Your Free Application for Federal Student Aid (FAFSA) was selected by the U.S. Department of Education for a review process called verification. Federal law
states that before awarding Federal Student Aid, the Office of Student Financial Aid (OSFA) may ask you to confirm the information you and, if applicable,
your spouse, reported on your FAFSA. Federal regulations require that we complete verification before processing federal financial aid. You and your spouse
(if applicable) must complete and sign this worksheet, attach any required documents, and submit the form and other required documents to the OSFA.
After a review of the information you submit, the OSFA may still ask for additional information. If there are differences between information on your FAFSA
and this worksheet, we will send corrections to the FAFSA processor. You will receive an acknowledgement reflecting these changes. If we do make corrections,
please do not make any subsequent changes to your FAFSA data. We are unable to process your application for financial aid until we can determine that we
have accurate information.

on “Get transcript online”, then log in or create an account, select reason for transcript: Higher
education/student aid, in the Return Transcript box select 2016, and it will generate your Tax Return Transcript.
To obtain a Verification of Non-filer Letter from the IRS, go to www.IRS.gov and click on “Get your tax record,”
then click on “Get transcript online”, then log in or create an account, select reason for transcript: Higher
education/student aid, in the Return Transcript box select 2016, and it will generate the Non-filer Letter. Then
print and submit this letter.
