TheTechnologyEnabledCareProgrammecommissionedYoungScottobringyoungpeoplefromacrossScotlandtogethertolookathowdigitaltechnologycouldimproveyoungpeople’shealthandwellbeing.
Introduction
Asagenerationwhohavegrownupwithdigitaltechnology,youngpeoplearewellplacedtobringtheirknowledgeandexperiencestotheco-designprocess,andeagertoadoptandintegratedigitaltechnologyintothewaystheyaccesshealthcareandhealthinformation.Thisreportprovidesaseriesofinsightsintohowandwhyyoungpeopleusedigitaltechnology,alongsidetheirideasandsuggestionsforhowtechnologycouldbeused
tosupportyoungpeopleinthefuture.TheseinsightsandideaswillhopefullygoontoinspiretheTechnologyEnabledCareProgrammetodevelopnewsolutionswhichwillimprovethehealthandwellbeingofyoungpeopleinScotlandandlookintohowexistingYoungScotservicescansupportthis.
Background
InDecember2016,YoungScot’sHealthierScotlandYouthInvestigationTeamdeliveredtheirreporton“CreatingaHealthierScotland”totheCabinetSecretaryforHealthandSport.
Thisprojectgaveyoungpeopletheopportunitytoexplorewhatreallymatterstothemabouthealthandsocialcare,andtomakeapositivecontributiontotheshapingofpolicyandservicedelivery.TheYouthInvestigationTeamconsideredtheScottishhealthsystemasawholeandidentifiedfourkeythemestobetackled:
TheYouthInvestigationTeamcreated34keyactionsfortheCabinetSecretaryandtheScottishGovernmenttoconsider,howevertheydidn’tspecificallyaddresstheroledigitaltechnologycouldplayinsupportingandenablingthesekeythemes.Thisprovidedanopportunityforyoungpeople,YoungScotandtheTechnologyEnabledCareProgrammetocollaboratetoexplorehowdigitaltechnologycanbeusedtosupportthesethemes.

contentofpersonal,socialandhealtheducation,andphysicaleducationinschoolstoensureallyoungpeople

YoungScot’sco-designprocesstakesyoungpeoplethroughfourstagesofideadevelopment,explore,create,reflect,andrecommend(Appendix1).Toobtainarangeofviewstoinformthefinalrecommendations,YoungScotdeliveredaseriesofengagementworkshopsinEdinburgh,Inveraray,WishawandtheninEdinburghagain,involving36youngpeoplefromarangeofbackgrounds,agedbetween11and23,andwithavarietyofhealthexperiences.
Co-design Process
Duringtheseengagementworkshops,YoungScotguidedtheparticipantsthroughtheexploreandcreatestagesoftheco-designprocess.Theyoungpeopleworkedonaseriesofactivitiestodefinethemostrelevanthealthandwellbeingissuesforyoungpeople,consideringhowdifferentlifeexperiencescanimpacthealthandwellbeing.Theyexploredthetechnologytheyuse,whytheylikeanddislikecertaintechnologies,andwhatthefutureoftechnologycouldmeanforyoungpeople’shealthandwellbeing.Theseexplorationsledintothecreatephaseoftheproject,withparticipantsacrossthesessionscontributingalmost100ideasforusingdigitaltechnologytosupportyoungpeople’shealthandwellbeing.
Followingtheengagementworkshops,YoungScotbroughttogetherapanelofyoungpeoplefromdifferentworkshopsatanIdeasDevelopmentSession.TheIdeasDevelopmentSessionwasachancefortheyoungpeopletoreflectontheissuesandideasgeneratedattheengagementsessions,andtoevaluateandcombineideastocomeupwithanumberofkeyconceptsandrecommendationstopassontotheTechnologyEnabledCareprogrammeforfurtherconsiderationanddevelopment.

Duringtheengagementworkshops,participantswereaskedtoidentifythekeyissuesaffectingtheir,andotheryoungpeople’s,healthandwellbeing.Awiderangeoftopicswerecoveredthroughthisprocess,butthemostfrequenttopicsraisedbytheyoungpeoplearesummarisedbelow.
Acrossalltheworkshops,youngpeopleidentifiedagreaternumberofbarrierstoachievinggoodmentalhealth,andno-onementionedbarrierstophysicalfitnessspecifically(suchaslackofaccesstosportsequipment),whichsuggeststhattheunderlyingreasonsbehindyoungpeoplenotexercisingaremorecomplex.

Youngpeopleuseawidevarietyofdigitaltechnologyintheirdailylivesforawiderangeofpurposes.Itisimportanttobearinmindthroughoutthisreporthowever,thatasignificantminorityofyoungpeoplestruggletoaccessdigitaltechnology,andmanywhodohaveaccessmaystrugglewithdigitalliteracy.

Connecting with other peopleThesocialconnectionsthatdigitaltechnologycanfacilitateisoneofthemostimportantelementsforyoungpeople.Connectionscanbebetweenexistingfriendsandfamilymembers,betweenmembersofadigitalcommunity,or
betweenyoungpeopleandpeopleinthepubliceye.Someplatformsmayalsofacilitatecommunicationbetweenlearnersandteachers,colleagues,oryoungpeopleandservicestheyengagewith.
InformationWhenyoungpeopleusetheirmobilephonestoaccessinformation,itislargelyafunctionofnecessity.Theymightcheckthebustimesorweatherbeforeleavingthehouse,downloadaboardingpasstoanairline’sapp,checktheirbankbalanceonthego,orfindagooddealattheirfavouriteonlinestores.Foryoungpeopleineducation,therearemorereasonstoseekinformationontheirphone–revisionnotes,classtimetables,learningaidsandsoon.
Thereisanotherwaythatyoungpeopleuseinformationontheirphone,mostobviouslywithhealthrelatedapps,whichistostoreandaccessinformationaboutthemselves.Thiscouldbetrackingnutrition,recordingrunningroutes,workingthroughasteppedexerciseprogramme,orlearninghowtocopewithanxiety.
EntertainmentMobilephonescanprovideawealthofentertainmentforyoungpeople.Someentertainmentisconsumptionbased,suchaswatchingTVprogrammesorlisteningtomusic,butmanyothersareusedbyyoungpeopletoactivelycreateimages,videos,fiction,reviews,andothercontentfortheir
peersortheinternetatlargetoengagewith.Alongsidemobilegaming,theseformsofentertainmentarenotstaticresources,butplatformstofacilitatecreativity,connection,andskillsdevelopment.

What do young people like and dislike about the technology they use?
Inadditiontothemobilephoneappsrelatedtohealthandwellbeingthatyoungpeopleused,andthefitnessrelatedwearabletechsuchasFitbits,youngpeoplefrequentlymentionedthattheyuseothertechtosupporttheirhealthandwellbeing.Thiskindofsupportcanbeinstructional,social,oraboutself-management.
InstructionalexerciseorworkoutvideosonYouTubewereoneofthemostpopularsuggestionshere,withsomeotherspointingtoInstagramasagoodsourceofinformation,inspirationandsuggestionforexerciseandhealthyeating.Otherssoughtoutinstructionalvideosontopicssuchasmake-upapplicationorhairstyling.Especiallyforyoungwomen,learningandpracticingthesekindsofskillsallowsthemtobemoresuccessfulintheirsocialcircles,increasingpopularityandbolsteringsocialstandingandfriendships.Thiskindofskillacquisitionthereforeplaysanimportantroleinsupportingmentalhealthandpreventingsocialisolation.
Forsome,thesocialnetworksthattechcansupportweremoreimportant,withmanyyoungpeoplementioningthewaythatmessagingtechnologyandappslikeSnapchatallowyoutokeepincontactwithsupportnetworksandaskforhelpwhenneeded.Forothers,accesstoonlinecommunitiesbasedaroundsharedinterestswasawayofnotfeelingsoaloneandofconnectingwithpeoplearoundtheworld.
Manyyoungpeoplethough,useavarietyoftechnologyforself-management.Thisincludesawiderangeofbehaviours,suchaslisteningtomusictorelievestressorimprovemoodwhenfeelingdown,consumingmediaandplayinggamestokeepthemindoccupiedordistractoneselffromfeelingsofanxiety,orwritingthoughtsandideasdownasamethodforself-reflectionandexpression.Oneyoungpersonmentionedthattheylistentoautonomoussensorymeridianresponse(ASMR)videostohelpthemsleep,andanothermentionedreadingontheirKindleasawaytomaintainhappiness.Finally,someyoungpeopletalkedaboutusingtechnologythatsupportstimemanagementandorganisation,suchascalendars,email,andbankingapps.Keepingontopofresponsibilitiesmadethemfeellessstressedandmoreableofmanagingtheirschedules.
Youngpeopleattheengagementworkshopswereaskedtorankthetechnologytheyusedfromtheirmosttoleastfavourite,andthentotelluswhytheylikedordislikedcertainpiecesoftech.Asisreflectiveofthepredominanceofmobilephoneappsinthelistoftechnologyyoungpeopleuse,manyoftheissuesraisedrelatetomobileapps.
Thefavouredpiecesoftechvariedfromindividualtoindividual,butthreekeyareasemergedfromthisactivity.
UsabilityUsabilitywasbyfarthebiggestdecidingfactorinwhetheryoungpeoplelikedthetechnologytheyused.Themanyissuesraisedunderusabilityindicatethatappdesignneedstobewellthoughtoutandtestedbyayoungaudiencetoworkthroughbugsbeforelaunch.Afterlaunchanappneedsadedicatedteamofstaffworkingbehindthescenestocreateseamlessupdateswithoutchangingfundamentalaspectsoftheappdesignandkeepontopoffeedbackfromusersaboutdifficulties,likesanddislikes,andcrashreports.
Instructional exercise or workout videos on YouTube were one of the most popular suggestions here, with some others pointing to Instagram as a good source of information, inspiration and suggestion for exercise and healthy eating.

Games for LearningInfact,usinggameswasabigthemeacrosstheboard,withyoungpeoplesuggestingusinggamesorquizzes(suchasKahoot!)tofindoutinformationinafunway,tochecklearning,andprovidetangiblebenefitsfromin-gamerewardsbeingexchangedforrealliferewards.Gameplayingcouldreplacetraditionalhomeworkactivities,oractasa‘braingym’stylewarm-upatthestartoflessons.Youngpeoplecouldbedirectlyinvolvedincreatingsuchtechnology,linkingupwithothersubjectareasatthesametime.

Physical EducationDigitaltechnologycouldsupportfitnessbyenablingyoungpeopletousethingslearntinlessonsafterschool.SuggestionsincludetheabilitytoaddexercisesthathavebeenlearntinPEforuseathome,loggingyourphysicalactivityandrecordingpersonalbests,orastructurepersonalfitnessprogrammewhichencouragesplanningfuturelearning.FitnessappscouldalsobeincorporatedintoPElessons,withteacherssupportingpeopletonavigateanewappandlearnhowtomakethemostofit.SchooltechnologysuchasTVscreensandemailbulletinsshouldalsobeusedtoraiseawarenessoflocalsportsclubsandclasses.
Mental HealthDigitaltechtosupportlearningaboutmentalhealthcouldbedeployedinasimilarwaytothosementionedabove,actingasasourceofinformation,providinggamesandknowledgechecks,andguidingyoungpeoplethroughaprogrammeoflearning.Techcouldalsobeusedinschooltoprovidedirectsupporttostudentswithadigitalchatservicethatstudentscouldaccessprivatelyduringmomentsofneed,orperhapsusinglikereadaloudtechnologytoallowananxiousstudenttocommunicatewithateacherwithouthavingtospeak.
StigmaForthereducingstigmatheme,thereweretwomainareasthatyoungpeoplemadesuggestionsabout,onewaswideningaccesstoservicesandtheotherwaspromotionofmentalhealth.YoungScotandtheScottishAssociationforMentalHealth(SAMH)arecurrentlysupportingaYouthCommissiononMentalHealthwhosefindings,whenpublished,willfurtherilluminatethistopicandprovideawealthofideasforconsideration.
Reducing the Gender GapMultiplepeoplesuggestedthatdigitalservicesshouldmakemoreofanefforttopromoteortailorthemselvestoboysandyoungmen.Therewaswideacknowledgementthatboysandyoungmenhavegreaterdifficultyexpressingtheiremotionsandseekinghelp,andthatthistranslatesintoahighersuiciderate.Appsandservicesthataretargetedatboysandyoungmen,andco-designedwiththem,couldbeonewaytotacklethisissue.
Awareness Raising and EducationDigitaltechcouldbeapowerfultoolinraisingawarenessofmentalhealthdisordersandcommonsymptoms,oraguidetowhatlanguagetousetotalkaboutmentalhealth.Techcouldprovidebasic,clear,easy-to-accessinformationaboutmentalhealth,couldteachpeoplehowtodetectsignsofpoormentalhealthinothersandsuggesttipsandadviceonhowtosupportthem.Suchtechshouldbeavailabletokeyfiguresliketeacherstoprovidethemwithguidelinesonhowtopromotementalhealthinclassortacklementalhealthrelatedbullyingormisinformation.
Greater Availability of SupportManyideasfortacklingstigmawereaboutaccessingsup-portservicesandsupportingself-management,suggestingthatreducingstigmamightmeannormalisingtheuseofmentalhealthservicesandgrowingtheseservicesexten-sively.Specificideasincludeonlinegroupschatstogainsupportfrompeoplewithsimilarexperiences,videocallswithcounsellorsortherapists,andtheabilitytoaccesssupportanonymously-whetherone-to-oneorinagroupformat.Forself-management,youngpeopleemphasisedappsthatcanteachdistractiontechniquesorcopingmechanisms,ortechthatfacilitatesself-explorationtoallowyoungpeopletogaininsightintotheirthoughtsandemotions.
Public AwarenessItnotedbyafewyoungpeoplethatthegeneralpublicalsoneedtobetargetedwhentakingmeasurestoreducestigma.Anyofthetechthatservestoeducateandraiseawarenessneedstoberolledouttoadultsacrosssociety,andthatpartnershipswiththemediacanbeforgedtoraiseawarenesstoo.Theremaybedifferentchallengesinchangingtheperceptionsofthegeneralpublic,soworkshouldbedonewithdifferentpopulationstolookatthemessagesandinformationthatwouldappealtothem.

Demystifying AppointmentsForsome,attendinganappointmentwithahealthprofessionalcanbeacauseofanxietyofuncertainty,sosuggestionsweremadetousedigitaltechtoraiseawarenessofwhathappenswhensomeoneseekshelpandsupport.Thiscouldincludedescriptionsofdifferenthealthservices,howreferralswork,whatanappointmentwillbelike,whatyouwillbeaskedabout,whatconfidentialitypolicieshealthprofessionalsabideby,andsoon.
Therewasadesirebyyoungpeopletoknowthehealthprofessionaltheywouldbeseeingaheadofameeting,sotheyfeelpreparedandcomfortable.Digitaltechcouldfacilitateanintroductorymeetingorconversation,orprovideinformationaboutstaffatahealthcentre,sothatthepersontheywillseeisnotastranger.Thiscouldevenbeagroupchatonlineaheadofaone-to-oneappointment.Anotherbenefitofthiskindofinterventionistoreassureyoungpeoplethathealthprofessionalswillbefamiliarwiththeissuesyoungpeoplefaceandthattheywillreceiveappropriatecarefromasupportiveandunderstandingsource.
Eating and fitnessEncouraginghealthyeatingandfitnessisacomplextask,butyoungpeoplehaveidentifiedanumberofwaysthattechnologycouldhelpmotivatepeopletoworkbetteratit.
Personal Tracking and SupportForbothfitnessandhealthyeating,youngpeopleemphasisedtheabilityofdigitaltechnologytobetailoredtotheindividual.Afitnessappcouldbuildupfitnessatyourpace,keeptrackofyouractivity,andhelpsetachievablegoalsforthefuture.Similarly,forhealtheatingadiary-liketrackercouldletyoungpeoplerecordwhatthey’reeating,makesuggestionsforwaystoimprovediet,andsuggesthealthyrecipesorwaystoplanabalancedmeal.
GamificationYoungpeoplewantedtomakefitnessmorefunbyincorporatinggamesandrewardsintoactivities.Peoplecouldreceivevirtualorreal-liferewardsforachievinggoals,theycouldbepresentedwithnewandfunfitnesschallengestoworkonbythemselvesorwithfriendsandfamily,potentiallytappingintotrendssuchasthe‘BringSallyUp’workoutchallengepopularonYouTube.
Fitnesschallengescouldtakepeopleoutofthehouse,withyoungpeopledrawinginspirationfromtheaugmentedrealitygame‘PokémonGo’tosuggestagamewherepeoplemovearoundtheiroutdoorenvironmenttofindrewards.Gameslikethiscouldtakeonasocialdimensionwithcompetitionsorevencollaborationstounlockcertaingoals.Fitbitsorsmartwatchescouldbelinkeduptothisgametoenablepeopletotracktheirmovementsandactivity.

Inaddition,therewouldbetwokeyfunctionalelementstothiswebservice;bookingonlineappointments,andremotevideoappointments.Theonlinebookingservicewouldallowpeopletoseetheavailabilityofappointmentsandself-selectatimethatisconvenientforthem.Aswellasincreasingefficiency,theonlinebookingsystemwillalsobeeasierforyoungpeopletonavigateasmanydislikespeakingonthephoneanddon’thavetheflexibilityortimetomakeanappointmentinperson.
