  • SPECIAL NOTE FOR LOCATION OF PERMITTED EXCESS FILL MATERIAL SITES

    PERRY COUNTY ROCK FALL MITIGATION PROJECT KY7

    ITEM NO. 10-5005.00

    Please see the included excess fill material site map, this map depicts 3 different sites, two of which are flood plains they are labeled Site #2 and #3, the Division of Water flood plain permit has been included in the proposal which describes the limits and capacities for Site #2. The KYTC was unable to secure a Division of Water flood plain permit for Site #3, if the contractor chooses to utilize Site #3 is shall be the contractors responsibility to obtain all applicable permits from regulatory agencies. The KYTC did not acquire any Right of Way at either of the two flood plain sites. The site labeled McIntyre Property Site #2 is an excess fill site that shall be used, this was a commitment made to the property owners during Right of Way Negotiations. The third site is a hollow excess fill material site is labeled Site #1; the KYTC has acquired a temporary easement at this site. Please see plan sheet 7A for the details and boundary description of the temporary easement for the Hollow Excess Fill Material Site. This site is located approx. 1.25 miles to the North of the Project. Copies of the Section 404 & 401 permits are included in the project’s specimen proposal. It is the responsibility of the contractor to review the 404 & 401 permits and maintain compliance with the 404 & 401 permits throughout the duration of the project. Note: If the contractor chooses to use other excess material site(s) (rather than or in addition to) the KYTC’s designated excess material sites #1 and #2, it will be the responsibility of the contractor to acquire. Any fees associated with obtaining approval from the U.S. Army Corps of Engineers or other appropriate regulatory agencies for additional waste and borrow sites are the responsibility of the contractor.

    SPECIAL NOTE FOR BLASTING OPERATIONS

    Traffic should be halted for no more than 20 minutes in an hour for blasting operations during the times of day specified. If traffic is blocked for more than this allowable time, a disincentive fee of $200 per hour will be charged for the first hour or fraction there of past the 20 minutes allowed. If the roadway is blocked for more than one hour, contractor will be charged disincentive fees at a higher rate. This disincentive fee for the second hour and any successive hours will be $400 per hour or fraction of an hour. During blasting operations, traffic may be halted for a maximum of 20 minutes per hour to allow execution of the blast and allow for removal of the rock fragments and debris. Traffic stoppage will not be allowed between the hours of 6:30 A.M. and 8:30 A.M. or 3:00 P.M. and 5:00 P.M. to allow for school traffic.

    SPECIAL NOTE FOR HAULING ACROSS BRIDGE NO. B00042N WHICH WILL BE

    UTILIZED TO ACCESS EXCESS FILL MATERIAL SITE

    The bridge over the North Fork Ky River 097B00042N which will be used to access the excess

    fill material site has been analyzed for the maximum loading which is equivalent to or less than a

    loaded Volvo 40E vehicle. This load is approved to cross the bridge with the following

    restrictions:

    1) The vehicle must approach and cross the bridge at a slow steady speed no greater than 30 mph (no braking on the bridge). 2) The vehicle must be the only vehicle on the bridge at the time. 3) The vehicle must drive down the middle of the bridge.
