This non-appointment service, part of MCHD’s Hot Shots Summer Immunization Program, will be provided at 251 N. Bayou Street from 8 a.m. to noon each Saturday until the start of the 2016-17 school year. The Alabama Department of Public Health (ADPH) requires each child enrolled in day care, Head Start, and public or private schools to have a valid Certificate of Immunization (COI) — previously known as “blue cards” — or a Religious Exemption Form on file at the facility they attend. MCHD implemented a new fee schedule associated with its immunization program on July 1. Patients who receive their vaccinations through Family Health will receive the up-to-date COI at no charge. All non-Family Health patients will be charged an administration fee of $10 per patient to receive an up- to-date COI. If vaccines are not up-to-date, parents or guardians should follow-up with their primary care provider, visit Family Health’s Saturday Clinic at 251 N. Bayou Street or schedule a same day/next day appointment at one of the Family Health’s clinics. For out-of-state individuals, the administration fee to receive an up-to-date Alabama COI will be $30 per request. For all Religious Exemption Forms, a one-time administration fee of $30 per patient will be charged. The process includes the viewing of an approximately 15-minute required video and counseling. Alabama’s School Immunization Law Section 16-30-4, states all students must present a COI. School Rules Chapter 420-6-1 (1) states the COI will be on a form approved by ADPH, which is the one printed from ImmPRINT. All required vaccines must be recorded before the COI can be submitted to the schools.
