Description of Facilities

Pursuant to the Telecommunications Act of 1996, OHS intends to provide a combination of Facilities Based Competitive Local Exchange Carriers (CLEC) and Reseller of broadband services to residential and business consumers in the State o f West Virginia.

An illustrative tariff is provided with this application and is intended to give a description ofthe services being proposed by OHS.

Compliance with Commission Rules and Regulations

OHS certifies that it is willing to comply with all applicable rules and regulations as set by both the FCC and the West Virginia Public Service Commission.

Illustrative Tariffs

OHS has not yet negotiated an interconnection agreement with the Facilities Based Competitive Local xchange Carriers in the State of West Virginia and as such cannot provide an accurate estimate of the rate services it intends to provide.

Financial and Managerial Ability

Optimal Home Solutions, LLC is newly established telecommunications business that will be providing services in rural Monroe Co~nty West Virginia.

Also attached is a sheet containing brief biography of principal of OHS in the hopesof shedding light on the managerial capabilities ofthe company.

Technical Capability

As per the attached biography of owner, Mr. James Ridgeway will spearhead the company's local exchange operations. Mr. Ridgeway has been in the industry for over two decades now, providing public service for both state and federal governments agencies. Managed budgets and provided oversite to more than 18 facilities in the Mid- Atlantic Region.

Universal Telephone Service

OHS agrees to participate to the extent it may be required to do so by the Commission, in mechanisms providing for the supg~rt of universally available telephone service at affordable rates.

Wherefore, Optimal Home Solutions, LLC requests that its Application for a Certificate of Convenience and Necessity in the State of West Virginia be granted. OHS is confident that approval of such an application is in the interest of the State, as its entry into the local exchange market will foster competition and cause existing providers to provide more competitive rates and improved customer service. OHS's approval will guarantee not only a new option for consumers but will also contribute to the rural economic development and well- being of the State.

OHS as is cannot offer local exchange telecommunications service to the public unless and until it has filed for, and received Commission approvd of a tariff governing the terms, conditions, rates, charges and regulations pursuant to which it will provide service.

Owner - James R. Ridgeway is the founder of Optimal Home Sslutisns, LLC and has created networking s~luttons €or both the state md federal government. Mr. Ridgeway has worked closely with local government to improve network solutions in rural areas. He has prepared detailed Architect and Engineering (A&E) task orders, cost estimates (including manpower hours), developed and interpreted technical specifications involving complex equipment and the adaptation of such equipment to local conditions, Mi. Ridgeway has managed a regional workforce comprised o f civilian employees and engineering firms in compiling all necessary construction contracts and negotiations in order to complete projects, James has 20 yeas in the industry.

Accountant - Johnathan P. Rice is a Certified Public Account (CPA) and provides quality accounting, tax, auditing, and financial services to our company. Mr. Rice has 30 years of experience with tax planning for individual, corporate and partnership tax returns.

(local dial tone) or switched, interexchange services at this time.

However, Applicant has knowledge of and will comply with applicable Commission rules and

regulations, including but not limited to those addressing (i) provision of 9- 1 - 1 and Enhanced

9-1-1 services (including any necessary coordination with other carriers and the relevant public

safety agencies); (ii) compliance with the requirements of Section 25 1 (t) of the
