EDIORIALNOTES EDITORIAL NOTES THE POST-GRADUATE JOURNAL is the official organ of the FELLOWSHIP OF MEDICINE. This introduction will suffice as an explanation of our raison d'&re, as a justification for the appearance of yet another addition to the vast accumulation of medical periodicals. But an explanation in this instance is in no sense an apology; we realise with humility, yet with confidence, that, to employ an overworked platitude, we supply a long-felt need. * * * The post-graduate student of medicine-and every qualified medical man remains in some degree a post-graduate student for the whole of his career-can hardly fail to appreciate the advantage of some system which will keep him en rapport with the details of post-graduate teaching not only in this country but elsewhere, and will supply a medium for expression of his individual requirements and, if need be, the ventilation of his grievances. We shall most cordially welcome any communications which will contribute to this end. Our correspondence columns will be open to the discussion of all that is germane to post-graduate education; and, as the mouthpiece of the Fellowship of Medicine, we shall always be most anxious to give advice and information as to lectures, demon- strations, and clinical instruction for which pro- vision has already been made in London and the provinces, whilst suggestions as to other arrange- ments will be gratefully received. * * * The Fellowship of Medicine has had to face, and still has to face, the thicket of difficulties which stand in the way of efficient post-graduate teaching in this country. He refers to the dissatisfaction which inevitably arises when undergraduates and post- graduates are taught together. The perfect system of post-graduate teaching may be long in its evolution. We do not flatter ourselves that the introduction of this Journal will achieve its destiny at a bound. But, vogue la galere ! post-graduate teaching has made at least one more step, and that unquestionably in the right direction. * * * And now we turn to consider the other aspect of our activities. We intend to publish a selection of the lectures which are delivered at the instance of the Fellowship of Medicine. These wiU be supplemented by specially contributed articles, by suitable clinical descriptions, and similar material. It will be our endeavour to select only such subjects as will from their essentially practical character make the strongest appeal to the general practitioner for whom the resources of post-graduate teaching are particularly indicated. It cannot be maintained that the spoken word can ever be superseded as a most important means of education; the living interest, the personality of the lecturer connote attributes that are irreplaceable. But circumstances are such that whilst only a com- paratively small proportion of would-be students are fortunate enough to enjoy this special opportunity of hearing an actual lecture, an unlimited number will be enabled to secure the next best opportunity and to read it at their leisure. We repeat that to this end we shall be influenced in our selection, not so much by the academic brilliance of certain treatises as by essentially practical considerations. * * * As is appropriate, our first number contains an address by the Chairman of the Executive Committee of the Fellowship of Medicine and Post-Graduate Medical Association, Sir HuMPHRY RoILESTON, President of the Royal College of Physicians and Regius Professor of Physic of the University of Cambridge. His masterly essay will be read with the greatest pleasure, not only for the scholarly style which characterises all his writings, but for the valuable therapeutic hints which he has included. critically examined the outstanding hypotheses,the influence of infection and the metabolic factor,both of which he accepts as setiological. The mostcommon antecedent of cholecystitis is an infectionof the alimentary canal, particularly with B. coli.Enteric fever has long been recognised as an ante-cedent; and whilst the relation between chole-cystitis and appendicitis is frequently remembered,the tendency to suspect and deal with the appendixduring an operation on the gall-bladder is moregeneral than the converse procedure of scrutinisingthe gall-bladder during an operation for a frankappendicectomy. On the metabolic side, an alterna-tive hypothesis depends upon the recognition ofan increased cholesterol content of the blood, and,as Sir HuIJMPRY ROLLESTON points out, thereappears to exist a gall-stone diathesis with aninherited predisposition or a racial incidence. Onthese lines prophylaxis must be in the direction ofavoiding foodstuffs rich in cholesterol, of whichSir H1IJMPHRY ROLLESTON provides an elaboratelist.

One feature relating to gall-stones calls forparticular consideration, the high incidence inwomen, and particularly women who have bornechildren. Such a circumstance lends support toeither hypothesis already cited: the possibilityof puerperal sepsis, and the influence of the endo-crine system, which would naturally play a partfrom its relationship both to the acute specificfevers and to pregnancy. Furthermore, SirHumRimy ROLLESTON points out as a possiblecause for the rise in the incidence of gall-stones,the more modern tendency to avoid breast-feeding;since, according to Aschoff, the elimination of fatsin the milk of parturient women who suckle theirinfants imparts a relative immunity to the sub-sequent development of gall-stones.

Sir HuIMPHRY ROLLESTON, in his concludingparagraphs, displays a judicial attitude to themedical treatment of gall-stones. Bile-salts heregards as the only true cholagogue that may bemedicinally employed. The much-vaunted oliveoil may, he admits, dissolve a gall-stone in a test-tube, but can its administration by the mouthensure its reaching the human test-tube, the gall-bladder ? The value of olive oil he believes to besymptomatic only, the relief of pylorospasm whichso often occurs as a reflex manifestation of gall-bladder irritation.

The subject of puerperal sepsis claims an impor-tance upon which no emphatic insistence isdemanded. In Dr. EDEN'S hands it becomes atthe same time a fascinating study. The chartswhich accompany his lecture will repay the closestinspection. As Dr. EDEN suggests, the causes ofpuerperal sepsis are by no means obvious having

regard to its appearance in everv branch of mid-wifery practice. It is encountered alike in thehouses of the well-to-do and of the poor, in thecountry as well as in the towns. And even thelarge training hospitals and the recently establishedsmall municipal maternity homes are not immune.

But statistical observation as embodied in thesecharts supplies certain conclusions from whichinvaluable prophylactic hints may be deduced.Analysis shows, for example, a far higher mortalityfrom puerperal sepsis in the winter months with aremarkable regularity reproduced in similarobserva-tions in other countries. An alarming increasein 1919 and 1920 is shown to be exactly correlatedwith an increase in demobilisation at correspondingperiods, and displays most graphically the influenceof urethral infections. The important conclusion isreached that we must accept as a basis the thesisthat severe puerperal sepsis is an exogenous infec-tion,. and the elaboration of this thesis is the burdenof Dr. EDEN's addiess.

Dr. EDEN advances reasons for the failure ofefficient notification, and this failure he regards asa serious menace. Not only is prompt and earlytreatment prevented, but the absence of isolationleaves a centre from which infection may becarried to other lying-in women.

We see that Dr. EDEN'S address is an indictmentof evils, but for these evils he has remedies toadvise. And we venture to congratulate our-selves equally with the gifted author on the con-viction that its publication will do much to embodythe remedies he has so clearly explained, andmarkedly to reduce the mortality of a diseasewhich of all preventable diseases should surelybe the first to be prevented.

Some two months ago it was publicly announcedthat the Minister of Health had appointed a Com-mittee, with the following terms of reference: " Todraw up a practicable scheme of post-graduatemedical education centred in London." attached to a hospital centrally situated in Londonshould be devoted solely to post-graduate medicaleducation. This, it was felt, should be a school ofthe University of London, but, the British MedicalJournal points out, the University of London, thebody which primarily ought to be interested, has,in spite of good intentions unofficially expressed,so far, done nothing.

Our contemporary recalls the meeting summonedlast March by the Fellowship of Medicine, an openmeeting at which free expression of opinion onarrangements for post-graduate teaching wasinvited. At that time representatives of theServices attended and expressed their satisfactionwith the arrangements made for their medicalofficers. But the British Medical Journal is rightto point out that the conditions in the Services arespecial and afford little guidance for the solutionof the larger problem presented by the civilianprofession. * * *

"Every credit is due to the Fellowship of Medicinefor its persistent efforts to supply the need," saysthe British Medical Journal, and then proceeds torefer to Sir Thomas Horder's expression of regretat the meeting to which we have referred that theauthorities of the majority of undergraduate schoolsshould havewithdrawn their support or, at anyrate,their active interest when post-graduate teachingis in question. This is a detail to which we havealready referred; it is perhaps the most importantdetail which will exercise the minds of the membersof the new Committee. Once more to quote ourcontemporary: "One of the first points to be decidedwill be whether to seek to adapt to the purpose ofpost-graduate medical education an existing school,or whether to follow the example of the LondonSchool of Hygiene and Tropical Medicine and strikeout for an entirely new institution."

The new Committee assembles in October, so thatwe may hope for some information of its activitiesto communicate in our next number. And we desireonce more to offer an invitation for the discussionof this and similar topics in our correspondencecolumns. One thing is certain, it is abundantlyevident, not only that the importance of post-graduate instruction is sufficiently recognised, butthat support will notI be lacking for the further-ance of any enterprise which can adequatelyestablish its claims.

MEDICAL ASPECTS OFGALL-STONES.*

THs subject is so large that it is necessarv toselect some aspects only for consideration, and itwill perhaps be most useful first to refer to thecausation of cholelithiasis and then, as it is inpart dependent on it, to the treatment. After thissome of the clinical pictures, for they are many,will be briefly sketched.

In the first place it should be mentioned that theclinical label gall-stones or cholelithiasis is often andnaturally used for symptoms common to them andto inflammation of the gall-bladder-conditions verycommonly combined but also occurring indepen-dently. Although a distinction is drawn betweenthem in systematic treatises, and of course exists,they are very closely bound up together, beingoften successive or altemating stages in the patient'shistory and giving rise to very similar symptoms.Unless a calculus is indicated by X ray examinationor is passed in the stools, a diagnosis of gall-stones,even after an acute attack of biliary colic, is opento doubt; and yet it is more often made than theless committal one of cholecystitis, which in theoriginal sense means merely disease of the gall-bladder. It is, of course, true that the termination"-itis " now has a universal significance of inflam-mation, but it was not always so. Originally theaffix "-itis" was a Greek feminine adjectivaltermination, and in words like neuritis, peritonitis,pleuritis, agrees with vo6ao (disease) understood, sothat v upt)pL't vo6ao simply meant the disease ofthe nerves, and the implication that the diseaseof organs was inflammatory came later. In a some-what similar manner melana signifies the blackdisease, and pneumonia the disease of the lungs.

There are two distinct views about the formationof gall-stones

(1) The infective or inflammatory theory enun-ciated by Naunyn to the effect that a local inflam-mation of the gall-bladder walls leads to an increasedformation of cholesterol by the epithelium liningthe gall-bladder, and that the liberated cholesterolis cemented together by bilirubin calcium. Accord-ing to this view cholecystitis is a necessary ante-cedent to the formation of calculi, but it has beenurged that cholelithiasis does not invariably followcholecystitis, and therefore that some additional
