Our mission is to provide health services to improve the quality of life of diabetic individuals in Clare County. Our focus is to assist low-income residents by educating

them on the disease process and management of diabetes.

Objectives● To educate diabetic clients on identifying the signs and symptoms of

hypo/hyperglycemia on the first clinic visit● To familiarize the community about signs and symptoms of diabetes and the need for

treatment if these symptoms are present● To teach diabetic clients how to check their glucose level at home on the first clinic

visit● To educate diabetic clients on proper disposal of lancets and needles on the first clinic

visit● To educate the diabetic client on how to read food labels and identify sugar content,

sodium, fats, carbohydrate, fiber, total caloric intake on food labels and establish a target blood glucose range on second clinic visit

● To teach diabetic patient to inspect feet daily and to wear properly fitting foot attire at all times on second clinic visit

● To educate diabetic clients to exercise 30-60 minutes most days of the week in order to improve their overall health on third clinic visit

● To educate diabetic clients to have kidney function, cholesterol, eye exam and dental exam yearly and Hemoglobin A1C drawn twice a year on fourth visit

● To educate the diabetic client on identifying and reducing own risk factors for obesity, hyperlipidemia, hypertension, stroke, and cardiovascular disease on fourth clinic visit

● To educate the community on identifying and reducing their own risk factors for developing diabetes at the diabetes awareness health fair

secure funding for clinic expenses and health fairs as needed ❏ Works on grants to facilitate funding for the diabetic education program needs❏ Makes sure that employee salaries are accurate and paid out on time ❏ Handles all incoming and outgoing money and donations ❏ Makes payments for any rent, utilities, or other bills that may arise❏ Attempts to come up with ways to conserve finances while still being able to provide

appropriate care to the clients per organizational objectives

Community Scheduler❏ Answers incoming telephone calls, acts upon return telephone calls when needed, and makes

appointments for clients as desired❏ Assists with incoming referrals from outside agency❏ If schedule adjustments are essential for the diabetic nurse educator or client the community

scheduler, the community scheduler will execute this duty❏ Assembles materials sought after for the community health fairs in conjunction with the

volunteer. ❏ Establishes dates and times for community health fairs in the local area

Nutritionist❏ Holds classes regarding proper label analysis to attain the exact nutrition value of the food ❏ Explains and demonstrates what portion sizes are in regards to actual consumption amounts❏ Educates on the carbohydrate to sugar ratio for accurate administration of diabetic

medications for coverage of blood glucose levels ❏ Explains what groceries are indicated as low sugar, no sugar, or to only be eaten while

hypoglycemia is current❏ Clarifies what groceries and ingredients can lead to hyperglycemia ❏ Instructs diabetic clients with healthy diabetic friendly meals ❏ Answers meal and food related questions from the clients

Job DescriptionClare County Diabetic Education Foundation is currently seeking candidates for our Volunteer Services position to be a part of our Diabetic Education Team, located in Clare, MI. This foundation services diabetic clients throughout Clare County, Michigan.Do you want to improve the quality of life for diabetic individuals? Our purpose is to assist all individuals in prevention, education and management of diabetes regardless of economical and insurance status. Your important work will ensure diabetic clients receive the support, tools and education they need to successfully prevent or manage their chronic disease. You will be working with a team of health care professionals such as community education manager, diabetes nurse educator, nutritionist, and community scheduler.

Volunteer Job Requirements:Keep supplies and handouts filled in the office and at the community health fair booth.Assist the community scheduler with scheduling clients for appointments.Make copies of brochures, flyers, and other educational materials used by the diabetic nurse educator.Provide assistance to community education manager, diabetes nurse educator, nutritionist, and community scheduler.

Education:Ability to read, write, and communicate effectively in English.

Experience:No previous volunteer experience required, will train qualified candidate.

Diabetic supplies: cotton balls, lancets, glucose meters, band-aids, alcohol wipes, test strips, syringes, sharps disposal

containers, first aid kit, blood pressure machine, a scale, and an eye examination tool

Supplies to hold community events: show tents, tables, and chairs

Educational materials: papers for pamphlets, practice insulin supplies, diabetes education books, and sample food labels

Other Materials: lab draw slips from the community lab to have clients A1C, kidney function, and cholesterol checked

and sent to their primary care doctor

Each paid position is full time and will include health insurance, retirement, paid time off, and paid holidays. Benefits cost approximately $8.000 per employee per year.
