Overview & ResponsibilityOverview & Responsibility Directed by the FMSC. Monitored by the ESU. FMSC is responsible to EMC for:
– Design,training,education,implementation and assessment of performance & improvement.
Participative Representation:– Safety Unit, Security, Risk Mgmt, Equipment Mgmt,
Utilities Mgmt, Hazardous Waste Mgmt, Administration, Engineering, Infection Control, Training & Quality and Standards
Review all incident reports, property, Infrastructural damage, occupational illness,& P,S & V injury.

Organizational StructureOrganizational Structure
1.ESU – full time working body with assistance from designated persons from each department.
Standing members of the committee. Designated OH,ESU & IC Officers Provide program direction, technical
assistance & coordinate the safety and health effort.
Continued - ActivitiesContinued - Activities Collect Information Review summaries of deficiencies, failures,user
errors, Haz/ Mat & Waste, Life Safety, Emergency Preparedness, Med Equipment, Utility Systems & Disaster Preparedness.
Implement FMSC Recommendations Participate in all hazard surveillance, emergency
drills, incident reporting, & development of safety procedures.
Participate as members of IC,QI,RM,EP. Assist with development & implementation of
safety & health training.
Incident Reporting & Incident Reporting & InvestigationInvestigation
Procedure: -– Report used to document the occurrence of any event
that is inconsistent with the hospital ordinary routine, whether or not injury occurs.
– Employee incidents, encompass events related to injuries and occupational illness & exposure.
– Report on Infrastructural matters requiring immediate corrective action to Engineering by telephone followed by requisition (mainframe) requisition.
Data & info obtained is processed by the relevant departments and members of ESU/FMSC.
Hazard SurveillanceHazard Surveillance Hazard surveillance or inspections are referred to as The Safety
Inspection. All areas are inspected for the following: -
– Physical Environment– Safety Manual Regulations– Adherence to Safety processes and procedures– Staff Safety Knowledge concerning safety issues.
Written reports are forwarded to the appropriate department manager for corrective action within a specified time frame.
All findings by ESU are submitted to FMSC noting trends of concern.
All deficiencies which pose an immediate threat to health and safety or pose a serious threat or damage to the facility or equipment must be corrected immediately.
Program EvaluationProgram Evaluation Done on an annual basis. Conducted by the FMSC. Evaluated for the following: -
– Attainment of objectives / TOR– Scope of activities– Performance and – Effectiveness
Results reported to the Governing Board by the Committee Chairperson.
Individual elements are also evaluated using the same criteria, these are: -– Security Management Program– Infection Control Program– Risk Management Program– Utilities Management Program etc
Safety Management Safety Management Performance StandardsPerformance Standards
1. All new employees must participate in the orientation program.2. All response and resolution time for Hazard Surveillance
rounds will be less than one month.3. All staff must know who the Safety Officers are to facilitate the
reporting of safety hazards.4. Hazard Surveillance major inspections will be conducted
throughout the hospital;– Clinical Areas, Semi-annually– Non-Clinical Areas, annually
5. Total employee reported back injuries will be 5% or less.6. Total Biohazard Exposures will be 5% or less7. The injury to hours worked variance ratio for each month will
be lower than the same month during the previous year.

Safety is a manifestation of a personal state of awareness, standardized practices & a sound system of beliefs by all persons.
This process provides a means for continuous improvement & methods to sustain physical development while adhering to hospital policy & international & national regulatory agencies.
This program will satisfy, train, sensitize and empower all clients to the importance of reducing risks and injuries in the workplace and ultimately assist in the provision of quality and cost effective health care services.
