• Individuals with a disability who receive Medicaid and are:

» planning to live in community, or currently live in community and who receive support services from:

─ HOME Choice program (Money Follows the Person)

─ HCBS Medicaid waiver

─ Providers from ADAMH licensed providers

» part of a low-income household that earns 30% or less than Area Median Income (AMI)

» at least 18 years of age but less than 62 years of age

» Outcomes: accepted or denied

2. Application at 811 Property for Tenancy» Requirements at LIHTC properties and for HUD programs

─ Income and supporting documentation, criminal/credit/rental histories

» Outcomes: accepted, denied, waitlisted

• Begins with Referral to Ohio Department of Medicaid from “Case Manager” (CM)

» HOME Choice Transition Coordinator

» Behavioral Health Case Manager

» HCBS Waiver Case Manager

» Services and Support Administrator

• CM completes application and submits to ODM for screening

• Letter will be sent to applicant and case manager indicating 811 eligibility status

» If eligible for 811, the letter will give approval to apply at 811 properties

─ Letter will be physically taken to 811 properties in order to apply for 811 unit

» If denied for 811, the letter will explain the reason

─ Under or over age limit, not receiving services, etc.
