Online Certificates of Insurance User Guide
(Approval Required)
IntroductionWelcome to the SERVPRO® Franchisee Insurance Center’s user guide for requesting an “approval required” Certificate of Insurance (COI) online using our self-service website. This manual provides step-by-step instructions on how to request special COIs online 24/7.
What is the difference between a “standard” and an “approval required” COI?The majority of COIs that a Franchisee will issue are to show proof of insurance to a client or vendor. On occasion, a contract may require you to add an Additional Insured endorsement to your General Liability. These are considered “standard” COIs. Franchisees may issue standard COIs on our self-service website. To learn more, please refer to the Online Certificates of Insurance User Guide (Standard).
The website also allows you to request COIs that include special wording and endorsements that require underwriting approval. These are considered “approval required” COIs. Examples include:
• Pollution Liability Additional Insured• Completed Operations Additional Insured• Crawford Contractor Connection• Alacrity Services• Waiver of Subrogation• Notice of Cancellation• Per Project Endorsement
What happens after I request an “approval required” COI?After you request a special COI, it will be reviewed by Aon Affinity underwriting. If the underwriter has any questions, they will contact you. If not, they will issue the COI, and send copies to you and your client or vendor. You are responsible for the content so please make sure it is accurate before submission.

To access the SERVPRO® Franchisee Insurance Center, log into rrrg.servpronet.com and click on our banner ad on the homepage. On the Franchisee Insurance Center homepage, click on the “Receive Certificates of Insurance” link. To log in to the COI site, you will need your Entity Franchise email address. Your temporary password is your Entity Franchise number. (If you have trouble locating this information, please call 1-866-231-2006 for assistance.)

How to Request an “Approval Required” COI
1. From the SERVPRO® Franchisee Insurance Center home page, click on “Receive Certificates of Insurance.”
2. On the COI Login page, enter your email address, password and click “Log In.” 3. On your Home page, on the policy details line labeled “Approval Required,” select “Add.”

4. Clicking “Add” will take you to the COI Certificate Holder Interest page shown below.5. Enter the name and address of the COI interest holder.6. Select the COI delivery method (print, email or fax).7. If the interest requires General Liability Additional Insured status or the General Liability
Subrogation Waiver, check the appropriate box(es).8. When finished, click “Save” to continue.

9. After completing the Certificate Holder Interest information, you will be directed to the COI Preview/Issue page shown below. (Although the page displays “Issue,” please note that special COIs require underwriting approval and are issued by Aon Affinity.)
10. This is the page where you will include special instructions for your “approval required” COI. • Use the “Notes” section to send messages to the Aon Affinity underwriting team. For
example, if you wish to add Crawford Contractor Connection or Alacrity Services wording. • The “Addendums” section allows you to place a message in the Description of Operations/
Locations/Vehicles box on the COI. • The “Attached Endorsements” section is for adding special endorsements to the COI, such
as a Completed Operations Additional Insured endorsement on your General Liability. • All COIs are automatically renewable unless you select “Mark as Non-Renewable.”

• On the COI Preview/Issue page, if you select “Attach” it will bring you the Add Endorsements page shown below.
• Check the boxes of the endorsements you wish to add, and then select “Attach Endorsements” at the bottom of the screen.

11. On the COI Preview/Issue page, click on “Preview/Issue” to go to the COI Confirmation Page shown below.
12. Click “Preview” to review the COI for accuracy.

14. Selecting the PDF icon will open a new window displaying a preview of your new COI.

15. If the information on the COI is correct, clicking on “Return” will take you back to the COI Confirmation page. If the information is incorrect, return to the COI Certificate Holder Interest page to make changes.
16. To request your COI, click on “Request Certificate.” The COI will be issued to your client by Aon Affinity after underwriting review. They will also send you a copy for your records.
17. When you are finished, you log out in the upper right hand corner of your Home page.
