Target Points• Blanket Release statement between DHHR/WVDE; grants access to

necessary student assessments/records. 

• Creating a referral process within West Virginia and with Out-of-State placements to notify the Out-of-State Transition Specialist when a student is being ordered Out-of-State for treatment and when the student is returning.

• Developing the transition process for Out-of-State returns, through collaboration with Regional OIEP Transition Specialists, LEAs, Counselors, and DHHR Staff.

Capturing the Whole Student and Implications

• 265 Students are currently placed in PRTF and Group Residential Care (This totals includes students not verified WVDE, but tracked by DHHR).

• On average, students reside in placement for 287 days.M

For coordination to adequately address students displaced, these students need to be reviewed clinically and educationally:

• Access to psychological evaluations to review IQ and diagnosis/diagnoses

• Strengths, triggers, and coping skills

• Identified learning style
