Welcome to the Medicaid Health Plans of America’s 2009 Annual Meeting! This year’s meeting — focused on innovations in Medicaid in this era of change and health reform — features many nationally recognized speakers who will share with us their expertise in health care and politics.

This year’s conference sets the tone for the new frontier in Medicaid managed care. We are proud to present such nationally well-known speakers, including John Gorman, President and CEO of the Gorman Health Group, and traditionally one of our most popular speakers annually; David Plouffe, the campaign manager for President Barack Obama’s 2008 presidential campaign, and in the President’s own words, the “unsung hero” of the campaign; Mark Shields, a nationally known columnist and commentator; nationally-known Medicaid expert Vernon Smith, a principal of Health Management Associates; former HHS Secretary and Governor Tommy Thompson; and former U.S. Congresswoman Heather Wilson, who served as a Congressional leader on Medicaid issues.

SPEAKER BIOS

John K. GormanJohn Gorman is CEO of Gorman Health Group, a company he founded in 1996 (under its predecessor name, Managed Care Compliance Solutions) to provide Medicare regulatory compliance advisory services to health care payers. Under John’s leadership, Gorman Health Group has since emerged to become the leading consulting firm

in Medicare managed care, providing thought leadership along with expert strategic, operational, financial and other professional services to the health care industry, including an array of leading-edge business process outsourcing solutions. Mr. Gorman and his team of more than 65 senior-level consultants are considered by many in the industry to be the go-to source for understanding the regulatory and business implications of the Medicare Modernization Act and the introduction of the Medicare Part D prescription drug program. John speaks at dozens of healthcare industry conferences each year, reaching thousands of senior healthcare executives and professionals nationally. In 2000, John received Best Speaker Award for the National Managed Health Care Congress. John is regularly quoted in the trade press and the national media, and serves on the editorial advisory boards of several industry publications. He is also frequently sought out by the investment community for his analyses and predictions about the evolving Medicare managed care market.

david plouffeDavid Plouffe was the campaign manager for Barack Obama’s 2008 presidential campaign. After winning the election on Nov. 4, President-elect Obama credited Plouffe in his acceptance speech, calling him “the unsung hero of this campaign, who built the… best political campaign in the history of the United States of America.” Plouffe began his political

career when he went to work for Senator Tom Harkin’s 1990 re-election campaign. From 1997 to 1998, Plouffe served as Democratic leader Dick Gephardt’s deputy chief of staff. From 1999 to 2000, as executive director of the Democratic Congressional Campaign Committee, Plouffe led a national campaign that led the Democrats closer to the majority. Beginning in 2003, Plouffe and David Axelrod worked on Barack Obama’s 2004 Illinois Senate campaign, beginning Plouffe’s association with Obama. Plouffe also worked with Axelrod on Deval Patrick’s successful 2006 campaign for Governor of Massachusetts.

mark shieldsMark Shields, a nationally known columnist and commentator, has worked in Washington through the administrations of nine U.S. Presidents. He was an editorial writer for The Washington Post where he began writing his column in 1979. Since 1988, Shields has provided weekly political analysis and commentary on national campaigns

for PBS’ award-winning The NewsHour with Jim Lehrer where he has matched wits with David Gergen, The Wall Street Journal’s Paul Gigot and most recently with David Brooks of The New York Times. For 17 years, Shields was moderator and panelist on CNN’s Capital Gang. He now is a regular panelist on Inside Washington, the weekly public affairs show which is seen on both ABC and PBS.

vernon K. smith, phdVernon Smith is a Principal with Health Management Associates, where he focuses on Medicaid, Medicare, SCHIP, state budgets and trends in the health care market place. He has authored several reports on enrollment, spending, policy trends in Medicaid and SCHIP, the Medicare prescription drug benefit, and on

state directions to address the uninsured. Dr. Smith has spoken on these issues before many national and state audiences, including the National Governors Association, the National Conference of State Legislatures, the Council of State Governments, the National Association of State Medicaid Directors, American Medical Association, American Hospital Association, the National Health Policy Forum, committees of the U.S. Congress, and Medicaid reform groups in several states. He has been a guest on National Public Radio and quoted on these issues in The New York Times, The Washington Post, The Wall Street Journal, USA Today, Newsweek and Time magazine. Before joining HMA, Dr. Smith served as Michigan Medicaid director and as budget director for the human services agency during 30 years of public service. He holds a doctorate degree in Economics from Michigan State University.

Tommy G. ThompsonTommy G. Thompson, the former Health and Human Services (HHS) secretary and four-term governor of Wisconsin, is National Policy Advisor for U.S. Preventive Medicine. Gov. Thompson’s state government career began in 1966, when he was elected to the Wisconsin State Assembly at the age of 24. He was first elected governor of

Wisconsin in 1986, and was re-elected in 1990 and 1994. Thompson was confirmed by the U.S. Senate as the 19th HHS Secretary in 2001 and served until 2005. As the nation’s top advocate for the health and welfare of all Americans, Secretary Thompson led the department, which employed more than 60,000 personnel and had a fiscal year 2004 budget in excess of $500 billion. Thompson was a candidate for the 2008 U.S. Presidential Election.

heather WilsonFormer Congresswoman Heather Wilson (R-New Mexico) represented her Albuquerque-based district in the U.S. House of Representatives from 1998-2008. A senior member of the House Energy and Commerce Committee Subcommittee on Health, she led efforts in Congress to improve the Medicaid program, establish the Medicaid

Commission, and reauthorize the State Children’s Health Insurance Program. Before being elected to Congress, Wilson was Cabinet Secretary of New Mexico’s Children, Youth and Families Department where she managed the state’s foster care system and treatment for mentally ill children. This position led her to take an intense interest in Medicaid and the ways in which the system can be improved to ensure the health of children and low-income vulnerable populations.

ruth atkins As Executive Director for Passport Health Plan and Passport Advantage, Ruth Atkins is responsible for oversight and execution of all health plan operations for the Medicaid and Medicare Advantage lines of business. Passport Health Plan has been ranked in the top 25 Medicaid ‘America’s Best Health Plans’ by US News and World Report and the National Committee for Quality Assurance. In 2008, Passport was ranked #9 in America’s Best Health Plans 2008-09’ of all Medicaid Health Plans in America. Ms. Atkins has been with Passport Health Plan since 1999 and has held various positions including Chief Operating Officer. Prior to joining Passport Health Plan, Ms. Atkins was Manager of Health Care Programs for General Electric Appliances in Louisville, Kentucky, where she was responsible for the management and delivery of health care programs for employees in the Appliance Division. She also served in key positions with Norton Healthcare and Aetna Health Plans. Ms. Atkins has over 25 years of progressive health care experience.

Clifford e. barnesClifford Barnes is member of the law firm, Epstein, Becker and Green in its Health Care and Life Sciences Practice. Mr. Barnes represents providers, including hospitals, hospital systems, ancillary service companies, managed care companies, specializing in Medicare and Medicaid managed care companies, non-profit associations and contract research organizations. He counsels health care providers on issues related to mergers, acquisitions, joint ventures, various forms of contracting, including risk arrangements, international transactions, antitrust issues, fraud and abuse, compliance programs, and federal and state regulations; serves as general and special counsel for managed care companies, including Medicare and Medicaid health plans; as well as serves as general and special counsel for non-profit associations, including the Medicaid Health Plans of America, and the National Medical Association, among others. Prior to entering law school, Mr. Barnes worked as the Executive Assistant to the Deputy Commissioner of the New York City Health Department and as the Assistant Director for Project Review at the Health System Agency of New York City.

bassem a. bejjani, md, faCmGDr. Bejjani is Chief Medical Officer and co-Founder of Signature Genomic Laboratories, LLC. He is also an Adjunct Professor in the Department of Ophthalmology at the University of Washington in Seattle. He is Director of the Genetic Fellowship Training Program in Clinical Molecular Genetics and Clinical Cytogenetics at Sacred Heart Medical Center Spokane. Dr. Bejjani obtained his medical degree from the American University of Beirut, in Beirut, Lebanon. He completed his fellowship training in Molecular Genetics and Cytogenetics at Boston University School of Medicine. He trained in Pediatrics at the University of North Carolina Hospitals-Chapel Hill, and in Medical Genetics at Baylor College of Medicine in Houston. He is board certified in Medical Genetics, Clinical Molecular Genetics, Clinical Cytogenetics and Pediatrics.

meg boothMeg Booth is the Deputy Executive Director of CDHP, where she manages policy efforts to improve the health and health care systems for children. Prior to joining CDHP, Ms. Booth served as a Policy Analyst for children with special health care needs and early childhood issues at the Association of Maternal and Child Health Programs. Her background includes working to educate policy makers on child health issues at the local, state and national levels. Ms. Booth worked with local health department in North Carolina to assess and evaluate their strategies to enroll children into public health care coverage. Additionally, she also worked in the Family Services Bureau at the Iowa Department of Public Health developing the state Title V Block Grant application and outreach strategies for enrolling Iowa’s children into Medicaid and SCHIP coverage. Additionally, Ms. Booth was a Policy Associate with the Alliance for Health Reform and a private consultant at the Center on Budget and Policy Priorities. She holds a bachelor of arts in Community Health Education with a minor in Women’s Studies from the University of Northern Iowa, as well as obtained a Master’s in Public Health from the Department of Maternal and Child Health at the University of North Carolina at Chapel Hill.

Cynthia burghardCynthia Burghard, Senior Vice President of Market Intelligence, spearheads ikaSystem’s market strategy and is a renowned expert in the health insurance industry. She has spent over 30 years in the health care information systems market, holding market research, consulting and analytic product development roles for health care payers, employers, providers and regulators. Before joining ikaSystems, Ms. Burghard was a research director for 14 years at Gartner, a leading IT research and advisory services firm, where she authored over 100 publications on topics such as care management, consumer engagement, magnetic

card and underwriting automation technologies. She was also a consultant and market researcher for Charles J. Singer & Co. and held management positions at Bay State Health Care, Blue Cross Blue Shield of Massachusetts and the Massachusetts Health Data Consortium. She holds a bachelor’s degree from the University of Colorado.

Karen L. Clark Karen Clark is the President and Chief Operating Officer of Horizon NJ Health, New Jersey’s largest managed health care company serving the publicly insured. At Horizon NJ Health and throughout her career, Ms. Clark has maintained her focus for ensuring that accessible, affordable quality health care is made available for all, especially the uninsured and at risk population. She is a leader and a businessperson who understands the role of public and private partnerships which address systematic health care issues. As an advocate, she understands the value in working with community groups as important partners in change. Ms. Clark has concentrated on developing “Best Practices” in managed care to impact outcomes, access and quality. She explores ways to effectively utilize funding to ensure access for the publicly insured population in New Jersey. Her participation with local, state and national organizations includes service with faith-based, school and community groups. As a volunteer, she has a focus on improving the health behaviors for young people.

Carrie Cunningham Carrie Cunningham has more than 22 years of leadership experience in Managed Care. As Vice President of Business Development, she leads HMS’s Managed Care Unit’s national agenda to bring HMS services to the Medicaid Managed Care and Medicare Advantage Health plans. She also serves as General Manager and Vice President of HMS’s Software Services Division, which provides proprietary software for public health clinics. Ms. Cunningham joined HMS in 1998, after previous success with Blue Shield of California and the Blue Cross Blue Shield National Association.

Karen m. dale, rN, msN Karen M. Dale is Executive Vice President of Operations and Strategy at DC Healthcare Systems, Inc. (DCHSI) and is responsible for the strategic direction and expansion efforts for DCHSI, the owner, of DC Chartered Health Plan, Inc., the oldest minority-owned and operated Medicaid Managed Care Organization (MCO) in the District of Columbia. Currently serving over 80,000 members, Chartered is the largest MCO in Washington, DC. Ms. Dale brings to DCHSI over 25 years of experience, including broad and comprehensive expertise in health care. She served as the CEO for Greater Southeast Community Hospital,

Hadley Memorial Hospital and Skilled Nursing Facility and DC General Ambulatory and Emergency Care Centers. In addition, she previously served as Chartered’s COO for the DC HealthCare Alliance Program. She was a senior staff member at the DC Department of Health, where she managed public health programs in the areas of diabetes, addiction, prevention and recovery, asthma, immunization, maternal and child health services and external affairs. Mr. Do Canto most recently served as chief executive officer of the AIDS Alliance for Children Youth and Families, a leading national, non-profit advocacy organization focused on improving access to HIV/AIDS prevention, care and treatment for under-served communities across the United States. Mr. Do Canto is largely credited with significantly strengthening the Alliance’s operational and policy structure and considerably expanding and fortifying its relations with public and private sector partners. Prior to the Alliance, Mr. Do Canto served as the director of federal affairs for the National Association of Community Health Centers (NACHC).

robert hallRobert Hall is currently an Assistant Director in the Washington, DC office of the American Academy of Pediatrics. His work is focused on federal advocacy related to children’s health, specifically in the areas of access to care; health care financing, quality and information technology; medical liability; and pediatric practice. He chairs the Children’s Health Group. Previously, Mr. Hall served as Legislative Counsel to Senator Mark Dayton (D-MN). As chief legal advisor for the senior senator from Minnesota, Mr. Hall managed the health care, labor, education, immigration, judiciary, pensions, and social security policy portfolio and staffed the Senator on the Governmental Affairs Committee (since renamed the Homeland Security and Government Affairs Committee). He also managed Senator Dayton’s signature campaign issue, advocating for prescription drugs coverage under Medicare. Mr. Hall holds a Juris Doctorate from the University of Texas School of Law in Austin, Texas and a Masters in Public Affairs from the Lyndon B. Johnson School of Public Affairs, where he was managing editor of the LBJ Journal of Public Affairs. He also earned a Bachelor of Arts in political science from Haverford College.

Kevin r. hayden Kevin R. Hayden is President of WellPoint’s State Sponsored Business. Appointed to this role in April 2008, Mr. Hayden’s responsibilities include the development, implementation, management and expansion of health coverage for the underserved through government programs such as Medicaid, State Children’s Health Insurance Programs, and other publicly funded programs for the uninsured and underserved populations. arthur r. henderson, Jr.Arthur Henderson has years of experience in government, private sector and university environments in the design, development and marketing of technology-based products and services. He currently is CEO of Affinity Networks, Inc., a systems integration firm delivering products and services to the health care and homeland security industries. In addition to the numerous other firms he has founded, he gained valuable experience at IBM, where he participated in the development and management of command and control systems for nuclear submarines, and implemented advanced systems for the Central Intelligence Agency. His latest venture, Affinity Networks, has recently launched IntelaCare, a cell and broadband phone based service that delivers multimedia health care content and applications to specific patient populations. The Army and several commercial entities are currently using IntelaCare to deliver two-minute diabetes management videos to the cell and broadband home phones of patients and employees. IntelaCare has demonstrated daily viewership as high as 70%, and has had a significant impact on the health of all participants, with similar results expected in other chronic disease areas.

Catherine hess Catherine Hess is a Senior Program Director based in the National Academy for State Health Policy’s Washington, DC office, where she develops and directs programs focused on child health coverage, community health centers, and other health policy topics important to states. She serves as the Program Director for Maximizing Enrollment for Kids, a $15 million national initiative of the Robert Wood Johnson Foundation; the Project Director for a David and Lucile Packard Foundation program funded for over a decade to provide technical assistance to and report on state CHIP programs; and Project Director for NASHP’s National Cooperative Agreement with HRSA’s Bureau of Primary Health Care to better inform state policy-making as it affects federally qualified health centers. Catherine has nearly 30 years of senior and executive level state and national health policy experience. She was the first Executive Director of the Association of Maternal and Child Health Programs (AMCHP), a position she held for 14 years. John Kaelin John Kaelin is a Senior Vice President at the AmeriChoice Corporation, a subsidiary of UnitedHealth Group. AmeriChoice health plans serve more than 2.7 million Medicaid, Children’s Health Insurance Program (CHIP) and Medicare beneficiaries in 22 states. John is heavily involved in the Company’s efforts to develop new products for Medicaid and the uninsured and works closely with states on Medicaid managed care rate-setting issues. Earlier in his career at the New York State Health Department, he played a leadership role in the establishment of New York State’s Medicaid managed care program in the mid-1990s. After leaving New York state service, John was appointed Chief Executive at the University of Maryland’s Center for Health Program Development and Management in 1998 where he helped implement the concept of risk adjusted payments in a number of states.

Kinzbrunner, md, faCp, faahpm Dr. Barry Kinzbrunner is executive vice president and chief medical officer for VITAS Innovative Hospice Care®, one of the nation’s oldest and largest providers of end-of-life care. Barry has cared for the terminally ill and their families since 1984. As chief medical officer, he serves as liaison between VITAS’ growing array of hospice programs and community physicians providing hands-on care to more than 11,000 patients each day. A national leader in defining the role of the hospice medical director, Barry has extensive experience in developing professional performance standards, pain and symptom management guidelines, quality assurance, and hospice and palliative care education. Timothy murphy Timothy Murphy joined Beacon in 2007 as President. Prior to joining Beacon, Mr. Murphy served as the Secretary of Health and Human Services for the Commonwealth of Massachusetts. A recognized leader in health care policy, Mr. Murphy’s signature accomplishment during his tenure was the development, passage, and initial implementation of the landmark Massachusetts health care reform law. Prior to his governmental service, Mr. Murphy was a successful investment banker in New York City. He holds a BA from Merrimack College, where he is a member of the college’s Board of Trustees, and an MPP from Harvard University.

Jeremy J. Nobel, md, mph Dr. Nobel designs health care management and delivery systems that focus on cost and quality concerns. Working with employers, government, as well as health plans, he develops and evaluates computer-based information technology applications that coordinate critical aspects of health care delivery, support optimal practice patterns and improve patient satisfaction. His contributions to this field span 20 years, and include providing health policy insight to the landmark Institute of Medicine of 1991 study addressing the automated patient record. He has worked in consultation to several major corporations and foundations in the design and evaluation of effective health care management programs including Blue Cross/Blue Shield of Massachusetts, J&J, Mercer, HBOC, Chrysler, GM, Safeway, Sanofi-Aventis, Pfizer, GSK, Verizon, CAREMARK, IBM, Medtronic, McKesson, Blue Shield of CA Foundation and the California Endowment. He is on the adjunct faculty of the Harvard School of Public Health where he teaches and does research on the health policy and management issues related

He received his medical education at the University of Pennsylvania and completed his internal medicine residency at the Beth Israel Hospital, Boston.

Lee partridge Lee Partridge is a senior health policy advisor at the National Partnership for Women and Families, where she specializes in performance measurement and quality improvement issues with special reference to lower income individuals and families. From 1992-2003, she headed the health policy section of the American Public Human Services Association; the Association provides the staff support for the National Association of State Medicaid Directors. From 1983-1992, she directed the Medicaid program for the District of Columbia. Her background also includes seven years on the staff of a U.S. Senator and five as chief of staff for the health and welfare committee of the DC City Council. She has co-chaired several performance measurement committees of the National Quality Forum (NQF) and recently completed serving on its committee to evaluate a set of perinatal quality measures. She is also a member of the NCQA Advisory Panel considering measurement approaches for evaluating the Patient-Centered Medical Home.

Lourdes rivas Lourdes Rivas is Vice President of Client Services – Managed Care for the DentaQuest government division. Prior to her role at DentaQuest, Ms. Rivas served as Chief Operating Officer of Atlantic Dental Inc. ADI was acquired by DentaQuest in 2008. DentaQuest is one of largest government dental benefits administrators in the nation. It currently serves nearly 50 managed care organizations, seven state agencies, 12 million members and approximately 17,000 participating dentists. DentaQuest also offers vision benefits administration through its affiliate, eyeQuest. Ms. Rivas is in the midst of guiding her clients through an expansive operational and system change. The initiative is comprised of enabling technologies that will provide DentaQuest with the tools to set benchmarks for the industry in claims processing, reporting, cost management and wellness research.

david J. sand, md, mba, faCs, ChCQm, faihQDr. David J. Sand received his undergraduate and medical degrees from Brown University. After nearly 15 years of private practice, he earned his MBA with a Health Care concentration from Regis University in Denver. harvinder sareen, phdDr. Harvinder Sareen is the Director of Clinical Programs at WellPoint’s State Sponsored Business. She plays a key role in leading WellPoint’s efforts to address childhood obesity among its commercial and Medicaid members and facilitating innovative BMI training and other resources for primary care physicians. Under her leadership, the initiative has partnered with academic institutions for the scientific evaluation of initiative activities. bob Tanner, Cpa, mbaAs PharmaStar’s Director of Operations, Bob Tanner currently oversees the operations and business functions for PharmaStar LLC, a pharmacy benefit manager specializing in Medicaid and Medicare.
