Social isolation is an objective measure that refers to the number of social contacts an individual has.

Loneliness is a subjective indicator that refers to an individual’s sense of being isolated and unsupported.

Not everyone who is socially isolated will be lonely, and not everyone who feels lonely will be socially isolated, but the two are strongly correlated.

Social Isolation & Loneliness

The WS Older People Survey 2013 showed: 21% of respondents aged 65 and over reported

being moderately or severely lonely in Mid Sussex Lonely people were 60% more likely to be in poor

health than non-lonely people and 78% more likely to have reported multiple emergency admissions to hospital

Commissioning Response: Social Support Framework

• Designed with the Voluntary & Community Sector and Older People themselves to establish the outcomes that support people to stay independent at Home

Help to Live at Home services

services to support people who may be struggling with domestic chores or shopping. This service is aimed at vulnerable people who cannot manage household tasks without assistance

and have a limited income

Home from Hospital services Service to ensure that following a hospital discharge vulnerable adults recover and avoid an unplanned re-admission into hospital & maintain/improve their

health and wellbeing

Provided by: Age UK Horsham Age UK West Sussex British Red Cross (Carers)

Collect prescriptions or shopping - Accompany you to a hospital or doctors appointment - Provide information on other services

or equipment needed - Pop in for a chat - Assist with finances/paperwork - Liaise with a wide range of other services on your behalf - Help with a pet - Return library books - Small

Tackling Social Isolation The Good Neighbours Scheme has been commissioned to support vulnerable residents who are at risk of loneliness.

Provided by: Royal Voluntary Service

Accompany you to the shops - Take you to visit friends Support you to attend local clubs, groups and activities

Enable you to take up hobby Arrange for you to be delivered books and dvds

Some company and a chat with regular visits from a volunteer

Information & Advice • This includes free advice that helps people resolve their

problems with debt, benefits, employment, housing, discrimination, and other legal type issues and;

• Opportunities for vulnerable people to access information & advice who would not contact Statutory Sector bodies to get help

Prevention Assessment Team (P.A.T)

The Prevention Assessment Team is a joint team with staff from the NHS, WSCC and the Voluntary Sector

The team includes staff experienced in health, social care and the community sector

What do PAT do?

• Check a persons general state of health & wellbeing and advise how to keep healthy & manage health issues

• Give advice on how to keep a home safe, secure & warm

• Link people with agencies that can maximise income and help to retain independence

• Put someone in touch with local social groups, services and activities

P.A.T.s comprise of Health Advisors (trained Health Professionals), Social Care Staff and Voluntary Sector workers

Wellbeing Hubs

Provides a hub of information for members of the public wishing to change behaviours to improve their health

Help to remove barriers that inhibit people improving their health by providing a wrap-around service that will help them to access local health and wellbeing services

Provide or commission additional services to support behaviour change that will reduce unhealthy eating, alcohol misuse, smoking and low levels of physical activity, and help people to better manage long term conditions through lifestyle changes
