Dear Colleagues,

With every fall quarter, the opportunity arrives to make decisions about the benefits that will best support you and your family during the upcoming year. This year’s annual Open Enrollment is from October 28 through November 15, 2013, with your benefits elections going into e!ect on January 1, 2014.

It is your once-a-year opportunity to determine if you’re getting the most out of your Stanford benefits. During that time, you can make adjustments to your benefits elections, including changing health plans, adding or dropping dependents from coverage, and re-electing a Flexible Spending Account or Health Savings Account.

To help you make these important decisions, you will receive a booklet by mail in October, called Selecting Your Stanford Health Plan: A Quick Guide. The booklet will give you a high-level overview of the features and benefits of each of the health plans that Stanford o!ers.

You will have even more robust choices this year, as we’ve expanded our health plan o!erings. Stanford HealthCare Alliance is a new plan that will allow you to receive care at the world-class Stanford Hospital & Clinics and Lucile Packard Children’s Hospital and by Stanford-a!iliated doctors at a competitive price. We are excited to tell you more about this unique plan o!ering for Stanford employees, and we will be providing you with more details in October in the Quick Guide and on our new Benefits website at benefits.stanford.edu.

In the meantime, here is a sneak peek at some of the changes to your Stanford benefits o!erings and what to consider when making your benefits elections.

WHAT IS HAPPENING? WHICH PLANS ARE IMPACTED?Kaiser EPO PPO HDHP Dental Vision Other

New: The Stanford HealthCare Alliance will be added as another health plan option. (Read more about this new plan in this newsletter.)

Change: Delta Premier will move to a PPO network and become Delta Dental Basic PPO. The Delta Dental PPO plan name will change to Delta Dental Enhanced PPO.

New: VSP members (and up to four dependents) will be o!ered a Tru-Hearing discount program for hearing aids at a reduced cost.

Change: Copayments to see a specialist increase from $35 to $50 for the Blue Shield EPO and PPO, and a $50 copayment will be added to see a specialist under the Kaiser Permanente HMO plan.

Change: Coverage for non-network ambulatory surgery center charges is capped at $4,000 (60% of allowed charges with a maximum of $4,000 for out-of-network ambulatory surgery center charges).

New: Up to 20 acupuncture o!ice visits per year are covered at a $15 copayment for the Kaiser Permanente HMO, and $20 for the Blue Shield EPO plan. For the High-Deductible and PPO plans it is covered at 80% in-network and 60% out-of-network of allowable charges up to 20 visits per year.

New: Up to 20 chiropractic o!ice visits per year are o!ered at a $15 copayment for the Kaiser Permanente HMO and $20 for the Blue Shield EPO plan. For the Blue Shield PPO and High-Deductible plans, up to 20 chiropractic o!ice visits per year are o!ered at 80% (in-network) and 60% (non-network) of allowable charges.

New: Emergency room (ER) visits are subjected to a $100 copay; copay is waived under the Kaiser Permanente HMO and Blue Shield EPO plan if you are admitted to the hospital.

New: All ambulance services are subject to a $50 copay for Kaiser Permanente HMO and Blue Shield EPO plans (coverage 100% a"er copay).

Change: In 2014, the HSA limit—the amount you contribute in 2014—is $3,300 (for employees only) and $6,550 (for employees + dependents).

New: Benesyst is the new vendor for Flexible Spending Accounts and will issue new debit cards to FSA participants.

WHAT YOU MUST KNOW• You must make your benefits elections

by November 15, 2013.

• The benefits elections you make during Open Enrollment begin January 1, 2014 and will stay in e!ect until December 31, 2014.

• You cannot change your benefits during the year unless you have a change in your family or employment status (called a qualified life event change).

• You don’t have to enroll your eligible dependents in all the plans and coverage you elect for yourself. For example, you may cover yourself and your dependents for medical benefits, but cover only yourself for dental. However, if you do want to enroll eligible dependents in a Stanford health plan, you must enroll in the same plan as your eligible dependents.

• If you and your spouse/registered domestic partner both work for Stanford:

» Each of you may enroll for employee-only coverage, or one of you may enroll in employee+spouse, or employee+family coverage. Eligible children can only be enrolled under one parent’s plan.

» You cannot double cover your spouse/registered domestic partner or children under Stanford’s life insurance benefits. Because your spouse/registered domestic partner automatically receives basic life insurance from Stanford as an employee, he or she cannot be covered under spouse/registered domestic partner life insurance.

» If your spouse/registered domestic partner is enrolled in employee Accidental Death & Disability (AD&D) insurance, you cannot cover him or her under spouse/registered domestic partner AD&D.

• Your costs for all benefit plans will reflect 2014 rates.

WHAT STAYS THE SAME?• Kaiser Permanente HMO, and Blue Shield EPO, PPO and High-Deductible

Health Plan, will continue to be o!ered in addition to the new Stanford HealthCare Alliance plan.

• Delta Dental remains our dental care provider with two plan options: Delta Dental Basic PPO (previously called Premier) and Delta Dental Enhanced PPO (previously called Delta Dental PPO).

• Vision Service Plan (VSP) remains our vision plan provider.

WHAT SHOULD I CONSIDER WHEN MAKING MY OPEN ENROLLMENT SELECTIONS?When making your Open Enrollment elections for 2014, think carefully about whether your current benefits are working for you. Here are some questions you may want to consider:

• Have you had, or will you have, any major life changes, such as the birth of a baby or an adoption, a marriage or divorce?

• Are the doctors, hospitals, laboratories and other health care providers that you use considered “in network” by your current health plan? Would another Stanford-sponsored health plan be a better fit for you?

• Are you contributing the appropriate amount of money to your health care and/or dependent day care Flexible Spending Accounts (FSAs) for your out-of-pocket expenses?

• Do you have enough life insurance to provide for your beneficiaries should something happen to you?

IF YOU DON’T ENROLL by November 15, 2013, your benefits elections from 2013 will roll over automatically, except for these benefits:

• You will not have a health care and/or dependent day care Flexible Spending Account.

• You will not receive your Child Care Subsidy Grant if one had been awarded to you.

• You will not have a Health Savings Account (HSA). You must re-enroll in the HSA to contribute money and receive contributions from Stanford. If you do not re-enroll, your election will default to waive participation in the HSA. (Note: You can enroll in the HSA and elect $0.00 employee contributions to receive the employer contributions provided by the university.)

WHAT HAPPENS IF I DON’T ENROLL?

Our goal is to o!er you the best health plans at the most competitive costs. That’s why we’re introducing a new health plan for 2014: the Stanford HealthCare Alliance (SHCA).

SHCA is a select network health plan in which physicians from Stanford Hospital & Clinics (SHC) and Lucile Packard Children’s Hospital (LPCH), and a!iliated providers in multiple specialties, take responsibility for working together to carefully coordinate and deliver your care.

Stanford HealthCare Alliance is Stanford-quality health care provided by Stanford doctors to Stanford employees. With SHCA, you receive care from world-renowned physicians from SHC and LPCH at a low cost. You must select a primary care physician under this plan for you and your covered dependents. Fortunately, SHCA features an expanded network of primary care physicians to ensure you have choice in selecting the doctor who is right for you.

Under SHCA, your Stanford primary care physician coordinates your care across Stanford’s network using a state-of-the-art electronic medical record system. You also have access to a SHCA member care specialist, who is available to help you navigate your health care journey with Stanford HealthCare Alliance and answer all of your questions along the way.

All of the SHCA providers use the same electronic medical records system, allowing for seamless coordination of care.

• Low Cost, High Quality: Stanford HealthCare Alliance o!ers you the high-quality care you deserve and expect from Stanford Hospital & Clinics while o!ering a health plan at a price that is extremely competitive with health plans o!ered by other local employers.

• Personalized Member Care Services: This exclusive service for Stanford HealthCare Alliance members provides personalized assistance in scheduling appointments, selecting physicians, navigating your care experience and answering all claims and billing issues.

• Medical Records at Your Fingertips: From your computer or on your mobile device, make appointments, view key elements of your health record, get test results, view and pay bills, and message your Stanford doctor through the MyHealth online patient tool.

• Reputation for Excellence: Stanford Hospital & Clinics are known worldwide for advanced patient care, particularly in the treatment of rare and complex disorders, including in the areas of cardiac care, cancer treatment, neurosciences, surgery and organ transplants. This is why other hospitals send their patients to Stanford.

Benefits-eligible employees participating in the program for the first time in 2014 can earn a $200 taxable incentive by completing these two steps by November 30:

• Complete the Stanford Health and Lifestyle Assessment (SHALA) — an online health risk assessment; and

• Complete the Profile, which includes a health screening, advising session and online plan.

This incentive is also available for returning participants who have elected not to share or be on a Stanford medical plan."

Or, Benefits-eligible employees may also earn a $480 taxable incentive in the coming year ($20 in each paycheck) for completing these four steps by November 30:

• Complete the Stanford Health and Lifestyle Assessment (SHALA) — an online health risk assessment;

• Complete the Profile, which includes a health screening, advising session and online plan;

• Be enrolled in a Stanford-sponsored medical plan in 2014; and

• Agree to share your SHALA and health screening information. Your information will be reviewed with you by BeWell advisors, who will use your results to suggest appropriate health promotion resources. Your medical plan may also use your information for the purpose of health promotion and/or disease management outreach. Rest assured that BeWell and Stanford are committed to protecting the security of your health information.

One of the easiest ways to make a commitment to your health in 2014 is to

participate in BeWell@Stanford. This employee incentive program encourages

Stanford employees and their spouses or registered domestic partners to adopt

BeWell@Stanford Employee Incentive Program

YOUR SPOUSE OR REGISTERED DOMESTIC PARTNER CAN BENEFIT, TOO Partners of benefits-eligible university employees who complete the same four steps will receive a $240 taxable incentive. Incentives will be added to the employee’s paycheck in amounts of $20 for an employee and $30 for an employee + spouse.

Spouses/registered domestic partners are only eligible to receive the incentive if the employee is also eligible for the incentive.

OTHER WAYS TO PARTICIPATEBenefits-eligible employees can also earn $100 by completing six “Berries”—health-related activities offered by BeWell—before the November 30 deadline. Learn how to get healthy and earn money with BeWell at bewell.stanford.edu.

FITNESS AND HEALTHY LIVING CLASSES WITH THE HEALTH IMPROVEMENT PROGRAM (HIP)Did you know you have more than 150 fitness and health education classes available to you each quarter through the Health Improvement Program (HIP), part of the School of Medicine? If you are a BeWell participant and have completed your SHALA, you are eligible for two discounted $20 classes per quarter. You may choose from:

• Group fitness: cross training, indoor cycling, yoga, Pilates, tai chi, swimming, dance and more.

• Healthy living: nutrition and weight management, stress management, disease prevention and management, and more.

• Behavior change: coaching and counseling, weight management, smoking cessation and more.
