You will also find a Handbook Signature Form, an Email Request for Teacher Correspondence, the Oakwood School Calendar, Oakwood Arts Bridge, and PTO membership information. The Computer Network/Internet Acceptable Use Policy and Agreement Form gives your child permission to access Oakwood’s computer network and the Internet. This year’s directory information will be available to parents online. Please refer to the PTO Welcome letter for details on entering and accessing this information; this will allow for your family’s address and telephone number to be printed in the Junior- Senior High School Directory. Your Progress Book login and password remain the same as what you created last year. In the event you cannot recall your login and password, visit the OJH website, then click on Progress Book and follow the prompts. You may also go to https://gb.mdeca.org/ParentAccess/account/recovery for assistance. If you are still unable to access your Progress Book please contact our technology department at (937) 297-1513. The Student Contact and Information Form (Emergency Medical Form) is required by the State of Ohio to be completed annually for each student by his or her parents or guardians and to be on file at school. In attempt to make things easier for our parents, we have created a partially populated form for you to closely review. Please make any necessary changes and additions. The accurate information will instruct the school authorities in the procedure you wish to be followed in the event of a medical emergency involving your child. The telephone numbers that you list will be used to reach you in case of an emergency – please be certain all contact numbers are accurate. Please be sure to list an alternate person to be notified if we are unable to reach parents in the event of illness or emergency. Remember to contact the OJH Office with any change in contact information over the school year. Free milk applications are available in the Jr. High Office or online at www.oakwoodschools.org. Please contact the office at 297-5328 if you have any questions regarding the free milk program. Please complete all of these forms and return them with your child to give to his or her TAKE 5 teacher during the first week of school, no later than Friday, August 18 th . All students have received their own school year agenda planner. The $5.50 cost of the planner is to be paid with the school fees. (Please remember that you can pay your school fees online with EZpay. Please recall that these services charge a small convenience fee if you will be using the system to pay for lunches and/or school fees.) The 2017-2018 planner also contains our Student/Parent Handbook. We ask all parents and students to please read the handbook and be familiar with the school rules and operating procedures. Included in this packet is a Student Handbook Form requesting both a parent and a student signature indicating that you have read this handbook. The Oakwood City School District uses MailChimp to communicate with our families. Subscribing to the school’s MailChimp Mailing list is needed in order to receive school information. Important information may be missed without this step. In order to receive important school information you must subscribe to MailChimp. You can visit any of the Oakwood School websites (oakwoodschools.org) to find the MailChimp link. Please be sure to read the first paragraph after opening the link and follow the steps. You have the option to choose which groups you wish to receive correspondence about. Please choose School Information to receive items such as school newsletters or other important information from your building principal. Also be certain to choose District News to receive important news from Dr. Ramey. If your email address changes over the course of the year please be sure to subscribe your new email address with MailChimp. Please take this opportunity to subscribe to MailChimp as this is our new way of communication with families.

Welcome! The Oakwood School District utilizes the SPS EZpay system for parents interested in paying for student fees, or adding money to student lunch accounts via the internet. If you do not wish to use the system or have already established an account, no action is necessary. The EZpay system allows you to use your credit/and or debit card for the online transactions. A convenience fee (3.99%) is charged by the vendor for their service. We cannot take credit card payments at our office, this is only available over the internet. We continue to accept cash and checks! Parents who use the on-line system must first create an account using the student PIN (Personal Identification Number), which will remain the same throughout your child’s school years. Your child’s PIN is a four or five digit number that is found on the top-right corner of their schedule in the box labeled “NUMBER”; please have them memorize this number. No personal information about your student is available online. The district does not keep your credit card information on file and you will need to re-enter your credit card # each time you use the online system. You can access the EZpay online payment website by navigating to http://www.spsezpay.com/Oakwood. The district’s website contains links to EZpay as well. Once at EZpay’s site, first-time users should create a new account, and follow the process- consisting of completing the registration page, and adding your child’s names and ID numbers. Once you have registered and created a password you will not need the student ID number again. You can check your child’s account balances as long as you have access to the Internet. You have the option to receive a Low Lunch balance email. Please be sure to allow SPS EZpay into your email otherwise it will be considered spam and will go into your junk email. Questions may be directed to your child’s school or 1-866-MyEZpay (866-693-9729) or email at [email protected]. Payments made online will be credited by the end of the next business day. We are glad to offer this option, and hope that you find it useful.

Oakwood Junior-Senior High School 2017-2018 Athletic Ticket Price Information

Student Athletic Pass: $70 The purchase of an Oakwood Student Athletic Pass for the 2017-2018 school year will admit the purchaser to all junior and senior high school athletic events for which admission is charged. The athletic pass provides the opportunity for the named student to participate as a spectator at a variety of athletic contests at the lowest possible cost.

A Student Athletic Pass is a real bargain. Compare the cost of the pass to the at-the-gate prices:

Junior High Athletics Family Pass: $60 A Family Pass for junior high athletics will admit the named parents and all elementary age family members to all junior high home athletic events throughout the year. (Admission to Invitationals, any school or OHSAA tournaments is not included in any Athletic Pass.)

“Fan”tastic Family Discount Pass: $50 For a cost of $50.00, you will receive $60.00 worth of admissions. This punch card pass allows any family member to attend any home sports event, excluding admission to Invitationals, any school or OHSAA tournaments. The appropriate admission fee will be punched from the various denominations on the card. (Admission to any OHSAA tournament is not included in the “Fan”tastic Pass) Replacement: If you lose your Student, Adult, Junior High Pass, a replacement card may be bought for $5.00 in the Athletic Office. The “Fan”tastic Family Discount Pass is treated like cash, and cannot be replaced. General Admission Tickets may be purchased at the gate. Costs are as follows:

Dear Oakwood Parents, The Oakwood City School District frequently has the opportunity to celebrate the amazing accomplishments of our students, including award recipients, participants in classroom projects, involvement in fine arts programs and other activities worth recognizing and promoting. This recognition appears on district websites, in print publications and on district-controlled social media, including Facebook, Twitter and Instagram. This recognition may include photos of students as well as their names and teachers’ names, in some instances. It may also include pictures or other representations of students’ artistic work, including visual and written pieces. We also occasionally have members of the local media visiting our buildings to cover events and unique approaches to education our staff is able to offer our students. Media access to students does not happen within our schools without district knowledge and supervision, and we do not provide direct access to students when the media is covering topics that might be considered controversial. Most parents enjoy seeing their children included in the day-to-day celebration of all the great things happening in Oakwood schools. For that reason, default consent and release for this type of recognition is granted to the district. However, we know there may be a few parents who prefer their children not be included in such photographs and recognition. Action is only required by parents who wish to DENY this consent. We require those parents to inform us, in writing, of their desire to have their children excluded from district-managed recognition by completing the Request to Revoke Consent form found on the district website or obtained from any school office. Completed forms must be returned to your child’s school by Sept. 1 each fall to ensure we have a current list of students whose activities the district SHOULD NOT publicly acknowledge on the website, on social media or in other public forums. STUDENT CODE OF CONDUCT – pages 12-13 in the Student Agenda/Planner General Statement:

All students at Oakwood Junior High School are expected to conduct themselves in a manner reflecting a basic belief in the worth and dignity of every person. Students are expected to (1) act courteously to adults and fellow students; (2) be prompt to school and attentive in class; (3) work cooperatively with others when involved in accomplishing a common goal regardless of the other’s ability, gender, race, or ethnic background; (4) complete assigned tasks on time and as directed; (5) help maintain a school environment that is safe, friendly, and productive; (6) act at all times in a manner that reflects pride in self, family, and in the school.

Specific Statement: 1. Fighting or assaulting another person while under the jurisdiction of the school; i.e., from the time students leave their home for school until they return home after school, is prohibited. This includes the entire lunch period. 2. While under the jurisdiction of the school, any behavior, which could result in harm to one’s self or another, damage of school property, or damage to the property of another, is prohibited. 3. Bomb threats or similar kinds of threats are forbidden and will be treated as most serious violations of school rules. 4. All forms of cheating are forbidden. This includes, but is not limited to, plagiarism, copying or downloading information without crediting the source, copying from another person or any circumstance in which academic credit is obtained dishonestly. No credit will be given for an assignment for which a student is found to have violated this rule. In addition, other academic and/or disciplinary penalties may be imposed. 5. The writing of graffiti on corridor walls, student lockers, classroom desks, walkways, trees, vehicles, or other school property is prohibited. 6. No student may cause another to be intimidated, bullied, pestered, harassed, or hazed (including “initiations”). No student shall use racial, religious, sexual orientation, or other derogatory slurs. 7. Students may not possess articles, e.g., guns, knives, chains, explosives, fireworks, etc., which would clearly have an adverse effect on the welfare of the school. 8. Students may not use, possess, transmit, buy, sell, or be under the influence of any mood-altering chemical, nor possess any drug paraphernalia while under the jurisdiction of the school, including any activity, function, or event, whether on or off campus. 9. The possession or use of tobacco products including e-Cigarettes on the school premises or at any school-related activity is prohibited. 10. Stealing in any form and by whatever name is prohibited. 11. Extortion is prohibited. No student may, through use of force, suggestion, or threats, take items from another person. 12. No student shall knowingly cause harm to an Oakwood School District employee in any manner. This prohibition applies at all times and in all places. 13. No student may verbally or physically threaten or show disrespect to another student or staff member, disobey a staff member’s legitimate directive, flaunt the rightful authority of a school official, use profanities and/or obscenities, either verbal or non-verbal, or

repeatedly violate school rules. 14. Truancies from school and unauthorized or unexcused absences from classes are in violation of the compulsory attendance laws and are not permitted. 15. Disruption of any class or school activity or any inappropriate behavior that could have an adverse effect on the welfare of the school is prohibited. 16. Running, roughhousing, throwing objects (including snowballs), or any other potentially dangerous behaviors are prohibited on school property at all times. 17. Public displays of affection are inappropriate on school premises and are not permitted. 18. During the time that school is in session, students may not leave the school premises without specific permission from the principal or his designee. 19. The sale of any commodity during school hours and upon school property without prior permission of the principal is prohibited. 20. Distribution or posting of non-school printed material while on the school premises without permission of the principal is prohibited. 21. The consumption of foods and beverages is limited to the student cafeteria area at noon and at other times, except when such consumption is a part of a recognized class activity under the direct supervision of the teacher. Students may eat lunch in other locations permitted by the school administration as long as the students clean up after themselves. 22. Gambling on school property is prohibited. 23. For the purpose of supervision, all sidewalks and streets adjacent to school property are considered under the supervision of school employees during school and extracurricular activities. 24. Unauthorized entry to school premises, including athletic facilities, is prohibited. 25. All school regulations shall apply to students during any school activity, function, or event, regardless of location. 26. The use of headphones is prohibited in the classroom during the school day, unless requested by the teacher for educational purposes. 27. Wireless communication devices are permitted in the building and may be used in the classroom when requested by the teacher for educational purposes. If not approved by the teacher, wireless devices should be turned off in the classroom. 28. Spraying of perfumes, colognes, and body sprays in the hallways or classrooms is prohibited. 29. “Sexting” is prohibited at any time on school property or at school functions. Sexting is the electronic transmission of sexual messages

or pictures, usually through cell phone text messaging. Such conduct not only is potentially dangerous for the involved students, but can lead to unwanted exposure of the messages and images to others, and could result in criminal violations related to the transmission or possession of child pornography. Such conduct will be subject to discipline and possible confiscation of the WCD.

30. “Cyberbullying” is prohibited at any time on school property or at school functions. Cyberbullying includes, but is not limited to the following: (1) posting slurs or rumors or other disparaging remarks about a student or school staff member on a website; (2) sending email or instant messages that are mean or threatening, or so numerous as to drive up the victim’s cell phone bill; (3) using a camera phone to take and send embarrassing photographs/recordings of students or school staff members or post these images on video sharing sites such as You Tube; (4) posting misleading or fake photographs of students or staff members on websites. To the extent permitted by the First Amendment, instances of cyberbullying off school grounds that disrupt the school environment or interfere with the learning process will be considered violations of the Student Code of Conduct.

The rules of conduct listed herein apply to all school sponsored activities, on and off campus, The list of school sponsored activities will include, but not be limited to, the following: band camp, all band trips and contests, all athletic events, all dances, after play parties, graduation rehearsal and ceremony, all banquets sponsored by school organizations, all intramural contests and practices, and all field trips.
