INMATE OPTICAL PURCHASE PROGRAM
CI Optical is the only approved vendor for inmates to obtain prescription eye glasses. There is no change to the Inmate Health Plan regarding state issued eye glasses. If an inmate is eligible under the Inmate Health Plan guidelines, they may obtain a pair of state issue eye glasses through that plan. State issue eye glasses are provided through medical.
Inmates interested in obtaining a personal pair of eye glasses may now purchase them through CI Optical. This is the only option for inmates as they will no longer be able to purchase prescription eye wear online nor receive them through the mail from family or friends.
This catalog shows the only frames, lens types and options available.
The inmate must obtain a copy of his/her current prescription for CI Optical use. If the inmate does not have a copy, he/she may request it from medical. Outside prescriptions (those other than DOC) are acceptable. If the prescription is expired, a new exam is recommended. Exams are obtained by sending a kite to medical. Typically exams are done once every 2-3 years. Medical does however do a prescreening to determine if an inmate will qualify for a new exam. If you do not qualify for a new exam, request a copy of your most recent prescription. We can use an older prescription if a newer one is not available but cannot make anything other than what is written in the prescription.
The inmate must have the funds to purchase the eye wear. An inmate may not incur debt for optical orders nor make partial payments. A catalog will be provided by CI Optical to each unit within all institutions. This catalog will help inmates see a selection of frames and lens options as well as some initial prices.
Note that final prices may be higher or lower depending on the inmate’s final option selections. Delivery time normally takes between four to six weeks on average from the date the order was paid for. Some specialty orders may run and additional 1-2 weeks. Eye glasses purchased are subject to the DOC Maximum Allowable Personal Property Matrix limits which currently stands at two pair of prescription eyeglasses. The two pair limit would include state issued pairs, those from CI Optical or current personal pairs.

DEFINITIONS
Photochromic lenses change color from inside to outside. They are lightly tinted inside and turn darker when exposed to sunlight. They do not get as dark as sunglasses but work well for most needs.
Polarized Photochromic lenses also change color. They have a bit more tint inside and get a bit darker outside; turn dark to reduce reflective glare from water, snow metal or glass. When dark they function like a fisherman’s eyeglasses.
Antireflective coatings are done to reduce glare from inside and night time lighting. It cuts the reflections and allows more light into the eye. It is commonly seen also on cameras, binocular and scopes. It is nice to have but is a softer coating and more easily scratched. More care is needed to keep the lenses scratch free.
U-V coating is basically sunblock for the eyes. It is a clear coating that can be added. Photochromic and thinner lenses already have it applied.
Polycarbonate lenses are the first step in going with a material that will make the lenses thinner and lighter. Typically for prescriptions in the +\- 2.00 to the +\- 4.00 range.
Mid index / thinner lenses is the second step in making the lenses thinner and lighter. Typically for prescriptions in the +\- 4.00 to the +\- 7.00 range.
High index / thinnest lenses is the third step in making the lenses thinner and lighter. Typically for prescriptions in the +\- 7.00 and over range.

PAYMENT OPTIONS
Family Pay Option:
There are no taxes or deductions taken out when this option is used.
The preferred method of payment for family members is with a credit/debit card. Optical orders will be processed once a credit/debit card payment has been made. Credit card payments must be paid by phone directly to Correctional Industries’ accounting office. Family members may call 1-866-580-9933 toll free. Family members must be prepared to provide the following information at the time you make the payment: Inmate Name and DOC Number, Facility, Order Number, Order Date and Order Amount. This information listed above will be provided to the inmate only at the time of ordering. The inmate will be given a form with the information filled out by the Optical representative. It is the responsibility of the inmate to pass the information along to their family.
As an alternative method of payment, checks or money orders are accepted. Optical orders paid by check will be processed once the check has cleared. This can delay an order up to two weeks. Check or money order payments must be mailed directly to Correctional Industries, P.O. Box 41116, Olympia, WA, 98504-1116. There is a $25.00 fee for returned checks. Checks or money orders must be accompanied with the following information: Inmate Name and DOC Number, Facility, Order Number, Order Date and Order Amount.
Inmate Banking Option:
The inmate may use their spendable account.
Savings may be used. However savings usage is dependent on facility staff approval. NOT EVERYONE IS APPROVED TO USE THEIR SAVINGS. IT IS DONE ON A CASE BY CASE BASIS. The inmate may wish to ask their counselor if they think they might be eligible to use savings. Inmates cannot submit requests to banking until they have met with CI.
Medical accounts may be used. All payment forms will be provided and are to be filled out only when meeting with the Optical representative.
ORDER PROCESS
1. The inmate forwards a request through the institutional kiosk (CI Optical Mailbox) to be put on call out for an appointment with a CI Optical representative.
For IMU or SOU units without access to a kiosk, inmates must forward a kite through their counselor to scan and email to DOC CI Inmate Optical: [email protected].
2. CI Optical will have a call out generated. The CI representative typically visits a facility once every 4-6 weeks.
3. The inmate will report to the call out and bring with themselves a copy of their eye glass prescription.
4. A CI Optical representative will assist the inmate in making the best choices for their needs in both frame and lens selection. The completed order will be held by the CI Optical representative awaiting payment approval. All payment options will be discussed and appropriate payment forms will be filled out only when meeting with the CI representative. Do not submit any payment forms prior to meeting with the representative. Signatures from inmate and CI are required.
5. Payment forms will be given to the inmate for return to their counselor for processing or to pass on to their family.
6. Upon inmate banking approval, the Business Office will notify CI Optical to place the order. The DOC CI Inmate Optical mailbox may be used for approvals.
7. When the glasses are ready, CI Optical will have a call out generated for the inmate to receive their eye wear.
8. The inmate will report to the call out and have their glasses fitted by a CI Optical representative.

WARRANTY
It is the policy of CI to provide its customers with products and services that conform to their specifications and quality requirements. CI's goal is to achieve a constant high standard of quality while maintaining economical operations.
All CI optical products are covered by a limited warranty as follows:• Lenses are warranted for 90 days against manufacturer
defect only. Lenses are not warranted against scratches or non-defective breakage.
• CI Optical frames are warranted for 90 days against manufacturer defect only.
• Reworks due to doctor errors are provided at no cost for the first rework. Dispenser and all lab errors are reworked at no cost to the inmate.
Progressive non adapts are warranted for 60 days. If an inmate is unable to adapt to the progressive style lens, CI will allow a one-time remake of the lenses into either single vision, bifocal, or trifocal lenses at no charge. There is no refund of the difference in price paid between the initial progressive order and the style of the remade lenses.
ADJUSTMENTS OR REPAIRS
Following the initial fitting at the dispensing of the eyeglasses, any further adjustment will incur a $4.00 charge
Minor repairs such as missing screws or nose pad replacement will also incur a $4.00 charge. Adjusting the frame after a minor repair is done at no additional cost.
For other issues such as broken frames or lenses not covered under warranty, full fees for replacements will need to be paid before work is started.
W A R R A N T Y & R E P L A C E M E N T
REPLACING ITEMS NOT UNDER WARRANTY
If a frame is broken, a complete new pair (frame and lenses) would need to be purchased. This is due to sizing variances in the product.
REPLACING OLD, SCRATCHED OR DAMAGED LENSES
New lenses may be ordered for CI supplied frames. The frame would need to be turned in at the time the order is placed. CI will not make lenses without the frame being present in the lab. The frame must be in good condition to use.
