Instructions for Students to Complete the Enrollment Form

• Please follow the directions as the instructor goes through them.

• Please do not get ahead of the instructor.• It is easier to complete the enrollment form

correctly the first time.• We will be filling out the boxes at the top of the

columns first.• When we complete filling in the boxes you can

then go back and fill in the bubbles.• All entries will be left justified, starting in the

first box on the left.

1. Student Name

Print LAST name.• Enter a comma.• Skip a space.• Print FIRST name.• Skip a space.• Print MIDDLE

• Fill in other section and course numbers if you are enrolling in more than one section.

23. Today’s Date

• This date corresponds to the first day of attendance for this class.

• Bubble in the month.• Print today’s date.• Print the last two digits

of the year.

23. Course Name

• Print the course name on the line next to the corresponding number.

• Ex: Medical Core

the form there are two lines for the Course Title.– On the top line enter
