Self-assessment panelsand to listen to the different proposals that thevarious organisations are planning to bring intoeffect.

The self-assessment panel process has proved tobe a very worthwhile exercise to identify the highlevel of positive safeguarding activity that is takingplace by the partners of the Safeguarding Adults

Board. Areas where there has been seen to bedeficiencies have been quickly addressed bymember organisations. Actions identified by theprocess have been translated into theSafeguarding Adults Board Business Plan and arekept under regular review. The self-assessmentplaces the Board in a strong position for itsdevelopment session in November and thecompilation of its new strategic plan for 2019.

Conclusions and futureprioritiesThis annual report highlights the considerableprogress the Board has made to raise awareness ofsafeguarding and embed safeguarding duties intopartner practice. In particular, success in theimplementation of making safeguarding personal,and application of the six safeguarding principlesof: Empowerment, Prevention, Protection,Partnership, accountability and proportionality.

It is the intention of the Board to review itsStrategic plan and priorities during 2019, buildingon the outcome of the Safeguarding Conference inOctober 2018. Consultation will take place withadults who use services, professionals and widermembers of the community about what being safemeans to them. The consultation will be theplatform on which to build the safeguardingstrategy and priorities for the Board movingforward.

Partner contributions to the Board Prioritiesnorth lincolnshire CouncilKeeping Adults Safe at Home

North Lincolnshire Council has embedded MakingSafeguarding Personal (MSP) and made it integralto all safeguarding activities, in a way thatenhances involvement, choice and control. TheCouncil recognises that MSP is not an approachlimited to statutory enquiries but is fully integratedthroughout the whole spectrum of safeguarding.

The Council have strong multi-agencypartnerships, joint working and sharingintelligence to ensure a swift and personalisedsafeguarding response, involving the adult in allaspects of decision making. The Councilrecognises the importance of early positiveinterventions with individuals and their families toprevent the deterioration of a situation orbreakdown of a support network.

The Council are members of the Humberside FireService Strategic Group who are currently workingon the development of a Humberside widestrategy for tackling self-neglect and hoarding.

Keeping Adults Safe in Health and Care Settings

The Council continues to work closely withproviders and partner agencies, providing adviceand support where appropriate. Regular audits areundertaken and action plans are developed toensure continued improvement. The Council alsohas an excellent relationship with the Cross SectorProvider Partnership and the Safeguarding Teamhas delivered tailor-made training around

Safeguarding and Making Safeguarding Personalto providers.

The Council maintains strong links with the CCGand CQC and are core members of the Health andSocial Care Standards Board. The Standards Boardprovides a local framework to enable sharedaccountability for improving standards withinresidential, nursing and domiciliary care. Partneragencies and providers work collaboratively todrive up the quality of health and social careprovision within North Lincolnshire.

The Safeguarding Named Nurse within the ClinicalCommissioning Group has co-located with theSafeguarding Adults Team providing advice andguidance to the team in relation to specialisthealth safeguarding concerns. The Council haswelcomed this opportunity to work collaborativelywith health colleagues to ensure goodsafeguarding outcomes for all adults.

Raising Awareness of Safeguarding

Throughout the year the Council have initiatedand taken part in a number of awareness raisingevents.

A Mental Capacity Act 10th Anniversary Day, tookplace on the 22 September 2017. The day wasaimed at increasing knowledge, and alsoemphasised the importance of the MentalCapacity Act in underpinning good safeguardingpractice amongst both professionals andmembers of the public. The Safeguarding Teamattended North Lincolnshire Rescue Day in July2017 to raise awareness and knowledge about

how to keep adults safe and report abuse.

The Council arranged and promoted an AutismAwareness Week in March 2017. The event wasaimed at raising awareness and educating peopleto know more about the condition and the links tosafeguarding. Various activities took place over thecourse of the week at both the Ironstone Centreand Brumby Learning Centre in Scunthorpe.

The Board Leads Multi-agency SafeguardingEffectively

The Council continues to play an active role on theSafeguarding Adults Board, providing arepresentative for each Action Group, and chairingthe SAB Quality Assurance and PerformanceGroup.

The Council is committed to multi-agency workingand have contributed to Safeguarding Week byleading two workshops; one in relation to MentalCapacity and the other from Trading Standardsaround scams and doorstep crime. Boardrepresentative also facilitated a developmentsession on Safeguarding Adults Reviews (SARs) tothe full Board to further knowledge and skills inthis area.

Humberside policeHumberside Police co-ordinate safeguardingthrough a Central PVPU based within the SpecialistCrime Command. Appropriate action is takenagainst those who offend either by prosecution orcivil remedies and where there are areas ofconcern these are shared with partner agencies toensure a multi-agency approach is considered.

Humberside Police works collaboratively with arange of partner agencies to support service userswho have been victims of crime or are at risk ofabuse or neglect.

Throughout 2017/18 numerous successfulprosecutions were brought in respect ofallegations of physical abuse, sexual abuse, theftand domestic abuse involving vulnerable victims.

In addition to the successful prosecutions therehave been hundreds of safeguarding concernswhich have been reviewed and then acted upon toensure appropriate multi-agency intervention toprotect those at risk and improve their lives.

We have a dedicated Detective Sergeant workingwithin the Local Authority who is responsible forthe co-ordination of referrals and concernsidentified through police contact. We ensure thatwe share appropriate information with our partneragencies to safeguard the needs of adults andchildren. The Force ensures that there isengagement with front line Officers and inparticular those who work within Communities tosafeguard the needs of the community.

The force has delivered extensive training tosupport front-line officers with a greaterunderstanding of how to respond to vulnerablepeople with mental health issues, AutismSpectrum Disorders and understanding theHerbert Protocol in respect of mentally vulnerablepeople who go missing.

Within Humberside Police awareness has beenraised in respect the emerging issue of Modern DaySlavery and Human Trafficking through local andregional training events and the creation ofspecially trained Police Tactical advisors.

In addition Humberside Police have deliveredDomestic Abuse Matters training to a significantproportion of the force aimed at raising awarenessof the impact of DA on victims and the need forpositive intervention and support.

To further improve the response to adults in crisisand those with acute mental health issues,Humberside Police now have a crisis worker fromMIND now based within the force command centreseven days per week between 1600hrs and2200hrs. This collaboration ensures specialisttactical advice to call-handlers and responseofficers and enables immediate mental healthintervention to support members of the publicwhere required.

Humberside Police have implemented OperationSignature focussed on identifying VulnerableAdults who have been subjected to financial harm.The operation is intelligence led through referralsfrom the National Crime Agency and other partnerorganisations using the Banking Protocol. The

Banking Protocol was set up to help financialinstitutions identify Adults who may be in theprocess of being financially abused or are beingfinancially abused. The operation has beenrunning since January 2018 but has identified 4victims from North Lincolnshire who have all beenvisited by officers from Humberside Police.

We have actively engaged in a number ofSafeguarding Adults Reviews and multi-agencyaudits and disseminated this learning across theOrganisation where appropriate.

Humberside Police are committed to sharinginformation with partner agencies in respect ofthose who work with vulnerable adults eithervoluntary or paid.

NLCCG have a duty to safeguard adults in allaspect of their commissioning. This includesestablishing effective structures for safeguardingwith clear strategies, robust governance and acompetent workforce that can lead and developsafeguarding across the local health community.Through their safeguarding standards, NLCCGseeks to ensure that all their commissionedservices:

• Support patients to reduce risks of neglectand abuse – according to the patientsinformed choices

• Reduce risks of abuse and neglect occurringwithin their service through the provision ofhigh quality, person centred care

• Identify and respond to neglect and abuse inline with local multi-agency safeguardingprocedures

NLCCG is the provider of the Continuing Health Care(CHC) assessment service for patients registeredwith North Lincolnshire GPs. In working with partneragencies and individual patients, this service is in aprime position to identify and respond tocircumstances in which risks are identified foradults with care and support needs, whether theirprimary health needs are met with a package ofcare in their own home, or in a nursing home.

The Designated and Specialist Nurses forSafeguarding worked closely with NLCSafeguarding Adult Team, with the SpecialistNurse co-located with the Team for part of theyear. This close partnership working aimed toensure that appropriate health professionalsupport was available to multi-agencyinvestigations.

The Designated and Specialist Nurses haveworked closely with CCG colleagues (in particularCHC staff) in identifying and responding toconcerns arising in home or care settings.

The CCG’s Head of Nursing has provided strategicclinical nursing input into consideration anddecision making in respect of quality ofprofessional care.

Raising Awareness of Safeguarding

All staff members within the CCG have mandatorytraining on Safeguarding adults which includesraising awareness of how to keep adults with careand support needs safe.

Staff in regular, direct contact with adults with careand support needs, including those involved in theassessment for Continuing Health Care haveadditional training to enable them to actappropriately. The CHC team have regularsafeguarding supervision from the SpecialistNurse.

All CCG staff have regular access to safeguardingcase management support via the Designated andSpecialist Nurses – Safeguarding

Both the Executive Lead, and the DesignatedNurse contribute to the development of the SAB,its systems and processes

NLCCG has appropriate representation on allAction/ Reference Groups with the DesignatedNurse for Safeguarding chairing the Workforce andDevelopment Group, and acting as vice chair forthe SAR Group. The Designated Nurse has ensuredthat there has been appropriate health servicerepresentation at SAR Action/ Reference Groups.

The Designated Nurse has supported the BoardManager in rolling out of new multi-agencyarrangements, in particular, following publicationof national guidance on responding to pressureulcers.

Humberside Fire & RescueServiceKeeping Adults Safe at Home

HFRS has a Safeguarding Policy in which all staffhave received training and awareness.

We are fully involved in Safeguarding throughoutthe service area and provide solutions to fire riskand general safety issues affecting people in theirown homes. This has particular relevance for themost vulnerable in our community who we mayrefer to other organisations to seek any specialistsupport that we are unable to provide.

Following a home safety risk assessment we areable to recommend/provide a range of equipmentto assist vulnerable people to live independently.This includes equipment such as fire retardantthrows and mats, sprinklers, cooker guards andsmoke alarms.

HFRS are also in the process of developing our Safeand Well Visits to enable us to identify and provideinformation on a wider range of issues to peoplewith vulnerabilities throughout the Service area.

We maintain an extensive secure database of allour interactions and personal vulnerabilities.

All staff are DBS checked.

HFRS are currently providing an Emergency FirstResponse within North Lincolnshire that assists inproviding a level of initial medical assistance insupport of EMAS.

HFRS continues to support Board training eventsand is working closely with partners to improve ourinteractions with vulnerable people in particular in

terms of cases of self-neglect and hoarding.

HFRS has resourced a 6 month trial to utilise theBlue Light Brigade Volunteers to assist us inengaging with more vulnerable people in ourcommunity. In North Lincs it is intended that thevolunteers will focus on rural locations rather thanScunthorpe. These volunteers have all had trainingfrom HFRS they are DBS checked and are formedfrom retired blue light service workers andtherefore have considerable experience already.

Keeping Adults Safe in Health and CareSettings

The Serviceprovides copy reports to North LincolnshireCouncil via the Health and Social Care Board of theoutcome of visits to adult care premises. TheService has made a pledge to the CorporateParenting Group to provide support, advice andassistance to young care leavers moving ontoindependent living.

Raising Awareness of Safeguarding

Staff receive regular safeguarding updates toensure that we provide the best possible service tothe most vulnerable in our community.

The Service utilises a range of communicationmedia to highlight timely, periodic safety updates.

HFRS know only too well the challenges we face asa Service when an adult we are working withrefuses to engage or accept our support; who self-neglect and hoard. HFRS are sure that otherorganisations face the same difficulties. Werecognise that there may come a point when aperson no longer has the ability to protect

themselves by controlling their own behavior;without multi-agency working together to managethe best way forward. To address this HFRS havecontacted all four Safeguarding Adult Boardswithin the Humberside area.

• We are jointly working to create a Self-Neglectand Hoarding Guidance which all agencieswill be able to easily follow.

• Clear training will be in place to enablepartners to identify risk; assess the riskthrough timely information sharing andmulti-agency intervention.

• All four Adult Safeguarding Boards will havemet HFRS by mid-June to start the ball rollingto create this new important guidance.

• The guidance purpose is to evaluate risk,formulate plans, identify which agencies willbe responsible for intervention and reviewaction plans to evaluate effectiveness andmonitor change and outcomes.

The Board has reviewed its structure and how itconducts its business in recent months. TheExecutive Group meet regularly to discuss theBoards objectives. Information is promulgated tomembers of the full Board as and when requiredand full board meetings areless frequent.

The new approach appearsto be working well whilstalso impacting less on theschedules of the widerBoard Members.

NLaG provide services within both the hospital and community setting. The safeguarding teamactively screen incident reports that link topossible safeguarding issues, this ensures aproactive approach to case management andallows for trends to be identified at an early stage(within hospital and community ) and improve thereporting of safeguarding issues. A member of theteam also sits on the community governancegroup looking at community incidents. Any issues /trends are monitored within the safeguardingadult’s forum.

The safeguarding adults training strategy wasreviewed in April 2017. Current training levelscontinue to be maintained at around 85% and as of 30th April 2018 84% of trust staff havereceived safeguarding adults training. As a result ofthis staff are more able to recognise safeguardingissues and as such more staff seek advice withregards to safeguarding concerns. Specifically wehave had several cases whereby elderly clientshave reported domestic abuse and have beenreferred appropriately and received the support ofan IDVA.

Keeping Adults Safe in Health and CareSettings

During 2016 and 2017 the trust introduced the roleof specialist nurses for dementia and learningdisability. These nurses work to ensure that theseparticularly vulnerable group of clients receive carethat is suitable and specific to their needs,reasonable adjustments are made (both prior toand during admission) and that any safeguardingissues are identified at an early stage.

In November 2017 the Trust appointed a NamedNurse with responsibility for Mental Capacity Actand Deprivation of Liberty. This post works withinthe safeguarding team to ensure that capacityassessment is at the forefront of patient care andmaintains a focus on care which is delivered in the‘ best interest’ of the patient.

Raising Awareness of Safeguarding

Discussions continue to take place as part of thecare pathway for clients with regards to issues of asafeguarding nature. Where these are low level,general advice will be given to the client / familymember in relation to “what to do next” and ifnecessary how to make a formal referral. Referralswill also be made by NLaG staff in conjunctionwith the client / family where there are issues of asafeguarding nature. Information sharing fromNLaG to the commissioning groups is alsomaintained to ensure that any trends can beidentified within other commissioned services.

The SAB maintain positive challenge with allpartner agencies. In order to maintain a fullytransparent working relationship the chair of theLSAB was invited to attend the NLaG safeguardingadult’s forum to observe its functionality andcascading of information via the governanceroutes.

The SAB maintain a performance data base forwhich NLaG fully contribute.

Safeguarding is at the heart of everything we dowithin RDaSH. In order to prevent or minimise therisk of abuse occurring, and to help protect thosemost at risk in our communities, we believe apartnership approach is essential. We continue toensure that our staff listen and respond to whatadults with care and support needs (and their carers)tell us about that they want to happen to safeguardthem, and to enable them to live their lives asindependently as possible in their own homes.

The staff within the Learning Disabilities Team havecontinued to develop their project in relation toraising awareness of mate/hate crime, and havebeen recognised at a national award level for thework they have achieved.

Keeping Adults Safe in Care and HealthSettings

There continues to be established oversight,assurance, and governance mechanisms formanaging safeguarding quality in RDaSH services.

The Safeguarding Adults Lead works closely with allstaff to ensure that safeguarding incidents arereported and effectively managed to keep patientssafe, and to protect their human rights.

The Safeguarding Adults Team has oversight ofserious or potential incidents via STEIS and IR1

reports; these are all reviewed to ensure thatsafeguarding is threaded through investigations andwider subsequent learning.

In response to the Department of Health and SocialCare’s safeguarding adults protocol for “pressureulcers and the interface with a safeguarding enquiry”(January 2018), a member of the SafeguardingAdults Team is now a member of the Trust’s PressureUlcer Forum, and contributes to the learning anddevelopment in this area.

The Trust continues to have representation on theSAR Sub-Group; during this period the trust hascontributed to one Lessons Learnt Review (LLR),which related to an adult at risk in a care and healthsetting. The SAR Sub-Group representative hasensured that learning points have been cascaded toall relevant staff, and embedded into frontlinepractice.

Raising Awareness of Safeguarding

A culture of “safeguarding adults is everybody’sresponsibility” permeates across the teams andindividuals within the Trust.

Over the last 12 months a significant amount of workhas been undertaken to ensure there is a robustsafeguarding adults governance structure in place.The capacity of Safeguarding Adults LeadProfessionals has been strengthened, and this hasresulted in greater visibility and the formation ofstrong relationships across the partnerships. Theyhave also provided leadership for the widerSafeguarding agenda, including Prevent, ModernSlavery, FGM, Hate and Mate Crime, and Self-Neglect.

The governance arrangements oversee and facilitatethe implementation of safe practice across ourworkforce. There is a strong culture of learning andsharing good practice, based on appreciativeenquiry, and safeguarding training is delivered in linewith research and best practice.

There continues to be a clear vision to achieve thehighest standards of quality and safety, and toembed safeguarding principles, dialogue, and aculture of early intervention/ prevention insafeguarding into all areas of practice.

RDaSH is actively involved in improvingsafeguarding arrangements for adults at risk. It isrepresented at a senior level at all key forumsproviding specialist health advice and leadership,and proactively contributes to the safeguarding ofadults at risk in North Lincolnshire. Ongo has a Safeguarding Vulnerable Adults Policy.

There are safeguarding guidance procedures inplace to assist staff in recognising and reportingthrough their concerns.

All staff are trained and aware of the importance ofreporting their concerns and that safeguarding iseveryone’s business.

Ongo has a named manager who acts aschampion for the remit and that supports all staffin raising their concerns.

The ICT system for staff that work out on estatesand in customers’ homes has been amended tomake it easier for them to raise safeguardingconcerns quickly.

Within Ongo’s support service, our customer riskassessment / support plan template contains aquestion in relation to safeguarding. Our supportstaff are all fully trained to ask appropriatequestions that may encourage customers toanswer honestly.

Raising Awareness of Safeguarding

All staff under take mandatory safeguardingtraining as a new starter. Customer facing staffreceive refresher safeguarding training on a threeyear cycle. Ongo’s customer risk assessment /support plan template contains a question inrelation to safeguarding.

Safeguarding articles are regularly included in ourtenant magazine (Key News – which is distributedto around 10,000 homes twice a year, as well as anadditional two digital Key News editions duringthe year).

We carry out monitoring visits to care settingsacross North Lincolnshire to listen to the views andcomments of those receiving care and their friendsand relatives.

We keep adults safe by ensuring any safeguardingissues are escalated and make recommendationsto improve quality of service where this isnecessary.

national probation Service North and North East Lincolnshire LocalDelivery Unit

The role of the National Probation Service (NPS) isto protect the public, support victims and reducere-offending. It does this by:

• assessing risk and advising the courts toenable the effective sentencing andrehabilitation of all offenders;

• working in partnership with CommunityRehabilitation Companies (CRCs) and otherservice providers; and

• directly managing those offenders in thecommunity, and before their release fromcustody, who pose the highest risk of harmand who have committed the most seriouscrimes.

In carrying out its functions, the NPS is committedto protecting an adult’s right to live in safety, freefrom abuse and neglect.

The NPS has a key responsibility for safeguardingand promoting the welfare of adults at risk. Itrecognises the importance of people and otherorganisations working together to prevent andstop both the risk and the experience of abuse andneglect, whilst at the same time making sure anindividual’s well-being is being promoted with due

regard to their views, wishes, feelings and beliefs. Italso acknowledges the important contribution theNPS can make to the early identification of careand support needs for an offender in thecommunity, as well as cases where an offenderwho is a carer needs support.

The NPS is committed to eliminating all forms ofunlawful discrimination and to encouragingdiversity amongst the services it provides. Its aim isto ensure equality and fairness for all and to notdiscriminate on the grounds of gender, maritalstatus (including civil partnerships), race, disability,sexual orientation, age, gender reassignment, andreligion or belief.

During this year the National Probation Service hascontinued to work with the North East LincolnshireSafeguarding Board members via the HumbersideMulti-Agency Public Protection Arrangements(MAPPA). Together we have ensured the,proportionate and necessary risk management ofadult offenders who presents a risk of seriousharm to communities whilst supporting desistancefrom crime through a continued and sharedrehabilitative ethos. We have continued to workclosely with our colleagues from the HLNY CRC tocommission services targeted at addressingdomestic abuse in terms of the risks presented byindividual offenders and the wider societal impact.The North & North East Lincolnshire NPS LocalDelivery Unit has taken the lead this year indeveloping practitioner awareness training with

the Humberside Modern Slavery Partnership,delivering an input for probation staff across theNorth East of England”.
