 Q: What residency (click here to view district map) documents do I need to enroll my child in school?   A. The following MUST be provided by t he parent/guardian at the time of enrollment: Photo I.D. such as a driver’s license, State I.D., or passport of parent, step -  parent or legal guardian. Driver’s license or State I.D. must show current address in the district.  Three current proofs of residency (one from Column 1 and two from Column 2) in the name of the parent/step  parent or legal guardian is needed: COLUMN 1 COLUMN 2 Mortgage Document or Deed  Gas Bill  Current Property Tax Document  Electric Bill  Lease/apartment/home rental documents Telephone Bill (land line or cell phone) (all tenants and occupants names must be  Cable/Satellite Bill  on documents, and include beginning/ending Water Bill (only if name and address are noted) lease dates. Lease must be signed by bot h Landlord and tenant.) Closing papers (must have moved into house)   Accepted purchase agreement, signed by all parties  ** If a parent/guardian does not own or rent the home in which they are living, the enrollment application process must begin at our District Administration Office, 35 S . Each student enrolling in any school district must provide a birth certificate, social security card, current immunization record and proof of residency. Q: Where do I go to enroll my child?   A. Each building enrolls students into t he grade in which they will be at tending at that building. Q: During the s ummer, when are school building offices ope n?  A. Enrollment at the build ing levels begins on Monday, August 6, 2012. Most offices are open 8:00 a.m. —3:30 p.m.  Monday—Friday.  Q: When is the Schools of Choice option available?  A. The district submits Schools of Ch oice openings to the MAISD in early spri ng of each year. Schools of Choice applications are submitted beginning the second week of April and ends the second week of May of each year.  Applications have already been accepted for the 2012 -2013 school year. Q. Once a student attends the district as a Schools of Choice pupil do they have to reapply each year?   A. No, the pupil is stil l a non-resident but can attend as long as they are enrolled and attend continuously in the district. Q. Is out-of -district transportation available for Schools of Choice pupils?  A. Transportation is not provided.  Q: I am interested in o btaining my GED, what are the req uirements?  A. To obtain additional infor mation on earning a GED, contact OV Community Education at 231.760.1350.  Q: My high school class gr aduated and I still need cre dits to finish my diploma, who do I c all?   A. To obtain additional infor mation on High School Completion (H SC), contact OV Community Educati on at 231.760.1350.  Q: What age does my child have to be to start Pre School or Head Start?  A. Orchard View Schools offers 3-year old and 4 year old Pre-School Programs.
