Please select this service if you would like to apply for an OCI card as a minor and you donot hold a PIO card. If both parents are Indian citizens, the minor applicant should applyfor a PIO instead.

A new OCI card for a minor applicant who DOES NOT hold a PIO card is given tothe child or grandchild of a non-Indian National who:

Was eligible to become a citizen of India on January 26, 1950 or later, orWas a citizen of India on, or at any time after, January 26, 1950, orBelonged to a territory that became part of India after August 15, 1947.Non US passport holders who hold a temporary visa such as B1/B2 are not eligible.

Your OCI application will not be processed without all items listed below. If all items are notprovided, your application will be returned to you in approximately 10 business days, andthe service fee of $12.00 will apply along with applicable shipping and handling costs.Documents should be provided in the same order as they appear on thischecklist.

We do not accept OCI applications submitted through travel agencies, visaservices, or any other agents. Applications submitted in this manner will bedenied and ALL Fees will be forfeited.

Please do not staple your documents, use paper clips instead.

1. OCI Online Application:

Complete the OCI Online Application Form. Once the OCI Online Application Form iscompleted and printed, the following should be submitted along with your applicationpackage:

One Part A per person that is applying, andOne Part B per person (if applying individually) OR one Part B per family (ifapplying as a family). Important, on Part B, question #21/#22regarding Family Members in India, you must answer the question. Ifyou do not have close family members living in India, 'No close familymembers living in India.' is an acceptable answer.

If applying as a family, all family members MUST reside at the same address. Familyapplications are limited to 4 family members per application. If applying as a family offive or greater, the family can be split up with the father and mother on separatefamily applications.

Complete the online OCI Online Application Form on the Indian Government's website.When you complete the online portion, you will need to print out Part A and Part B.After printing, complete and sign Part B. You will need to submit both parts (A and B)along with your application. You do not have to submit the instructions that will printafter Part B.

2. Payment - Travisa Order Form:

After you have completed the OCI Online Application, then complete the online

Travisa Order Form. This form tells Travisa Outsourcing how you will pay for yourapplication. You MUST include a copy of this form with all other supportingdocuments.

3. Photos:

Two (2) passport sized photos. Photos are required to be recent, identical, sized 2inches by 2 inches, in color, depicting front pose against light background onphotographic paper. Do not wear glasses in the photo. Detailed information aboutphotos can be viewed at OCI Photo Instructions.

4. Signature:



The OCI application has multiple places for signature. The following sections must besigned correctly:

OCI Part A: Each applicant must sign COMPLETELY inside the box on theirPart A form next to their picture. Minors who cannot write their name canuse a thumbprint in black ink instead. If any part of the signature orthumbprint for Part A is outside of the box, the application will berejected.OCI Part B: On Part B, it is very important that ALL applicants sign Part B. Forminor applicants, both parents must sign the Part B application form even ifboth parents are not applying for OCI. If only one parent is available to signfor the minor, proof of custody or a death certificate must be provided.

5. Copy of United States Passport:

You must provide a copy of your valid original United States passport (or Non Indianpassport if Non-US nationality) along with your application package at the beginning ofthe OCI request process. Your original US or non-Indian passport will be required onlyafter the OCI card has been received back from Delhi, India. A copy of the followingpages is required:

Copy of the information page. This page contains the picture and basic detailsof the passport.Copy of the endorsement/amendment pages. On US passport books that endwith page 24, copy pages 22, 23 and 24. On US passport books that endwith page 28, copy pages 25, 26 and 27.

6. Original Passport To Be Sent At The End of Processing:

Your original passport will be required as the last step in the process after your OCIcard has been approved and returned From Delhi, India to the Embassy/Consulate.When the OCI card is at this stage, you will receive notification and shippinginstructions automatically by email. At this point, you will need to provide your validoriginal United States passport (or Non Indian passport if Non-US nationality) so thatthe U-Visa (OCI) sticker can be affixed to a blank visa page. The passport must have aminimum validity of six months from the date of application with at least two blank visapages.

7. Copies of Parents US Passports or OCI Cards:

If you are sending an application for your minor child as an individual (not as a family),then photocopies of the parents OCI Cards or US Passports are required. If theparents OCI cards are copied, both information pages on the OCI card should becopied. If the US passport is copied, the following pages should be copied:

Copy of the information page. This page contains the picture and basic detailsof the passport.Copy of the endorsement/amendment pages. On US passport books that endwith page 24, copy pages 22, 23 and 24. On US passport books that endwith page 28, copy pages 25, 26 and 27.

8. Copies of Parents Indian Passports:

If you are sending an application for your minor child as an individual (not as a family),then photocopies of the parents Indian Passports are required. The copies should beof the first 3 pages in each Indian passport and the last 2 pages of each Indianpassport.

9. Copy of Naturalization Certificate:

If you are a former Indian national, a copy of your Naturalization Certificate is required.If a minor gained naturalization through his/her parents and possesses no separatenaturalization certificate, then copies of the parents' naturalization certificates and anotarized written statement signed by both parents is required stating thecircumstances that their child received his/her naturalization status and why the childdoes not possess his/her own certificate.

10. Copy of Birth Certificate:

For applicants born outside of India, a photocopy of the birth certificate or anygovernment issued document that shows parents name, date of birth and nationalitymust be provided.

You should not send the originals - we cannot be responsible for original documents.If you don't submit a copy, we will make a photocopy for you for $2.00.



11. Proof of Indian Origin:

Copy of first three and last two pages of the last Indian passport held, or anydocument that provides sufficient evidence that you held Indian Citizenship, such as:

A notarized copy of educational certificate issued by an institution in India andland ownership certificate by which eligibility may be reasonably ascertained,ORA duly notarized affidavit to that effect, ORA copy of an Ancestral Nativity Certificate issued by an Indian official ormagistrate with sufficient authority to confirm Indian origin of the applicant.

The decision of whether to accept this type of document as proof of your Indiancitizenship will be made by the Consular Officer and will be final in such cases.

If you do not have an Indian passport and you are applying for OCI on the basis thatyour parents and/or grandparents were Indian nationals, then you must enclose thefollowing documents:

Your birth certificate (in which your parent's name is mentioned) is required.If you are applying as the grandchild of an Indian national, then you mustsubmit your birth certificate as well as your parent's birth certificate. Allsubmitted birth certificates must show the parent's names clearly so that itestablishes the relationship with your parents and grandparents, ANDCopy of your parent/grandparent's Indian passport, ORCopy of your parent/grandparent's domicile certificate issued by thecompetent authority, ORCopy of your parents OCI cards, ORAny other document that provides sufficient evidence of yourparent/grandparent's Indian citizenship, such as a notarized copy of schoolcertificate and a land ownership certificate by which eligibility may bereasonably ascertained. The decision of whether to accept this type ofdocument as proof of your parent/grandparent's Indian citizenship will bemade by the Consular Officer and will be final in such cases.

12. Proof of Renunciation of Indian Citizenship:

If the applicant has already renounced Indian citizenship, one of the following must beproduced as proof:

A certificate of renunciation issued by a Consulate of India or,Your cancelled Indian passport with stamp stating the passport is canceleddue to obtaining US (or any non Indian country) citizenship.

If the applicant has NOT renounced their Indian citizenship, or cannot show proof, thefollowing must be provided:

Copy of your naturalization certificateMost recent Indian passport.

If the most recent Indian passport is available, provide the following:Two (2) copies of the completed Renunciation FormCopies of the first three pages and the last two pages of the mostrecent Indian passport.

If do not have your most recent Indian passport, your naturalization datewill determine what requirements are needed.

If you were naturalized over 10 years from today, you must provide two(2) copies of a deemed surrender certificate form.If you were naturalized less then 10 years ago, you must provide anotarized Affidavit of Loss and either a copy of your birth certificate or aschool leaving certificate.

There will be a separate renunciation fee automatically calculated on theOrder Form depending the date of naturalization:

Naturalized before June 1, 2010: $20.00 renunciation feeNaturalized on or after June 1, 2010: $175.00 renunciation fee

If the most recent Indian passport is retained for more than 3 years after acquisitionof citizenship of another country, a penalty of $250.00 will be automatically calculatedon the Order Form except where the most recent Indian passport expired beforeJanuary 1, 2005 or where the most recent Indian passport shows a 'Cancelled' stamp.In addition, a penalty of $250.00 will be charged for each case of misuse of the Indianpassport for travel after 3 months of acquisition of citizenship of another countrysubject to a maximum penalty of $1250.00.

13. Proof of Address:

Applicants must submit a proof of residence/address that matches the presentaddress on the OCI application exactly. This proof must also match the prepaidenvelope return address exactly. Photocopy of state issued ID (Driver License or ID),or the original major utility bill (Water, Gas, Electric, Sewage) must be submitted. Donot cut your drivers license copy down to a smaller size. Please leave the copy onregular sized paper. If you don't submit a copy, we will make a photocopy for you for$2.00.

The following items are not accepted: cell phone bill, credit card statement, bankstatement or lease. The address cannot include a P.O. box.

For minors, a proof of address from both parents or a legal guardian may be submittedinstead. For family (group) applications, only one copy of proof of address is requiredper family (group) as long as all family members reside at the same address.

For College/University students living on campus in university housing/dormitories, aletter from your institution on the institution's letterhead explaining the exact addressthat you presently reside in will suffice.

14. Name Change Documentation:

For former Indian nationals, if your name in your Indian passport is different in any wayfrom the name in your United States passport, one of the following documents mustbe provided.

Marriage Certificate showing the name change if the name was changed atmarriage.Official court order for the name changeNotarized letter stating that the name was changed during the naturalizationprocess.

15. Two (2) Copies of all Required Documents:

You must submit two (2) copies of all application forms and required documents,including the OCI Online Application Form. All copies should be self-attested with astatement that 'This is a true copy of the original' and your signature.

16. Prepaid Return Shipping Envelope - Must Match Home Address:

You must provide a prepaid envelope along with your application so your documentscan be returned to you once your application has been processed. A copy of thereturn shipping label or prepaid envelope is also required. If you don't provide a copy,we will make a photocopy for you for $2.00.

The prepaid envelope return address must match the home address as given on theproof of address. Do not provide a handwritten or manual UPS or FedEx air bill.Handwritten and manual airbills cannot be used by our office.

We strongly recommend using a traceable airbill generated online from FedEx or UPS.We also recommend that your return shipping package is insured. Travisa Outsourcingis not responsible for any damages or missing documents. No compensation fordamages is available.

17. Travel Rules for OCI Card Holders:

When traveling to India, an OCI card holder must carry both the OCI booklet and thepassport with the 'U' visa sticker.

18. Fees:

A Processing Service Fee of $12.00 will be collected by Travisa Outsourcing perapplication. Method of payment is Money Order or Cashier's Check (Payable to TravisaOutsourcing). If payment is not paid in full, processing delays will occur. If your OCIapplication is withdrawn mid-process, the Consular Fee is refundable less $25.00Consular processing fee. The Travisa Outsourcing service fee of $12.00 is non-refundable.

19. Processing Time:

OCI processing consists of 2 steps. The first step is applying for the OCI card. Thesecond step is after the OCI card is approved, the original US passport is sent in to bematched up with the OCI card.

Processing times for each step in the application process:

40 days (70 days if applying in the Houston Jurisdiction) from the time theapplication is sent from Travisa Outsourcing to the Indian Embassy orConsulate until the time that your physical OCI card is received by theEmbassy/Consulate from Delhi, India.Once the OCI card arrives at the Consulate, you will be required to send inyour original passport. Once your original passport has been received by theTravisa outsourcing Office, processing will resume3 days for Travisa Outsourcing to receive and send the US passport to theConsulate for match up.7 days for the Consulate to match up the US passport with the OCI card3 days for Travisa Outsourcing to ship the US passport and OCI card
