NotificationThe Empanelment period of the following Health Care Organizations was extended up to

31.01.2015 vide this department's Notification No-4979-F(MED),Dated-27.1 0.20 14.However,their renewalprocess could not be completed in time due to non-submission of up-to-date Clinical Establishment License,Fire Safety License, Memorandum of Agreement(MOA) and other requisite papers.

Accordingly, the governor is pleased to extend the empanelment period of the followingHealth Care Organizations up to 31.03.2015 on existing terms and conditions read with notificationnos.4803-F(MED), dated: 12-09-2014 and 5069-F(MED), dated:26-09-2014 so that the beneficiaries underthe West Bengal Health Scheme may get services there uninterruptedly.
