Question: “When a woman of about fifty years of age suffers
from vaginal bleeding for several tens of days without stopping,
with heat effusion at dusk, lesser abdominal urgency, abdominal
fullness, vexing heat in the palms of the hands, and dry lips and
mouth, what is this?”
The Master says: “This disease is ascribed to Dai Xia 带下, How is this caused? The woman must have previously experienced a
miscarriage, as a result of which static blood is present in the lesser abdomen instead of having been eliminated. How can you know
this? You know this by the sign of dry lips and mouth. Use Wen
Jing Tang to govern it.”

Body pain is a common symptom of Tai Yang illness. Generally after there has been sweating the illness is resolved as the sweat issues and the body pain is also resolved. If the body pain is not resolved after sweating then it was not caused by an exterior pattern and the sweating was too extreme and so the Ying was damaged. This causes the sinews and vessels to loose their nourishment. This is distinguished by the hesitant pulse. The Da Lun says, “When the pulse is floating and tight, the method should address the body pain and be one of inducing sweat. When the cun is hesitant sweating should not be induced. Why is this? When the Ying Qi is deficient this makes the blood scanty.” In this case, the treatment is Gui Zhi Tang as the foundation as it will harmonize the Ying and Wei. Bai Shao is increased because it will nourish the Ying and blood. Ren Shen is added as it with supplement the deficiency of Wei. Sheng Jiang is also ingeniously increased to take advantage of the pungent scattering to strengthen the movement to the exterior. Altogether this formula has the function to boost the Qi and nourish the blood and move this to the exterior of the body. When the exterior is supplemented, there will be no stagnation. Generally, when there is sweating, for example after giving birth, after a miscarriage or after menstruation, the Ying and Wei will be insufficient and there will be body pain.

The Master says “If a woman experiences spotting, in some cases it is caused by continued and incessant vaginal bleeding after late miscarriage; in some cases it is vaginal bleeding in pregnancy. If it causes pain in the abdomen during pregnancy, this means uterine obstruction, Jiao Ai Tang governs.
Decoct the six ingredients in 5 sheng of water with 3 sheng of clear wine until 3 sheng remains. Discard the dregs and add the E Jiao.

The Chong is the sea of blood and the Ren governs the fetal wrapper. When the Chong and Ren are deficient or damaged, they are unable to contain and absorb the blood vessels. The Yin and blood are guarded. This leads to dripping or heavy passing of blood water with continuous soaking through. There may be a late miscarriage with bleeding that does not stop. The pregnant woman has bleeding below with abdominal pain. This is called uterine obstruction. Due to deficiency and damage of the Chong and Ren, the Yin and blood drip downward and so are unable to enter the uterine vessels to nourish the fetus. This hinders the normal development of the fetus. This is uterine obstruction as described in the Yi Zong Jin Jian: “When there is uterine obstruction, the Qi and blood within the uterus is not harmonized and so the transformation and development is hindered.” The Mai Jing says: “Uterine dripping” means the same thing. E Jiao is used to nourish blood and stop bleeding.” The Ben Jing “When there is bleeding in women, calm the fetus.” The Bie Lu says: “when pregnant women drip blood, one must both regulate the channels and calm the fetus. One will want to use medicinals that treat flooding and stop dripping as the chief herbs. [E Jiao and Ai Ye]. Gan Di Huang, Dang Gui, Shao Yao and Chuan Xiong nourish the blood and regulate the channels. They transform stasis and generate the new, guarding against the blood stasis taking up residence. These are the assistant herbs. Blood is not generated spontaneously. It is generated from the Yang Ming sea of water and grains. Gan Cao supplements earth in the service of nourishing blood. In addition, it is able to harmonize the other herbs in the formula. When Gan Cao is combined with E Jiao, it is able to stop bleeding. When it is combined with Shao Yao, the sour and sweet generate Yin, moderate spasms and stop pain. Adding the clear alcohol stimulates the herbs to enter the blood vessels so that the stasis of blood does not reside. This is an envoy herb. This deals simultaneously with the root and branch. Staunching, flowing and settling the source are all achieved so that the nourishing blood regulates the channels, the fetus is calmed and the dripping is stopped.

There are three clauses relating to Xiǎo Bàn Xià Tāng in the Jin Gui Yao Lue. Two of the clauses relate to vomiting. Generally speaking, vomiting is due to rebellious stomach Qi. The stomach follows a downward path, receiving and downbearing. When the stomach is without harmony or downbearing, the Qi rebels upward. This is why vomiting arises. This vomiting is the upward rebellion of the stomach Qi and is due to propping fluids in the epigastric area. When water and fluids stop in the epigastric area, the stomach is restricted and is unable to receive food. As Zhong Jing said, “When there is vomiting, grains cannot go downward.” These words describe this pattern. This refers to vomiting and hiccups as well. This can also be due to stomach Qi deficiency or phlegm blockage. Zhong Jing said, “When there is vomiting, Xiǎo Bàn Xià Tāng is the principle formula. It can move phlegm, downbear rebellion and harmonize the stomach Qi.Let us consider this formula: Shēng Bàn Xià is in an important position. It is pungent, warm and without toxin. It transforms phlegm, dispels fluids, downbears rebellion, stops vomiting and harmonizes the stomach. It is the chief herb. Shēng Jiāng assists the chief herb in dispelling phlegm and down bearing rebellion. It is also able to control an intense toxin. It is the assistant herb. These two herbs cooperate with each other and strengthen the effect of downbearing rebellion, stopping vomiting and stopping cough. This is a well-organized ancient and simple formula that is also very effective. When I use this formula I generally use prepared Bàn Xià. If the vomiting is severe, one can also use 4.5 – 6 gm of Shēng Bàn Xià cooked with 9 gm of Shēng Jiāng. This will ameliorate any worry of side effects. In addition, this formula is the foundational stop vomiting formula. It is in Zhong Jing’s formulas such as Bàn Xià Xiè Xīn Tāng, Xiǎo Chaí Hú Tāng and Huáng Qín Jiā Bàn Xià Shēng Jiāng Tāng. Later generation formulas include this combination as well such as Wēn Dàn Tāng and Èr Chén Tāng. These formulas all use Bàn Xià and Shēng Jiāng. It is just as Yin Li-Wen said in his classic医宗金鉴Golden
Mirror of the Medical Tradition says “The warmth of Bàn Xià and Shēng Jiāng are able to harmonize the stomach Qi. Their pungent nature is able to disperse rebellious Qi. It is the efficacious herb combination for habitual vomiting.” However, when all is said and done, this formula tends toward being warm and drying and so it is used for vomiting without thirst and with a white sticky tongue moss and a tendency to cold fluids. For this it is suitable.

This area is crowded with micro-
circulation vessels such as small intestine,
mesentary and caul. Thus Saibo Atsu
suggests the existence of a morbid state of
blood occlusion due to deterioration of
micro-circulation such as hemostasis or
hyperemia. Even though, as caul has a
limited number of pain receptors, pain
could be from the small intestine or
mesentery due to a morbid state of blood
occlusion.

• Switching out Gan Jiang for Sheng Jiang and adding Huang Lian
and Huang Qin to integrate the Ban Xia Xie Xin Tang pattern.
• Same as above but increasing the Gan Cao to integrate the Gan Cao
Xie Xin Tang pattern when there are mucous membrane dryness and
inflammation issues.
• Adding Fu Ling, Ze Xie and Bai Zhu to integrate Dang Gui Shao Yao
San when there are water signs showing that the water that should be
in the blood is outside of the vessels.
• Case 1: Chronic Staph InfectionAli came to me because she had suffered from recurrent staph infections over a
period of 1 year. This manifested as large boils on various parts of her body
including her lower legs, thighs, butt, arms and back. She had already undergone
5 courses of antibiotics and was at her wit’s end. At the time of our first visit she
had a large boil on her left thigh. The boil was red without any sign of
immanent rupture.
Ali, age 22, was very thin and pale. Her lips appeared red and dry. She felt cold
all the time and sweat very minimally. Her menstruation was scanty and often
late. She suffered from severe dysmenorrhea each month, wanting to stay warm,
use hot water bottles and take hot baths. Her appetite was poor and she was
someone constipated. Her spirit was depressed and anxious.
Palpation: Pulses were all thin and weak. Skin was very dry. Her abdomen was
dry with tight rectus muscles and a chopstick. There was tension around her
navel and pulsations on the CV line above her navel.
Tongue: Her tongue was pale and purplish

• Case 1: Chronic Staph InfectionAfter 2 weeks on Wen Jing Tang (raw herbs) the current boil resolved with no
trace.
After 3 weeks Ali’s menstruation came and there was 70% less pain and a richer,
longer flow. Ali also reported feeling happier and warmer in general and her
appetite had improved.
We continued Wen Jing Tang for 2 months before concluding treatment.
A follow-up occurred when I ran into Ali a year or so later. There had been no
recurrence of boils.
• Case 2: Recurrent MiscarriagesWendi had suffered from 3 very early miscarriages. She was diagnosed with elevated
homocysteine levels and her doctors were recommending she begin taking aspirin. She
decided to pursue Chinese medicine first.
Wendi had a dusky complexion with freckles. She worked in the fish department of a
local grocery store so she was in and out of the refrigerated area all day, almost every
day. She complained of being cold and having to wear layers upon layers at work. She
suffered from intense dysmenorrhea that craved warmth and desired cold. Her tongue
was pale and purplish with depressive heat spots. Otherwise Wendi was in good health
though she suffered from bouts of insomnia and depression. She was very upset about
the miscarriages, fearing she would never be able to have a child. Her pulses were rough,
thin and wiry.
We decided to work for several months until she felt warmer and no longer had
dysmenorrhea. After 2 months, her menstruation came with no pain. Wendi felt
warmer and her cheeks got rosier. After the 3rd month I believed that the blood
circulation in her womb was open, flowing and warm. Wendi became pregnant right
away and had a successful full term son.

• Case 3: Dark DepressionFiona was 38 when she came to see me. After a break up she had suffered a profound
and scary depression. Though she was not suicidal, she felt haunted by dark and
hateful thoughts toward herself. She was in therapy but nothing touched the depression
so she came to try Chinese medicine.
Fiona’s complexion was pale and lusterless. She had dark circles under her eyes and a
haunted look. She felt generally very cold and when I asked her if the negative
thoughts felt cold, she said yes, that they were “cold like death.” She described her
break up: Her husband, who is the father of her two kids, had been an accommodating
person and was obliging when she fell in love with a woman. Suddenly, when she
moved out to live with her new partner, he turned on her with hateful accusations and
then he completely cut her off emotionally. In other words, in my mind, she received
an attack from cold deep into her heart. Her body was vulnerable because the fire was
already low.
Fiona’s tongue was pale and purplish with a red tip. Her sleep was dream disturbed,
her menses were somewhat though not severely painful and she felt cold all the time
but worse during menstruation. Her abdomen was flacid and very empty below her
umbilicus.
• Case 3: Dark Depression
I treated Fiona with Wen Jing Tang with Gan Jiang
instead of Sheng Jiang and added Fu Zi.
After a 3 month course of treatment, Fiona felt fully
like herself again. Her menses were painless and she
felt generally warmer in spite of the weather turning
bitter cold.
